Abstract

Introduction: Estimation of fetal weight is very important in preventing delivery complications and also in selecting delivery method. This is specially important in potential macrosomia, IUGR and also preterm labor. The objective of this study is to compare the accuracy of ultrasonic, clinical and mother, fetal weight estimation in pregnant women in Kerman. Materials and Methods: In this cross sectional study 246 women with singleton, term pregnancies admitted for cesarean section within the following week, were studied. Estimation of fetal weight (EFW) was done using sonography and then by two obstetricians and one senior resident who were unaware of the other clinical and ultrasonic results. Mothers' estimation of their baby weight was also asked. Statistical analysis was done by paired T and Chi square tests. We also used ROC (Radar Operating Characteristics) curve to compare the accuracy of different tests. Results: Fetal birth weight was 3339±443 gr. In the 2500-4000 gr birth weight the sensitivity of predicting clinical birth weight was 11.8% with 99.6% specificity and for sonographic EFW was 12.6% with 92.1% specificity. Conclusion: Clinical estimation of birth weight in term pregnancies is as accurate as routine ultrasonic estimation obtained in the preceding week of delivery.

Abstract

Abstract
Background: Ultrasonography (US) is the most important imaging modality in detecting both intrauterine and ectopic pregnancies. This study aimed to compare serum β-hCG level with Transvaginal Ultrasound in symptomatic patients with ectopic pregnancy.
Materials and Methods: A total of 210 patients with definite diagnosis of ectopic pregnancy were enrolled prospectively. All patients underwent transvaginal US by an experienced radiologist who was blind to the final status of the patient. Serum levels of β-hCG and the time gap between US examination and last menstrual period (LMP) were compared between patients with positive and negative US findings.
Results: US were positive in 194 patients (92.4%) and negative in the remaining 16 patients (7.6%). The two groups were similar in terms of age (median, 31 years vs. 30.5 years, respectively; p=0.57). Both the median time gap between US and LMP (median, 43 days vs. 34 days, p=0.03) and serum level of β-hCG (median, 1027 mIU/ml vs. 172.5 mIU/ml, p<0.001) were significantly higher in patients with positive US findings. Both differences were independent of each other in multivariate analysis. The best discriminative zone was set at a serum β-hCG level of 326 mIU/ml with a sensitivity and specificity of 86.6% and 87.5%, respectively.
Conclusion: According to our findings, both the median serum level of β-hCG and time gap between US and LMP in patients with missed diagnosis of ectopic pregnancy were significantly lower in comparison with correct diagnosis and proposed discriminative zone for serum level of β-hCG in our study is different from the previous studies though further studies are recommended especially in symptomatic patients with suspected ectopic pregnancy.

Abstract

Background and Aim This study aimed to investigate the demographic and clinical information of patients undergoing surgery with ultrasound in internal jugular vein cannulation.
Methods & Materials After the approval of the plan in the ethics committee of Guilan University of Medical Sciences, this study was performed in the general surgery department of Razi Hospital in 2018 on 100 patients in need of central venous catheter implantation. The neck type (distance between the two designated areas) was entered in a special form made by the researcher and after the completion of the design, it was examined and statistically analyzed.
Ethical Considerations This research ethically approved by the Ethics Committee of Guilan University of Medical Sciences (Code: IR.GUMS.REC.1396.20)
Results The mean age of the studied patients was 58.72±16.09 years. The anatomical and sonographic margins between the two Land Mark areas were higher in men VS women, but there was no statistically significant difference (P>0.05). However, there was no statistically significant difference between the other variables. (P>0.05).
Conclusion The findings of this study show that catheterization of the internal jugular vein under the guidance of ultrasound was less access time and fewer complications.

Abstract

Background: Imaging of the vascular system supplying the brain parenchyma is a diagnostic tool in the evaluation of stroke patients. This study aimed to compare the results of digital subtraction angiography with color Doppler ultrasound of the neck blood vessels in patients with ischemic stroke.
Methods: In this cross-sectional-analytical study, the medical records of patients with ischemic stroke admitted to Imam Khomeini Hospital from July 1401 to July 1402 were reviewed to collect data. Demographic information, along with the results of color Doppler ultrasound and digital subtraction angiography of the patients, were extracted and evaluated.
Results: This study included 48 patients with ischemic stroke with a mean age of 12.8 ± 0.67 years. Hypertension was present in 68.8%, diabetes mellitus in 20.8%, and smoking in 25% of patients. Based on the results of digital subtraction angiography and color Doppler ultrasound, the most common vascular abnormality was plaque (79.2%). Vascular abnormality and occlusion were present in 91.7% of patients. Color Doppler ultrasound in examining the right and left internal carotid arteries had a sensitivity of 94.9% and 92.8%, specificity of 66.7% and 38.9%, positive predictive value of 92.1% and 70.3%, and negative predictive value of 75% and 77.8%.
Conclusion: Color Doppler vascular ultrasound with high sensitivity and specificity was able to detect the presence and severity of occlusion in the internal carotid arteries in patients with ischemic stroke and can be used as a non-invasive and cheaper method in the clinical course of patients.
