Abstract

Introduction: One of the most important skills which should be learned and performed by physicians and nurses is the correct method of intubation. Therefore, more attention should be given to such trainings. The main purpose of the recent study is manufacturing and utilization of a detector device for correct intubation which can be used by inexpert staff who are not members of anesthesia team.
Materials and Methods: In a quasi experimental study a detector device was manufactured and utilized for correct intubation. In this research 400 patients undergone surgery, had been chosen for testing the device. Their ages varied from 5-75 years old and they had no chronic obstructive pulmonary disease (COPD), asthma and heart disease. These patients have been generally anesthetized. In all patients the tracheal tube has been placed in esophagus and then immediately in trachea, using the device. The sensitivity of device was evaluated. During the procedure patients, oxygenation was not disturbed.
Results: All 400 intubations were performed succesfuly using the device. In this study using air flow out of lungs, intubation was done successfuly. So the devices, sensitivity was 100%. By a professional the mean time for intubation using this device was 5 seconds, and it,s price was 1000 Rials.
Conclusion: Regarding the results, it seems the device is useful in emergencies and facilitates intubation. This device is cheap and portable and seems to be more useful than other reported devices in determining correctiveness of intratracheal intubation.

Abstract

Background: Radiotherapy is one of the cancer treatment methods. Prescribed dose for each fraction is considered based on radiosensitivity of tumoral and normal tissues. Viral agents are the effiectiv factors on tissue sensitivity. This research aimed to determine the effect of ionizing radiation of Cobal 60 on radiosensitivity of Hela cells infected with Measles virus. Methods and Materials: In this study, the radiosensitivity of Hela cells is investigated experimentally and qualitively. The cells have been cultivated in two groups (experimental and blank) and plating efficiency has been obtained. Then 100λ measles virus with serial dilution method was used to induce infection in different ratio for experimental group. After cell growth and passage, the two groups were irradiated with 2Gy gamma radiation of cobalt 60. Results: Results respectively indicated cell death increases up to 5-7%, 15-20% and 50-65%, after 2Gy irradiation by Co 60 for contaminating to Measles in low, moderate and high concentrations. Conclusion: Radiosensitivity of tumoral cells increases when they are infected by viral agent. The result in radiotherapy of cancers showed, in prescribing dose fraction non cancer disease should be considered.

Abstract

Background: Diagnosis of acute appendicitis is one of the challenges that surgeons face in the emergency ward. Although Alvarado score has been recommended in the diagnosis of acute appendicitis, this criterion has some limitations in terms of age, sex, and race. The aim of this study was to evaluate the effectiveness of Alvarado score in the diagnosis of acute appendicitis based on the age and gender. Materials and Methods: In this cross-sectional study, 407 patients who underwent operation with the preoperative diagnosis of acute appendicitis from June 2008 to August 2009 were included in the study. The patients’ data were recorded by the investigators. The data were compared by parametric tests. The sensitivity and specificity were determined by the optimal cut-off point on ROC curve using MedCalc software version 10.2.0.0. Results: The optimal cut-off point for women was lower than men (6 and 7, respectively). The rate of negative appendectomy in the group with Alvarado scores 9 and 10 was 11.8%, while the negative appendectomy rate in the other two groups with Alvarado scores 7 and 8 and 5 and 6 were 29.9% and 52.8%, respectively. Conclusion: The diagnostic value of Alvarado score of 8 and less in physical examination is lower than the abdominal tenderness exam alone. Also, the cut-off point for the diagnosis of acute appendicitis in women should be considered lower than that in men. On the other hand, the cut-off point in adolescents and children is higher than adults.

Abstract

Background: Brucellosis is a zoonotic disease in humans and animals that its control is dependent on the prevalence of diseases in animal populations. The purpose of this study was to compare the sensitivity and specificity of common diagnostic tests for brucellosis. Materials and Methods: In an analytical cross-sectional study, 297 serum samples from an infected herd that had a history of abortion were prepared and evaluated through standard tube agglutination, Rose Bengal, 2-Mercaptoethanol, complement fixation, and ELISA tests. In addition, microbial culture was done for positive serum samples, lymph nodes, and infected organs. Results: Based on the total 297 serum samples tested, the sensitivities for standard tube agglutination, Rose Bengal, 2-Mercaptoethanol, complement fixation, and ELISA tests were 89, 81.5, 75.3, 89.7, and 93.2% , whereas specificities were determined to be 97.4, 94, 96, 98, and 99.3%, respectively. Conclusion: The findings of this study show that the standard tube agglutination test, complement fixation test, and ELISA are more sensitive than other tests. Moreover, ELISA test was the most sensitive and specific serological test for diagnosis of brucellosis in the present study

Abstract

Background: Brucellosis is one of the most common zoonotic diseases in Iran. In most cases, the diagnosis of brucellosis is difficult not only because of its clinical similarity to many infectious and noninfectious diseases, but also because diagnostic methods often fail to detect organisms. PCR is a rapid and safe diagnostic method applied to the diagnosis of brucellosis. The purpose of this study was to determine the sensitivity and specificity of PCR for diagnosis of human brucellosis by using serum samples. Materials and Methods: This study which was done to evaluate diagnostic tests included30 serum samples from patients with clinical presentation of brucellosis with positive Wright test and serum samples of30 healthy people with negative Wright test. These samples were examined by PCR. Results: PCR results were positive for 15 samples of the patients group in comparison with 4 samples from the 30 healthy subjects. The sensitivity and specificity of PCR were 50% and 86.6%, respectively. Conclusion: Although in some studies, the preferred sample for diagnosis of brucellosis was serum, in this study, PCR on serum samples did not indicate high sensitivity and specificity in diagnosis of brucellosis. Hence, using a combination of methods for diagnosis of human brucellosisis suggested.

Abstract

Background: The aim of this study was to determine the prognosis of visual acuity and contrast sensitivity in keratoconus patients according to the correlation between aberrations changes before and after RGP lenses.

Materials and Methods: A total of 40 eyes of twenty patients 20 to 40 years with mild, moderate keratoconus were participated in this study. Ocular examinations were included VA with standard Snellen chart, CS with CSV 1000 chart and HOAs with technolas aberrometer objective (Bush & Luamb). All these tests were performed before and after RGP lenses. Refraction and corneal radius of curvature were measured using Autokeratometr. Data were then analyzed with Spss softwair version 11.5.

Results: Mean contrast sensitivity at each spatial frequency with RGP lenses was higher than the best correction in both male and female. Compairing with best correction mean high order aberration of the 5 and 6 mm pupil was reduced with RGP lenses but the effect was not statistically significant different. Mean high order aberration with pupil 6 mm was more than pupil 5 mm. Using Pearson correlation test, there was no significant correlation between visual change and Contrast sensitivity changes with high order aberrations.

Conclusion: Visual performances and visual acuity were improved with RGP lens than best correction by reduction of high-order aberrations. However, there was no correlation or relationship between high order aberrations and visual performance. This indicates that Aberrometery condition was unrelated with Prediction of visual acuity changes in this patients.

Abstract

Background: The efflux pumps are one of the main mechanisms of resistance to antibiotics in E. coli and K. pneumoniae strains. The aim of this study is to study the association between genes coding efflux pumps AcrAB and Qep A and ciprofloxacin resistance among E. coli and K. pneumoniae clinical strains

Materials and Methods: This study was done on 100 strains of E. coli isolated from Taleghani and Labbafinejad Hospitals and 100 strains of K. pneumoniae isolated from Taleghani and Mofid Children Hospitals. Antimicrobial susceptibility tests were performed by disk diffusion method based on CLSI guidelines. Identification of genes encoding efflux pumps Acr AB and Qep A was done by PCR technique.

Results: In this study, fosfomycin and imipenem had the best effect against E. coli clinical isolates and fosfomycin and tigecycline had the best effect against K. pneumoniae clinical isolates. PCR assay demonstrated that the prevalence of AcrA, AcrB and QepA genes among E. coli isolates were 92%, 84% and 0%, respectively and among K. pneumoniae isolates were 94%, 87% and 4%, respectively.

Conclusion: The prevalence of genes encoding efflux pumps in E. coli and K. pneumoniae clinical strains, which causes resistance to fluoroquinolones, is cause for concern. Therefore, controlling infection and preventing the spread of drug-resistant bacteria needs to manage medication carefully and identify resistant isolates.

Abstract

Abstract

Background: The purpose of this study was to compare the cognitive emotional regulation strategies, sensory processing sensitivity and anxiety sensitivity in patients with multiple sclerosis and normal people.

Materials and Methods: Statistical population of this study was all of patients with multiple sclerosis that referred to M.S association of Iran in the Tehran. Sample of this study was 30 individuals of patients with multiple sclerosis selected by available sampling method and were matched with 30 individuals of normal people. Two groups completed cognitive emotion regulation, high sensory processing            sensitivity and anxiety sensitivity questionnaires. Data were analyzed by one-way analysis of variance and Multivariate Analysis of Variance.

Results:  The results indicated that there is significant difference between two groups in view of cognitive emotion regulation strategies in which the mean of scores of patients with multiple sclerosis in maladaptive strategies of self- blame, catastrophizing and other blame were more than normal people and mean of scores of them in adaptive strategies of positive refocusing, positive reappraisal and putting into perspective were less than normal people. The results also indicated that there is a significant difference between two groups in anxiety sensitivity and sensory processing sensitivity.

Conclusion:  The most of emotional problems in patients with multiple sclerosis can be the result of more application of maladaptive strategies of cognitive emotion regulation, high sensory processing sensitivity and high anxiety sensitivity.

Abstract

Background and Aim American College Of Emergency Medicine (ACEM) guideline has a recommendation for early diagnosis of head injuries following mild trauma. In this study we examined the prediction power, sensitivity, and specificity of this clinical guideline in the need for computed tomography (CT) scan 
Methods & Materials This cross-sectional study was performed for 6 months on patients over 18 years old referred to the emergency department of Vali-e-Asr Hospital in Arak who met ACEM criteria for head CT scan for suspected mild trauma. Demographic characteristics, clinical symptoms, trauma mechanism, physical injuries caused by head trauma, and history of drug abuse were recorded. The consciousness level (Glasgow Coma Scale) was checked every two hours. Patients underwent treatment if there was a pathology in CT images, and those with no clear pathology were discharged after 6 hours and, followed up by phone for two weeks, and in case of any abnormality in the level of consciousness, they were re-examined by CT scanning.
Ethical Considerations This study has an ethical approval obtained from Arak University of Medical sciences (code: IR.ARAKMU.REC.1396.227).
Results 500 patients, 335 male (67%) and 165 females (33%) with the mean age of 46.39± 2.01 years were studied; the sensitivity the ACEM guideline for predicting the need for CT scan in patients with mild head trauma were 100% with a specificity of 3.46% (for the second recommendation, the sensitivity was 100% with a specificity of 6.7%) which indicated that the test was highly sensitive to diagnosing the patients, but its specificity was low.
Conclusion The ACEM guideline had high sensitivity to predicting the need for CT in patients with mild head trauma, but had very low specificity which makes it an unacceptable criterion for rejecting or performing CT scan in these patients.

Abstract

Introduction: The present study was conducted with the aim of the effectiveness of philial therapy on social acceptability and anxiety sensitivity of children with symptoms of self-destructive mood disorder.
Methods: It was a semi-experimental study with a pre-test-post-test design with a control group and a follow-up period. The statistical population included 9-12-year-old children with symptoms of suicidal mood disorder in Isfahan city. 30 children with symptoms of self-inflicted mood disorder and their mothers were selected by purposive sampling and randomly assigned to experimental and control groups. The experimental group received Filial Landreth play therapy intervention (2013) during two months in 8 sessions of 60 minutes. Research tools included Ford and Robin's (1970) social acceptability questionnaire and Tsao et al.'s (2006) anxiety sensitivity questionnaire. The data obtained from the research was analyzed by the method of variance analysis with repeated measurements using SPSS-29 statistical software.
Results: The results showed that the filial therapy intervention had a significant effect on the social acceptability and anxiety sensitivity of children with symptoms of self-destructive mood disorder (P<0.001). In this way, this intervention was able to lead to the improvement of social acceptability and the reduction of anxiety sensitivity of these children.
Conclusions: Based on the findings, it can be concluded that philial therapy using techniques such as lectures, discussion, role playing, group processes and supervision can be an effective treatment to improve social acceptability and reduce anxiety sensitivity of children with the symptoms of self-inflicted mood disorder should be used.
