Abstract

Background: Oetection of congenital hypothyroidism (CH) in Arak has begun since 2006. The aim of this study was to investigate on recall rate (RR) and the incidence of this disease in Arak. Materials and Methods: In this descriptive study, according to the national screening program for CH in Iran, heel blood samples of 3-5 day- old neonates were taken on special filter paper and TSH values were measuned using ELISA method. TSH values bigger than 5mu/lit were recalled and thyroid function tests (TSH, T4 and T3RUP) were done on venous blood samples and assayed using ELISA method. Neonates with TSH values >10 mu/lit, T4<6.5 ug/dl were viewed as patients. Data related to neonates born up to the end of March 2009 were recorded in special forms. Results: Of 25685 screened neonates, 608 newborns were recalled. RR was estimated to be 2.36%. TSH values in 3.78% were less than 5, in 83.55% between 5-9.9, in 7.56% between 10-19.9 and in 5.09% more than or equal to 20. Eighty six neonates had CH (1/300 live births). Disease rate in recalled neonates was 14.2%. In 80.26 percent of neonates, sampling was done between 3-5 days while the times of sampling in 18.5 and 1.1 percents of the newborns were in days 6-21 and after day 22, respectively. Conclusion: The incidence of CH in Arak is remarkably higher than national and international rates. RR in Arak is higher than the national but lower than the international rates. Also, the ratio of the disease to the recalled neonates is low.

Abstract

Background: Genu varum is a deformity marked by outward bowing of the leg which is accompanied with pain only in severe cases. Failure in timely diagnosis results in the loss of quality of life in future and high health care costs. The purpose of this study was to evaluate genu varum prevalence in male students aged 7-16 years and identify the cases in need of treatment. Materials and Methods: In this cross-sectional study, femoral intercondylar distance (ICD) of 3692 male students aged 7-16 years who were chosen randomly in Arak city was measured. ICD>0.5 cm was considered as genu varum and to determine whether cases need treatment, the MT-MF angle and WBL% were used. Results: The results indicated that the prevalence of genu varum was 2.53% in primary school, 6.98% in junior high school, and 16.33% in senior high school students. The findings showed a significant relationship between the prevalence of genu varum and age, BMI, and the history of lower limb trauma with or without fracture. Overall, 33% of the students required treatment. Conclusion: Considering the importance of having healthy knees, clinical examination and regular screening programs for identifying cases with mild genu varum is recommended. The importance of implementation of regular screening programs is confirmed by the increasing prevalence of diseases associated with age and the high percentage of patients requiring treatment.

Abstract

Background: Noticing the particular importance of timely detection of scoliosis due to its progression over the time and associated complications such as non-symmetric appearance, movement restrictions, and sometimes disturbance in cardiac or respiratory function, this screening study was designed to investigate the prevalence of scoliosis in pupils aged 9-16 in Arak.

Materials and Methods: In this cross-sectional study, 984 students aged 9-16 years in Arak, Iran, who were randomly selected by clustering method, were screened for scoliosis by Adams test and suspected pupils were examined by orthopedics and PA X-ray was applied for them. For diagnosis confirmation, the Cobb angle was calculated and angle>6 was considered as scoliosis.

Results: Based on the results, the prevalence of scoliosis was 1.52% and 1.21% in female students and 3.21% in male students. The Cobb angle of about 67% of the cases was less than 10 which indicated minor scoliosis.

Conclusion: The results of the present study necessitate school-based scoliosis screening so that early detection of the disease allows for implementation of a suitable treatment program which can prevent the development of the disease and the financial burden of surgery and its complications.

Abstract

background: Early detection and treatment are crucial to prevent mental retardation in congenital Hypothyroidism (CH). The aim of this study was to evaluating of screening program of CH in Markazi Province.

Materials and Methods: In cross sectional descriptive study we used recorded data in Markazi province health center. From 2006 to 2012,127112 newborns were screened by measurement of serum TSH level by heel prick. Neonates who had blood TSH&ge5MIU/L were recalled for more evaluation. Neonates with confirmed hypothyroidism underwent treatment.

Results: From127112 screened neonates, 414 were diagnosed as CH patients (both permanent and transient). The prevalence of CH was . Recall rate was 4.8%. In 94.4% of patients, treatment were begun before the 40th day of life. The coverage percent in the province was 100% from the second year of the program.

Conclusion: Recall rate and the incidence of CH were higher than those in other studies, both in Iran and other countries. The mean age of treatment initiation and coverage percent were in favorable range.

Abstract

Abstract
Background: Early diagnosis of prenatal illnesses and timely treatment of congenital anomalies has been the mainstay of the health system. In this study, our aim is to provide Roc curve multivariate modeling in detection of fetal abnormalities using associated markers in screening Down syndrome in the first and second periods of pregnancy.
Materials and Methods: This is a descriptive-analytical study that uses information from two sets of data. In the first set, 152 individuals, who had the results of the first- trimester and second screening tests at risk and in the second group, 75 individuals with normal results. The studied variables included the serum markers in the first- trimester and the second- trimester screening, auxiliary variables (includes demographic information). Statistical analysis was performed by using ROC regression, incremental value analysis and Stata 12 software.
Results: In evaluating the value of each diagnostic test in the presence of auxiliary variables using logistic regression and rock curves, the results generally showed that in screening the first- trimester of PAPP-A and in the screening the second-trimester,Inhibin-A can be used alone as a diagnostic test.
Conclusion: Best diagnostic test in the first- trimester, respectively, PPAP-A, NT, FREE B-HCG and in the second- trimester of screening, respectively, Inhibin-A, HGG, UE3 and AFP were based on the area under the ROC curve. In addition, the most significant effect of the predictor variable on the outcome of the diagnostic test was family history.

Abstract

Background and Aim: Human papillomavirus (HPV) infection is a prevalent, life-threatening disease and cause of cancer among women. Therefore, in recent years, developing novel anti-HPV agents is highly regarded. The study was planned to bioinformatic screening for E1 and E2 potential inhibitors of HPV serotypes including 16,18,31,33 and 45 types from medicinal plants.
Materials and Methods: This is a descriptive-analytic study. In the first step, three-dimension structure of phytochemicals were retrieved from PubChem database and then the cell cytotoxicity and mutagenesis potential of them were evaluated. In the next step, the amino acid sequences of two key proteins of mentioned types of HPV including E1 and E2 were obtained from Uniprot database. Furthermore, the conserved and variable regions of the protein sequences were predicted using multiple sequence alignment method. Finally, the three-dimension structure of mentioned proteins was determined by homology modeling method and potential interactions of the phytochemicals with the proteins were investigated using molecular docking method through Autodock 4.2.6 software.
Findings: The results demonstrated that ursolic acid has no cytotoxicity and mutagenesis potential with appropriate physicochemical properties. Results also showed that mentioned compound had strong interaction with both E1 and E2 of all studied serotypes. Furthermore, the evaluation of ursolic acid and E1 and E2 interactions showed that amino acid is involved in conserved regions of mentioned serotypes.
Conclusion: Based on the obtained results of present study ursolic acid could be good candidate for more in vitro and in vivo studies of its anti HPV activity.
