Abstract
Introduction: Patient hospitalization rate (P.H.R) is an important issue in health decision making because its increasing can imposes abundant expenditure on each patient or therapeutic or health systems like assurance companies. Moreover, it can increase the danger of nosocomial infections. So, description, examinating and modelling of P.H.R. can be as a guideline for above subjects and as an introduction would lead one to study the issue with insight.
Materials and Methods: For determining of P.H.R., Iwo samples of size 1428 and 816 of patients were chosen systematic randomly from Vali-Asr and Ta'min-Ejtemai hospitals, respectively during the year of 2002. Hospitalization rate, age, sex, location of birth place and residence place of patients recorded. Mixed distributions modelling and Poisson regression models as well as ordinary regression methods were applied to analyze and modelling the data.
Results: Means of P.H.R of patients at Vali-Asr hospital was 4.1±5.4 days. Two component Poisson was a relatively appropriate model was a very relevant model for analyzing of their P.H.R. By using mixed Poisson regression method a significant relationship among patient's age, marriage status and location of life with P.H.R was obtained. Findings in Ta'min-Ejtemai showed that the mean of P.H.R was 4.7±3.2 days. One component Poisson model was appropriate model for data distribution. A significant relationship among age, sex, location of life with P.H.R in this hospital was seen.
Conclusion: Our findings showed that P.H.R in Vali-Asr hospital follows two component Poisson model. For Vali-Asr data, the normal model was not appropriate even with changing the statistics variables, but for P.H.R data of Ta'min-Ejtemai, changing the logarithmic variable could transform data into approximate of normal model. Finally, comparison between mixed distribution Poisson model and acquired regression with simple linear regression in Val-Asr hospital presented different results, but in Ta'min-Ejtemai hospital had similar results.

Abstract
Introduction: Sensory stimulation of comatos patients is an important nursing intervention for increasing their LOC and tactile stimulation especially massage is one of the most important stimulations. The nurse as a person, who is responsible for this, must know the effects of this intervention on patients. But there are some conflicts about this intervention's effects especially on vital signs (pulse, blood pressure and respiration). This study was done to determine the effect of massage on comatos patient's vital signs who were hospitalized in intensive care unit. Materials and Methods: This study is a clinical trial. Samples were consisted of 100 comatos patients, divided into two equal groups randomly. The two groups were matched in age, geneder and glascow coma scale. Samples in experimental group received 20 minutes massage on their back, feet and hands in three continuous days. Vital signs in experimental group were measured before and immediately after intervention and vital signs in control group were measured at the same time with experimental group. A check list used was used for collecting data. Data was analyzed using descriptive and analytic statistics (T, paired T and Chi-square tests). Results: Findings showed that massage decreased vital signs significantly as followed: pulse rate and diastolic blood pressure in each three days (p<0.05), systolic blood pressure in first and third days (p<0.05), respiration rate in second day (p<0.05). Conclusion: This study showed that massage can decrease comatos patients' vital signs which can be the indicator of their relaxation and decreasing stress.

Abstract
Introduction: While elderly people enter the hospital in biological crisis, family caregivers enter the hospital in psychological crisis. But professional caregivers haven’t adequate knowledge of family caregivers' experiences. Therefore they forget that family caregivers are in suffering equally. Since suffering is a significant concept in nursing science and knowing and comprehending sources of suffering in family caregivers by professional caregivers is very important, this study is done to describe the nature and structure of sources of suffering in family caregivers of hospitalized elderly patients. Materials and Methods: This is a qualitative study with phenomenological method. Population of the research is all elderly patients' families that were hospitalized in one of the hospitals of Isfahan University of medical sciences. Sampling method was purposive and continued until data saturation. The numbers of participants are 12. Data were gathered through in-dept interview and data analysis done by “Parse” method that was not used in Iran. Results: The finding of this research is summarized in these core concepts: Patients and therapeutic interventions as source of suffering, hospital as source of suffering, disturbance in family and social life as source of suffering, self neglect as source of suffering. These concepts will be discussed in the full article. Conclusion: Participants' experiences reflect the need for planning interventions such as developing supportive groups, providing facility and instruments and continuous support for family caregivers during hospital stay. Also results show that educational programs are necessary for effective adaptation in family caregivers.

Abstract
Background: From an administrational point of view, patient satisfaction with healthcare, diagnosis, and rehabilitation services is highly important. The present study was done to evaluate the satisfaction of patients referring to the emergency departments of hospitals in Arak.
Materials and Methods: This study was a descriptive-analytic, cross-sectional one which was carried out using a questionnaire. The collected data were analyzed by SPSS software and the related tests.
Results: Overall, over the first period, 72.9% of the patients were satisfied with the services, whereas this level of satisfaction reached 73.3% in the second period. By comparison, levels of satisfaction with the physical environment (86.2%), health condition (85.9%), speed and accuracy of care provider (69.6%), the quality of information provision (23.1%), behavior of guards and staff (60.2%), facilities (60.1%), drug availability (66.4%), discharge and costs (51.6%), and consideration of religious issues (86.6%) in the emergency departments over the first period reached 87.4%, 84.1%, 69.7%, 25.6%, 61.7%,67.9%, 50.9%, and 88.1%, respectively over the second period.
Conclusion: The findings of this study showed that patients’ satisfaction with emergency departments’ services was reasonable. The greatest dissatisfaction was related to costs, drugs, procedural problems, method of treatment, facilities, wait time, and insufficient information provision.

Abstract
Background: Idiopathic nephritic syndrome (INS) is one of the chronic illnesses in children. Therefore, clinical and laboratory factors that influence the clinical course of this disease are very important. Among these factors, low birthweight due to reduction of the number of congenital nephrons and subsequent glomerular hyperfiltration has been postulated as an important factor. This study was conducted to evaluate the clinical course of INS in patients with lowbirth weight in comparison to patients with normal birth weight.
Materials and Methods: In this descriptive-analytic study, 170 children with INS were selected from hospitals in Kermanshah, Urmia, Hamedan, Qazvin, and Ahvaz and divided into two groups of low birth weight (<2500gr) and normal birth weight (>2500gr). Then, the clinical course of these patients was evaluated by reviewing their medical records.
Results: Of the 170 patients, 22 had low birth weight and 148 had normal birthweight. Statistical analysis revealed no correlation between low birth weight and INS course except for age of presentation (P=0.014).
Conclusion: According to the results of this study, there is not any correlation between body birth weight and the clinical course of INS except for age of presentation.

Abstract
Background: Occult hepatitis C virus (HCV) infection is defined as the presence of HCV-RNA in liver or peripheral blood mononuclear cells (PBMCs) in the absence of detectable hepatitis C antibody (anti-HCV) or HCV-RNA in the serum. Although occult HCV infection is less aggressive than classic chronic HCV infection, nevertheless it is important in management of hemodialysis (HD) patients. Occult HCV infection has been described in two different clinical settings: in patients with normal liver enzymes or in patients with abnormal values of liver enzymes. The aim of this study was to detect the occult HCV infection in hemodialysis patients with elevated liver enzymes.
Materials and Methods: This cross sectional study was performed in hemodialysis patients referring to 3 dialysis units in Tehran. In 30 anti-HCV negative HD patients with elevated liver enzymes, presence of HCV-RNA in plasma samples was tested by Reverse Transcriptase-Nested Polymerase Chain Reaction (RT-nested PCR). In cases with negative anti-HCV and plasma HCV-RNA, HCV-RNA was checked in PBMC specimens by RT-nested PCR.
Results: A total of 30 HD patients with mean age 54.4± 14.1years and mean dialysis duration 81.2 ±64.4 months were enrolled in the study. All HD subjects were negative for HCV-RNA in plasma and occult HCV infection with RT-nested PCR method.
Conclusion: Occult HCV infection was rare in HD patients with elevated levels of liver enzymes.

Abstract
Background: There is a relationship between diabetes, dialysis and oxidative stress. The aim of this study was the comparison of structural changes in Hb, oxidative damages in plasma proteins, and antioxidant capacity in diabetic hemodialysis patients with those of control subjects.
Materials and Methods: In this experimental study, blood samples obtained from diabetic hemodialysis patients and control group. Oxidative damages in plasma proteins were determined by carbonyl assay and antioxidant power of plasma was performed by FRAP assay. Conformational changes in Hb were detected by spectrophotometric analysis. Blood glucose, urea, creatinine and uric acid in patients were determined usnig routine laboratory methods. Statistical analysis were performed by using regression analysis and t-test in SPSS20 software.
Results: This study showed a significant correlation between carbonyl content of plasma proteins and optical density of Hb at 630 and 275 nm which corresponds to structural changes in Hb. ferric reducing ability of plasma (FRAP), as an index of total antioxidant capacity of plasma was found to be enhanced significantly in diabetic patients receiving hemodialysis (from 1019.62±129 to 1354.54±129 molare, p<0.05).
Conclusion: The results obtained in the present study showed that inducible factors in diabetic hemodialysis patients contribute to plasma antioxidant activity and probably responsible for prevention of carbonyl formation and oxidative damages in hemoglobin.

Abstract
Background: Migraine is a chronic neurological disorder that leads patients to avoid any kind of activities. Since different factors are involved in migraine incidence and its triggers, so drugs are used to prevent or treat it are so variable. Also, combined medications are used to relieve migraine. This study examined the effectiveness of self-control training on quality of life in patients with migraine.
Materials and Methods: Statistic population of this study included all migraine patients in Ardabil in 2014(Estimation: N=1150) that 40 patients were selected by convenience sampling. Demographic and disease information questionnaire and quality of life questionnaire (SF-36) were used to collect data in clinical centers. Multivariate analysis of variance (MANCOVA) was used to analyze data, because present research was a experimental and clinical trial with pre-test and post-test with control group.
Results: The results showed that there is a significant difference between mean in quality of life in migraine patients and control subjects. It means that physical health and mental health of quality of life was different between control and experimental groups after self- control training.
Conclusion: Self-control training can be used to enhance quality of life in migraine patients. These results have important and effective applications in the treatment of migraine patients. Generally, specialists of clinical centers can use this method alongside other treatment interventions.

Abstract
Background: The purpose of the present research was to compare the perceived stress, self-efficacy and mental health in patients with HIV and Hepatitis C.
Materials and Methods: The method of present research was causal-comparative research. Population included of all patients with acquired immune deficiency syndrome or AIDS referred to a behavioral disease counseling centers of Arak city and all patients of chronic hepatitis caused by the hepatitis C virus referred to Gastroenterology Research Center and private practice of digestive specialists in Arak city. The sample consisted of 200 people, including 100 HIV and 100 people with hepatitis C who participated in the study and were selected for screening. All participants responded to the three instruments including Cohen Perceived Stress, Kamark and Mrimlstyn, Sherer's Self-efficacy and Goldberg general health questionnaires. Data analysis was done in two descriptive and inferential levels using SPSS version 20.
Results: The results of MONOVA showed that two groups were different in the perceived stress, self-efficacy and mental health, physical symptoms, depression, anxiety, social dysfunction, at the level of 0.01.
Conclusion: It is suggested that workshops to be held to careful inform how much stress and self-efficacy had affected on physical and mental health of patients with acquired immune deficiency syndrome or AIDS.

Abstract
Abstract
Background: Multiple sclerosis (MS) is a myelinating inflammation and an impairment of neuronal cell regeneration in the central nervous system with unknown causes. The aim of this study was to evaluate serum melatonin levels in the treatment of patients with multiple sclerosis.
Materials and Methods: A prospective study and a Cohort were conducted on 40 MS patients with multiple sclerosis patients referring to instituate MS in Arak city for one year. In this study, patients who were treated with interferon-moderating drugs and who had not been treated for more than one year were randomly selected. A questionnaire containing demographic information and clinical information and separation of interacting factors was completed and completed. Clinical information included MRI, VEP, EDSS (Expanded Disability Status Scale) score, and clinical symptoms. The patients also received peripheral blood samplers 3 times in 6 months intervals to measure melatonin and their melatonin levels, and then the relationship between the data obtained from the patient's clinical data, EDSS, serum melatonin levels using the software SPSS with ANOVA and Post-hoc statistical analyzed.
Results: In patients with MS, in spite of receiving drug treatment, the level of melatonin as an antioxidant decreased in second order sampling, which is consistent with the deterioration of the patient's clinical condition (EDSS test), and this is not observed in the control group.
Conclusion: According to the findings of this study, it can be argued that the level of melatonin is a measurable objective and measurable variable (with the help of ELISA) that can be used in the follow-up of these patients, along with clinical evaluations and questionnaires.

Abstract
Background and Aim: There is increasing evidence that cancer cells in addition to multiple genetic mutations, also acquire epigenetic abnormalities during development, maintenance, and progression. By utilizing the reprogramming technology as a tool to introduce the ‘pressure’ to alter epigenetic regulations, we might be able to clarify the epigenetic behavior that is unique to cancer cells. So far, iPSCs have been generated from normal primary cells, but it is unclear whether human primary cancer cell can be reprogrammed. We investigated the production of the iPS cells from the pancreatic adenocarcinoma cells using defined transcription factors.
Materials and Methods: We sought to reprogram patient derived xenograft from human PDAC, by introducing lentiviral mediated induction of Yamanaka Factors (OSKM) and characterized of induced cells by Alkaline Phosphatase staining, Real-Time PCR and immunostaining.
Ethical Considerations: This study with research ethics code EC/93/1025 has been approved by research ethics committee at Royan Institute.
Findings: Alkaline Phosphatase staining, Real-Time PCR and immunostaining showed that induction with the OSKM results in generating iPS cell line from fibroblast cells but not from PDAC PDX cells .We showed that, PDAC cells could not fully reprogrammed by the expression of 4 transcription factors.
Conclusion: This study demonstrated that the PDAC-PDX cancer cells were distinct from PDAC induced cells with regard to their epigenetic modifier genes expression pattern, although the expression of pluripotency genes did not increased significantly in the induced PDAC cells.

Abstract
Background and Aim: Coronary Artery Disease (CAD) is one of the leading causes of death and mortality in today's societies. Physical activity increases some of the influential factors for this disease. The purpose of this study was to investigate the effect of 8 weeks of aerobic and resistance training on endostatin in patients with Coronary Artery Bypass Graft (CABG).
Methods & Materials: The study participants were 24 male patients who were randomly divided into the experimental (n=12) and control (n=12) groups with Mean±SD age of 55.37±6.90 years, weight 75.45±5.87 kg, height 173.27±3.36 cm and body mass index of 25.11±1.55 kg/m2. The experimental group performed 8 weeks of aerobic and resistance training (3 sessions per week and 1.5 hours per session) based on the measurements, while the control group did not exercise during this period. To measure endostatin concentrations, blood samples were taken 48 hours before and 48 hours after the last training session while all subjects were fasting. Data were analyzed using the Shapiro-Wilk test to normalize the data and Student t-test in independent and correlated groups at the significant level of 0.05.
Ethical Considerations: This article was ethically approved by Azad University of Neyshabur (Ethics Code IR.IAU.NEYSHABUR.REC.1398.018) and with the Clinical Trial Code IRCT20191228045916N1 in the Iranian Registry of Clinical Trials.
Results: The present study showed that the aerobic and resistance training group significantly decreased endostatin concentrations (P=0.001) (t=1.672) compared with the control group.
Conclusion: Based on the findings, aerobic and resistance training decreases endostatin concentrations, known as an anti-angiogenic factor.

Abstract
Introduction: One of the clinical manifestations of coronavirus is neurological symptoms. However, due to the emergence of this virus, little information is available about its symptoms. Therefore, this study aimed to evaluate the prevalence of neurological symptoms in patients with coronavirus at hospitals in Arak.
Methods: In this study, all patients hospitalized with COVID-19 in two hospitals in Arak who were willing to participate were selected by census. The questionnaires consisted of two parts: demographic information and the prevalence and severity of neurological symptoms, which were completed by interviews with patients.
Results: 208 patients with a mean age of 60.5±14.1 years participated in the study. Of the 20 neurological symptoms studied, only two (tingling around the mouth and cranial nerve palsy) were absent in patients. The most common neurological symptoms in patients were headache (82.5%), fatigue (81%) and muscle pain (74.9%), respectively. Also, according to the visual scale, participants reported the highest severity for headache (average: 7.5), followed by fatigue (average: 7.03), and muscle weakness (average: 6.9).
Conclusions: The results showed that the most common neurological symptoms in patients with coronary heart disease include headache, fatigue, and muscle pain and the most severe neurological symptoms in these patients are headache, fatigue, and muscle weakness. According to the findings of this study, these results can be used to diagnose patients and follow them up.

Abstract
Introduction: The main consequence of heart failure is a decline in patients' functional abilities, leading to limitations in occupational and social-family tasks, as well as impairments in cognitive and emotional health, ultimately resulting in a reduced quality of life. This research was conducted to assess the effectiveness of emotion regulation training in improving the quality of life and life expectancy of cardiovascular patients referred to health centers in Khorram Abad City.
Methods: The current research used a semi-experimental design of pre-test-post-test type and three-month follow-up with experimental and control groups. The statistical population of the research included all patients with cardiovascular diseases referred to the health centers of Khorram Abad City in 2023. Among the statistical population, 40 people were selected by direct sampling and randomly divided into two experimental (20 people) and control (20 people) groups. The experimental group was trained in emotion regulation during eight 90-minute sessions for 8 weeks. Both groups completed the World Health Organization Quality of Life Questionnaire (WHO-QOL-BREF) and Schneider et al.'s (1991) Life Expectancy Questionnaire in the pre-test, post-test, and follow-up. The multivariate covariance analysis method and Bonferroni post hoc test were used to analyze the results.
Results: The results showed that emotional regulation training is efficacious in improving the quality of life and life expectancy of cardiovascular patients (P < 0.01), and this effect remained stable in the three-month follow-up phase (P < 0.01).
Conclusions: Emotion regulation training is recommended to enhance cardiovascular patients' quality of life and life expectancy.
