Abstract

Introduction: Considering the high incidence of hip fractures in the elderly, anesthetisis and orthopedic surgeons are frequently encountered with these fractures. Timely fixation of hip fractures decreass morbidity and mortality. Surgical bleeding and the resulting hemodynamic compromise, decrease in vital organ perasion and complications of blood transfusion are important problems in this age group. The ability to decrease Surgical rise; in these patients.
Material and Methods: In a semi-experimental and prospective study 40 victims of hip fracture (50-90 y/o) were selected. The goal was to clarify the relation between anesthesia technique and surgical bleeding 22 patients received spinal (group A) and 18 patients received general (group B) anesthesia.
Results: According to recorded data surgical bleeding was greater in group B (general anesthesia) Than group A, and these data was statistically significant.
Conclusion: In this study %60 of group A and %61.2 of group B were males and %40 of group A and %38.8 of group B were females. During the operation, blood loss was measured and recorded for each patient.

Abstract

Introduction: Studies showed that following spontaneous epilepsy in rats, the permeability of CA1 region of Hypocampus to calcium is increased. In this study the role of voltage dependent calcium channels on the development of kindling induced by Pentylenetetrazol (PTZ) was investigated in rats.
Materials and Methods: In this experimental study rats were divided into two groups. In the test group, Verapamil (calcium channel blocker) was injected in the Hippocampus (4 g/4 min). After 20 minutes kindling was established by PTZ in subconvulsive dose (37.5 mg/kg ip). Convulsing activities were monitored for 20 min. The control group was the same age and undergone the same procedure exept for the injection, in which ACSF was injected without Verapamil.
Results: Verapamil significantly (p<0.01) reduced the number of needed stimulations to progress from stage 0 to 5 of the convulsion and also significantly (p<0.05) prolonged the fifth stage of seizure.
Conclusion: The results of this study suggested that interahippcampal injection of Verapamil facilitated the Pentylenetetrazol kindling in rats but had inhibitory effects on kindled animals.

Abstract

Background: Marital conflicts have great impact on social, economic and cultural aspects of the couples and their children. New Psychological interventions like “Imago Relationship Therapy” (IRT) have been widely used for resolving these conflicts. This study examined the efficacy of couple dialogue technique on marital relationship and children`s behavior in Iranian society. Methods and Materials: In a Clinical Trial study eighty consecutive couples refered to general pediatric clinics due to their children’s diseases, randomly allocated to intervention group(couple dialogue technique) and control group (consult usual treatment method) in consultation center. They filled "Distance and Isolation Questionnaire" and "Gottmann Love and Respect Scale" at baseline, post treatment and three months after intervention as follow up all of them. General linear Model with Repeated Measurements was used to analyze the data. Results: In this study, scores of mutual love and respect in marital relationship (Gottmann scale) and scores in Flooding, Loneliness and Retreatment subscales of Distance and Isolation Questionnaire had a better condition in IRT group just after the trial (P<0.05) but after three months, the differences between two groups were only marginal significant (P<0.1). In Parallel lives` subscale, scores were decreased in two groups, but right after trial there was not significant scores of the groups did not difference between scores. Scores after three months in IRT group was significantly lower in compare to control group. Scores of CBCL have not statistically significant differences in both groups at any time (P>0.05). Conclusion: This study revealed couple dialogue technique was an effective intervention in solving marital conflicts in Iranian population but had not a significant effect on children’s behavioral profile. This may be due to other factors or insufficient duration of intervention and needs more investigation and needs more investigation.

Abstract

Abstract Background: This study was done to asses the educational barriers and problems, and improves the problems from the medicine students’ viewpoint in Arak, 2006-07. Materials and Methods: In this descriptive, cross-sectional study, a valid and reliable questionnaire concluding 71 questions about problems related to education, equipments, personnel, professors, motivation and evaluation methods was distributed among all interns (n=28) and externs (n=44). Results: Inappropriate duration of basic science education was 68%, too much general lesson was 86% did not provide a task description was 84.5% and lesson plan was 91.7% were considered at the start of course. Most of students were asked for learning clinical skill on mulage before practicum (84.5%) and internship (79.9%). Most of students believed that teaching pathology (86.1%) and pharmacology (97.3%) of each course may present by that course in clinic. Lake of sufficient diversity (71.8%), training equipment and inappropriate conference hall (54.9%) and library (76.1%) were considered. Inappropriate behavior 81.7% and corporation with students (58.6%) were reported. About professors, the mean was 16.8±5.1. The most of students worried about their unclear occupational future (87.1%) and 78.6% were believed that exams can not evaluate their knowledges. Conclusion: Asking for lesson plan present in start of course and clinical skills learning on the mulage before practicum and internship are including students demands that can easy to do. Umlaut of basic and clinical courses and teaching pathology and pharmacology courses in clinic can facilitate the basic and clinical courses learning.

Abstract

Background: Transient hip tenosynovitis is one of the common causes of pain and limping in children and includes 0.4% to 0.9% of admissions in emergency wards. The aim of this study is to evaluate this disease in terms of clinical presentations, age and sex distribution, and six-month recurrence. Materials and Methods: In this cross-sectional study, 51 children with diagnosis of transient hip tenosynovitis were investigated. Inclusion criteria were physician's clinical suspicion of acute tenosynovitis according to clinical presentations, physical exam, and age range of 3 to 8 years. Results: Among the 51 children with tenosynovitis, 34 patients were male with age of 61.70±19.1 months and 17 patients were female with mean age of 48.35±20.49 months that presented a significant statistical difference (p=0.026). The most common complaint was hip pain and the most commonly involved joint was the hip. Also, most of the patients had the history of viral diseases. Conclusion: Transient hip tenosynovitis is more common in boys. The right hip is the most involved joint and the majority of patients have the history of recent viral diseases.

Abstract

Background: Sumatriptan is a serotonin agonist. Hippocampal receptors contribute to the serotonergic control of learning. It seems that sumatriptan can also affect learning through serotonin receptors. Thus, this study was performed to investigate the effect of sumatriptan on cellular mechanisms of learning in male rats. Materials and Methods: In this experimental study, male Wistar rats were kept in standard conditions in the Animal Laboratory of the Department of Physiology, Arak University of Medical Sciences. After doing the surgical procedure and locating the electrodes in the CA1 region of hippocampus, synaptic transmission and long-term potentiation were measured, and the effect of three doses of sumatriptan on these parameters was compared to the control group. Results: Sumatriptan could significantly inhibit the effect of tetanic stimulation on long-term potentioation in the CA1 region of hippocampus, as compared to the control group. However, there was no significant difference in the synaptic transmission between the sumatriptan and control groups. Conclusion: This results show that sumatriprtan can probably impair learning and memory through inhibition of LTP in the CA1 region of hippocampus.

Abstract

Introduction: This study was to examine the effects of endurance training and high-fat diet in brain-derived neurotrophic factor (BDNF) in the male adult rat hippocampus.

Materials and Methods: Twenty eight Wistar rats were divided into four groups: (1) Regular diet-sedentary (RD-Sed), (2) Regular diet-Exercise (RD-Exe), (3) HF-sedentary (HF-Sed), and (4) HF- Exercise (HF- Exe) RD-Sed group was regarded as control. Animals in exercise groups received 8-wk endurance training and animals in HF groups were exposed to the HF diet for 8-wk. Hippocampal BDNF protein was assessed using commercial ELISA kits and the data were analyzed by one-way ANOVA. Statistical differences were considered significant at p<0.05.

Results: The results showed that the endurance training had no significant increase in BDNF protein level comparison with the RD-Sd and HF-Sed groups moreover the high-fat diet had no significant effect in BDNF protein level.

Conclusion: According to results of present study, looks applications of endurance training can increase amount of hippocampus BDNF protein level.

Abstract

Background: Considering the increased activity of hypothalamic orexinergic neurons due to morphine administration, and its extensive projections to the hippocampus, it is probable that morphine effect on CA1 neuronal function is mediated by orexinergic system. So the effect of hippocampal orexin-1 receptors (OX1R) blockade on CA1 baseline synaptic response and short term synaptic plasticity was investigated.

Materials and Methods: In this experimental study, animals received morphine 10 mg/kg/12h/(SC) for 10 days. SB-334867-A, OX1R antagonist (0.5&mug/0.5 &mul), was microinjected intrahippcampally for OX1R inhibition before each morphine injection. Baseline synaptic response and short term synaptic plasticity were evaluated by field potential recording. fEPSP was recorded from CA1 following Schaffer collaterals stimulation. After Input/Output construction, short term synaptic plasticity was induced by paired pulse stimulations.

Results: Chronic use of morphine did not affect the baseline synaptic response (p>0.05). SB- 334867-A microinjection in CA1 did not have any effect on baseline synaptic response in morphine dependent rats. Morphine increased paired pulse index (PPI) at 80 ms inter pulse interval (IPI, p<0.05). SB-334867-A pretreatment did not affect this morphine induced PPI change.

Conclusion: The results suggest that orexin-1 receptors (OX1R) do not mediate the effect of morphine on baseline synaptic response and short term synaptic plasticity in CA1 area of the hippocampus.

Abstract

Background: Considering the increased activity of hippocampal glial cells due to chronic morphine administration and the involvement of hippocampus in restoration of the addictive drug-associated experience, the role of these cells on morphine induced conditioned place preference (CPP) was investigated.

Materials and Methods: In this experimental study, four groups of animals were evaluated. After habituation to CPP apparatus on the first day, conditioning was done by injection of morphine (5 mg/kg) or its vehicle (saline) during three consecutive days. On the fifth day, the time spent in each compartment of CPP cage and locomotor activity was recorded for 20 min. To investigate the role of hippocampal glial cells in CPP, these cels were inbibited by bilaterally injecting fluorocitrate (1nmol/1ml), before each morphine injection. CPP testing in this group and animals received fluorocitrate vehicle (Phosphate buffer saline) was done before morphine injection.

Results: Fluorocitrate pretreatment reduced morphine induced conditioned place preference acquisition, so that a significant decrease was observed in conditioning score (unpaired t-test, p<0.01) in this group (n=8) compared to animals received morphine (n=9). Neither morphine nor fluorocitrate pretreatment did not affect locomotor activity (ANOVA, p>0.05).

Conclusion: The results suggest that glial cells in the hippocampus are involved in morphine induced conditioned place preference.

Abstract

Background: The Amyloid beta (Aβ) level increases in the brain of patients with Alzheimer's disease. The present study aimed to investigate the effects of eight weeks continuous training with low and high intensities on Aβ1-42 levels in hippocampus of Alzheimer model rats.

Materials and Methods: Fifty male Wistar rats (12 weeks old and mean weight 219.82±13.10 g) were divided into five groups including: healthy control, Alzheimer’s control, Alzheimer's low-intensity training, Alzheimer's high-intensity training and sham. To induce Alzheimer's disease, homocysteine is infused into the rats cerebroventricular (dose of 0.6M). Low intensity groups trained with 20m/min (50-55% VO2max) and high-intensity groups trained with 27m/min (75-80% VO2max), 60min/day, and five days per week on the treadmill. For data analysis, one-way ANOVA and post hoc Tukey test were performed (p<0.05).

Results: The Aβ1-42 levels in hippocampus of Alzheimer's control group was significantly higher than healthy control group (p=0.001) and in training groups with both low and high intensity was significantly lower than Alzheimer's control group (p=0.02). But no significant differences were found between two intensity (p=0.99).

Conclusion: It seems that continuous exercise training, through reducing the level of Aβ1-42 in hippocampus, can be useful for Alzheimer’s disease model rats and continuous training can be studied as a complementary therapy in Alzheimer's disease.

Abstract

Abstract
Background: Alzheimer is a neurodegenerative disease wich caused memory impairment, reduced cognitive functions, intellectual ability and behavior changes. In this study, the effect of N-acetyl-cysteine (NAC) as a strong antioxidant on memory deficiency and number of CA1 pyramidal neurons in Streptozotocine (STZ) - induced Alzheimeric rats were studied.
Materials and Methods: 32 Male Wistar rats were divided into four groups: sham group, streptozotocin group, treated group with streptozotocin plus N-acetyl-cysteine, and treated group with N-acetyl-cysteine alone. Intracerebroventricular (ICV) administration of STZ was done in the first and the third day of surgery and i.p injection of N-acetyl-cysteine was done in the fourth of surgery. After the memory test, the animals were killed and their brains were fixed and density of intact neurons in the CA1 area of the hippocampus was investigated. Statistical analysis was performed with software SPSS, ANOVA and Prisme software. The level of statistical significance was set at p < 0.05.
Results: The ICV injections of STZ significantly reduced memory retention and intact pyramidal cells compared to the sham group (p<0.001). Administration of N-acetyl-cysteine improved STZ-induced effects on memory retrieval and increased intact neurons in hippocampal CA1 area compared to the STZ group (p<0.001). N-acetyl-cysteine alone doesn’t have any significant effect on memory retrieval and the number of intact neurons in hippocampal CA1 area compared to the sham group (p>0.05).
Conclusion: N-acetyl-cysteine improved memory retrieval and hippocampal CA1 area intact neurons in streptozotocin-induced Alzheimeric male rats.

Abstract

Background and Aim: Neuropathy is the most common abnormality in diabetes mellitus which characterized with cerebral damages especially in hippocampus. This study evaluates the effect of Cinnamomum Zeylanicum extract (CZE) on memory, hippocampal neuron damage and antioxidant enzymes levels in animal model of diabetes.
Materials and Methods: 50 adult Sprague dawley rats were randomly divided into 5 groups: Control, STZ (Streptozotocin, 50 mg/kg; i.p.), and STZ + CZE100, STZ + CZE200 and STZ + CZE400 which were treated with CZE in 100, 200 and 400 mg/kg, respectively. CZE was administered in 14 days, orally. After evaluation of working and spatial memory, activity of catalase (CAT) and glutathione peroxidase (GPx) enzymes was assessed by ELISA. Then, histopathological assessment of hippocampus was done.
Findings: In comparison with the controls, STZ group showed an increase in latency time and distance to the hidden platform in MWM, a decrease in alteration behaviors, cell density and activity of CAT and GPx enzymes in hippocampus (p˂0.05). In addition, treatment with CZE decreased latency time and distance in MWM and increased alteration behavior, hippocampal cell density and activity of antioxidant enzymes in comparison with the STZ group (p˂0.05).
Conclusion: Diabetes with reduction of neuronal density and activity of antioxidant enzymes in the hippocampus causes deficits in spatial and working memory. However, Administration of CZE ameliorates these neuropathologic disorders.

Abstract

Background and Aim: Conventional post-operative rehabilitation programs focus on quadriceps strengthening and knee range of motion. However, hip muscle weakness is evident in patients with knee osteoarthritis. The present trial study aimed to investigate the effects of adding early hip muscle strengthening exercises to conventional rehabilitation programs on pain, Range of Motion (RoM), Quality of Life (QoL), and physical function in patients with unilateral Total Knee Arthroplasty (TKA).
Methods & Materials: The intervention was initiated from the second day after surgery and continued for 10 sessions, 3 times a week on 24 participants in the control and intervention groups. Both research groups received Transcutaneous Electrical Neural Stimulation (TENS) and Infra-Red (IR), followed by exercises. The control group performed knee extensor and flexor strengthening and RoM exercises. The intervention group conducted hip-strengthening exercises in addition. Outcome measures included pain, knee flexion, and extension RoM, and QoL using the 36-Item Short Form Survey (SF-36), Timed Up and Go (TUG) test, Step Test, Six-Minute Walk Test (6MWT), and Hip and Knee Muscle Strength test.
Ethical Considerations: This research was approved by the Research Ethics Committee of Iran University of Medical Sciences (IR.IUMS.REC.1396.9511340012) and it was registered in the Clinical Trial Registration Center (Code: IRCT 20150314021459N6).
Results: All outcome measures improved in both research groups. The improvement in the scores of pain (P=0.03), knee extension RoM (P=0.007), and TUG test (P=0.033) were significantly higher in the intervention group, compared to the control group. The strength of knee flexors (P=0.023), hip flexors (P=0.040), hip extensors (P=0.028), hip adductors (P=0.040), and hip external rotators (P=0.047) significantly improved higher in the intervention group.
Conclusion: According to the present research results, both treatment approaches were effective on patients with unilateral total knee arthroplasty. Considering the better result of some of the outcome measures in the intervention group, the addition of hip strengthening exercises to knee exercises can be useful.

Abstract

Introduction: The fundamental role of marital satisfaction is vital in family formation and marriage survival. Marital dissatisfaction can make couples fail to solve their conflicts. Marital discontent, combined with job-related stress such as pre-hospital emergencies, can lead to heightened conflict in job responsibilities. It leads to the effect on personnel, patients, and their families. Therefore, this study aimed to investigate the marital satisfaction of pre-hospital emergency personnel at Qom University of Medical Sciences in 2023.
Methods: This cross-sectional study was conducted on 200 pre-hospital personnel of Qom University of Medical Sciences in May 2023. The sampling method was a stratified cluster. The data collection tool included the "Demographic Information" checklist and Enrich's "Marital Satisfaction" questionnaire. Data analysis was done using descriptive tests, Independent T-test, and Regression.
Results: The average age of the participants was 37.3 ± 7.06. More than half of the participants had a bachelor's degree. Most people had rotating shifts. The average score of marital satisfaction was 145.94 ± 22.21. Based on the results of the multivariate regression test, the variables of education, employment status, work experience, type of shift, having a second job, and having a job other than medical staff explain 63% of the variance of marital satisfaction of pre-hospital personnel.
Conclusions: According to the findings of the present study, in general, the marital satisfaction of pre-hospital emergency personnel of Qom University of Medical Sciences is at an average to high level. People with a fixed shift, with a second job, high work experience, without a fixed job status, and bachelor's and associate's degrees had less marital satisfaction than others. Therefore, managers and policymakers in the health field must pay more attention to the quality of family life, especially the married life of pre-hospital emergency workers with the mentioned conditions.

Abstract

Introduction: The thickness of the molecular and pyramidal layers in the hippocampus represents a pivotal aspect of Alzheimer's research. This study was conducted to investigate the effect of swimming endurance, resistance, and combined exercises on the molecular and pyramidal layers of the hippocampus tissue thickness of Alzheimer's rats.
Methods: In this experimental study, 40 large white Wistar laboratory rats (6 weeks old; Weight 180-200 g) were randomly and equally divided into five groups: healthy control, Alzheimer's control, endurance, resistance, and combined. Alzheimer's disease was induced by intraperitoneal injection of trimethyltin chloride (8 mg/kg). Two weeks after the injection and confirmation of Alzheimer's induction, the training protocols of endurance (5-sessions/week, the first to the fourth week incrementally from 5-15-min to 45-min of swimming, the fifth to the twelfth week 60-min of swimming), resistance (5-sessions/week, The first week of familiarization with the exercise, week 2; 30%, week3-5; 70-90%, week6-8; 100-110%, week9-10; 120-130%, and week11-12; 140-150% of body weight), combined (2 resistance sessions/week and three endurance sessions/week) was performed for 12 weeks. Forty-eight hours post-intervention, animals were dissected, and hippocampus tissue was harvested. Finally, the data were analyzed at the significance level of P < 0.05.
Results: The thickness of the molecular and pyramidal layers of the hippocampal tissue of Alzheimer's animals decreased compared to healthy animals, and all three endurance, resistance, and combined exercise protocols increased layers (P < 0.001). However, the increase in the molecular and pyramidal layers thickness of the hippocampal tissue of combined group rats compared to the endurance and resistance group had a greater increase (P < 0.01).
Conclusions: These findings highlight the changes in the thickness of the hippocampal tissue layers concerning Alzheimer's pathology and the effect of combined exercises on this parameter. While exercise may have positively affected hippocampal volume and synaptic plasticity, more research is needed to fully understand the impact of exercise on hippocampal layer thickness in Alzheimer's.
