Abstract

Association between biological factors and disruptive behaviors in children and adolescents is important to investigate. Antisocial, aggressive and delinquent behaviors in adults often begin early in life. Basal cortisol is a valuable biological characteristic of children with disruptive behavior disorder (DBD). In this study the effect of family training on basal salivary cortisol in children with DBD was investigated. Materials and Methods: This is a clinical trial study. Basal salivary cortisol levels were studied in 19 children between 8-13 years old with DBD, before and 2 months after intervention (family training). The disruptive behavior of the child was assessed with child behavior checklist (CBCL). Cortisol levels and score of behavior were compared before and after intervention. Data was analyzed using descriptive statistics, paired t-test and pearson correlation. Results: Salivary cortisol before family training was 7.9±4.6 nmol/L and after that was 10.46±3.84 nmol/L which was significantly different (p<0.001). Behavior score before intervention was 72.05±10.10 and after that was 49.361±1.89 that was also significantly different (p<0.0001). Children with lower basal salivary cortisol had a better response to intervention. Conclusion: Parent training is an effective method for behavioral modification in DBD. Salivary cortisol can be considered as a predictive factor for severity of disruptive behavior, also a factor to assess the response to parent training in children with DBD.

Abstract

Background: Researches showed that there is a relationship among alopecia areata, psychological factors and prevalence of psychological disorders. In addition alopecia areata has a great effect on the quality of life of these patients. The purpose of this study is to investigate the effectiveness of cognitive-behavioral stress management therapy on female patients`s the quality of life with alopecia areata. Methods and Materials: This is a case-control study with pre and post-test exam which is carried out in Isfahan Skin Disease and Leishmaniasis Research Centre. The participants consisted of 20 alopecia areata patients diagnosed by specialists. The questionnaire was Skindex-16 which was completed by patients before and after the intervention. Cognitive-behavioral stress management therapy was done on the case group in eight sessions over a period of two months. Data was analyzed with covariance (ANOVA). Results: The mean of quality of life scores in experimental group in v.s control group significantly decreased (P<0.05). Conclusion: It seems, cognitive-behavioral stress management therapy couses in quality of life improvement in patients with alopecia areata.

Abstract

Background: This study was designed to investigate on efficacy of metacognitive–behavioral therapy (MCBT) on panic beliefs of panic female patients. Materials and Methods: This is a clinical- trial study. The participants, who were 24 panic female patients diagnosed with panic disorder, were selected from 76 panic patients referring to psychiatric clinics. The participants were randomly assigned in to two experimental and control groups. The data were gathered using the panic belief questionnaire which was completed in pretest, posttest and follow-up stages by the participants. MCBT was conducted in 8 sessions on the experimental group while the control group, in addition to the reception of common medical treatments, was laid in waiting list for psychological therapy. Results: According to the results of covariance analysis, the mean of panic beliefs scores obtained in the experimental group were significantly lower than those of the control group (p< 0.05). Conclusion: Panic female patients who attended in MCBT acquired lower scores in panic belief than the control group. Therefore, the results demonstrate that MCBT approach causes to keduce panic beliefs in panic female patients.

Abstract

Background: Sleep behavior and movement disturbance (SBMD) is of a relatively high prevalence in children and has a significant impact on their performance in daily activities and quality of life. Snoring, on the other hand, is considered one of the main causes of sleep disturbance in children. This study aims to assess the relationship between snoring and SBMD in 2 to 12 years old children in Qom city.

Materials and Methods: In this case-control study, 100 children with snoring and 100 healthy children were studied over 3 months. The parents took Berlin questionnaire on snoring and childhood sleep questionnaire. Data analysis was done using SPSS software version 18.

Results: There was a significant correlation in mean SBMD between case (13.78) and control (11.45) groups (p<0.001). Independent t-test indicated a significant difference between the case and control groups in the mean of SBMD (p<0.001). Also, significant relationships were seen between snoring and adenotonsilar hypertrophy (p<0.001) and apnea and SBMD (p=0.036), but there was not any relationships between SBMD and age, children less than 7 and more than 7 years old (p=0.231), and severity of snoring and SBMD (p=0.202).

Conclusion: This study demonstrates the importance of sleep disorders, especially snoring and SBMD and their relationship, in children. Sufficient attention to this issue can raise parents and physicians awareness and pave the way for early diagnosis and treatment of these disorders.

Abstract

Background: Impulsivity is a core social pathology. Therefore, the purpose of this study was to Effectiveness of group dialectical behavior therapy based on core distress tolerance and emotion regulation components on Expulsive Anger and Impulsive Behaviors.

Materials and Methods: Research method is a semi experimental socio-statistic approach consisting of experimental group (dialectical behavior therapy) and control group. Participants were patients referred to Amir Kabir Hospital in Arak. and who were Expulsive Anger and Impulsive Behaviors. Based on stratified random sampling, 16 patients (women) were placed in each group. Research tools included the structured diagnosis interview according to DSM-IV-TR (2000), Barrat impulsivity scale (1994) Distress Tolerance Scale (2005) Difficulties of Emotion Regulation Scale (2004) and dialectical behavior therapy were done for two months, 8 groups Sessions.

Results: The results of this study showed that there was significant difference between control and experimental groups after implementation of dialectical behavior therapy relying on component tolerance and regulation of emotional distress variables impulsive behavior and explosion furies (p<0.01).

Conclusion: Distress tolerance and emotion regulation components effective on Expulsive Anger and Impulsive Behaviors.

Abstract

Background: Depression is on the top list of mental disorders which account for about 25 percent of patients referred to health centers in your world. So, is presented in different ways to treat it. Therefore, the aim of this study is to compare the effectiveness of Dialectical Behavior Therapy and Cognitive Behavioral Group Therapy in reducing depression in mothers of children with disabilities.

Materials and Methods: This study is quasi-experimental and consists of experimental and control groups. This study population was mothers referred to mobility, occupational therapy and physiotherapy centers who had depressive symptoms. 8 patients in each group were selected by convenience sampling. The research instrument were the Structured Clinical Interview for DSM-IV Axis I disorders and the revised Beck Depression Inventory form (1996). Dialectical behavior therapy and cognitive behavior therapy groups were instructured for 2 months (8 sessions of 2 to 2.5 hours). But the control group did not receive intervention.

Results: The results showed that there were significant differences between the mean depression scores of dialectical behavior therapy and cognitive therapy group with control group (p<0.001). Also, there is a significant difference between the mean depression scores of dialectical behavior therapy with cognitive therapy (p<0.001).

Conclusion: In the area of treatment and working with depressed people and those who are in crisis mode, it seems that dialectical behavior therapy and cognitive therapy group in view of its nature is very efficient and promising.

Abstract

Background: Premenstrual Syndrome (PMS) consists of repetitious physical and psychological symptoms that occurs in luteal phase of menstrual period and will be over when menstrual period starts. The goal of this study is to investigate the effect of calcium therapy plus vitamin D and cognitive behavioral therapy (CBT) on the parameters of PMS symptoms in women suffering from PMS.

Materials and Methods: This study has an experimental plan of pretest and post-test of the control group. The statistical society is 250 female staff of Tehran Universities in academic year 2013-2014. Premenstrual syndrome screening tool (PSST) questionnaire was used as an evaluation tool. The collected data were analyzed using the indexes of the descriptive statistics and covariance analysis (p<0.05).

Results: The minimum age of the statistical society was 24, 52.5 % of them held a master's degree and 60% were single. Cognitive behavioral therapy method, calcium and vitamin D therapy were significant in improving the parameters of symptoms at p<0.05 level of significance.

Conclusion: Combining cognitive behavioral therapy and calcium supplementation and vitamin D is significantly effective in improving components of PMS sympotoms and applying these methods is recommended by the consultants and gynecologists.

Abstract

Background: Exclusive breastfeeding for the first six months after delivery plays a crucial role in preventing infants morbidity and mortality. Exclusive breastfeeding intention during pregnancy is one of predictors of this behavior. Determining the effective factors in exclusive breastfeeding intention helps plan for promoting breastfeeding intention and behavior. The aim of this study is to determine predictive factors of exclusive breastfeeding intention in pregnant women, based on the theory of planned behavior.

Materials and Methods: This analytical study was conducted in 2014 on 224 primigravide women referred to Health Centers of Kashan. The samples were randomly selected from 16 health centers. The mothers completed the questionnaires of demography, intention, attitude, subjective norms and the perceived behavior control. A linear regression analysis was used to identify independent predictors of the exclusive breastfeeding intention.

Results: In regression model, the perceived behavior control (&beta=0.42, p<0.001), attitude (&beta=0.27, p<0.01) and subjective norms (&beta=0.18, p=0.059) were predictors of the exclusive breastfeeding intention. This model could explain 63% of the variance of the exclusive breastfeeding intention. The knowledge and demographic factors were not predictors of the intention.

Conclusion: The results reveal the perceived behavior control, attitudes and subjective norms could predict exclusive breastfeeding intention, so they should be considered as general guidelines in designing educational interventions in order to improve exclusive breastfeeding in women.

Abstract

  Background: M others must gain enough knowledge and practice about danger signs in children to achieve behavior change. Knowing such factors influencing behavior change will facilitate change. So, training about danger signs in children using models that identify the factors influencing behavior is essential. Health belief model is one of the effective models in education. Therefore, the aim of this study was to assess the effect of education on behavior of mothers about danger signs in children less than five years.

  Materials and Methods: This is an educational trial study that was conducted on 116 mother with children under five years old referring to treatment health centers in Arak city in 2014. They were divided into two groups case (n=56 ) and control ( n=60 ) randomly.

  The data collected by filling the valid and reliable questionnaire that reseatcher made along with interview. An educational intervention was conducted in case group during the four learning sessions. Three months after educational intervention, post-test was conducted by filling questionnaire. Finally, data were analyzed by SPSS software in significance level of p<0/05.

  Results: There was no significant difference between mean score of health belief model construct and behavior in the experimental and control groups before the intervention(p<0.05) , but significant differences were shown after intervention(p<0.001). The mean score of the mothers in the intervention group before and after intervention was 39.58±42.15, and 84.52±23.05 respectively, that showed significant differences (p<0.001).

  Conclusion: According to the results of this study, educational intervention based on initial assessment on Health Belief Model was effective in improving mothers' practice about danger signs in children. Thus, it can be helpful to design training programs.

Abstract

Background: The purpose of this study was to determine the effectiveness of dialectical behavior therapy in reducinng expulsive anger.

Materials and Methods: This is a pre-test and post-test semi-experimental study with control group. The study population included 32 patients with expulsive anger that reffered to Amir kabir remedial education center based on psychiatrist diagnosis and were randomely divided into case (dialectical behavior therapy) and control group. The case group was educated the dialectical behavior therapy skills for 16 sessions from 2 until 2.5 hours at teamly. To collect data, the patients filled Eysenk expulsive questionnaire before and after intervention. Considering no necessary assumptions were fulfilled to conduct the multivariate analysis of covariance and variance, so, the univariate analysis of variance was used.

Results: The results of variance analysis proved that dialectical behavior therapy significantly reduced expulsive angers.

Conclusion: With due attention to the results, it can be said that dialectical behavior therapy leads to reducing expulsive angers in treated patients, thus, this method can be included in interventional programs for target groups in psychotherapy centers. Aslo, it helps to improve the life of patients by reducing expulsive angers.

Abstract

Background: Recently it is accentuated on the role of psychological symptoms in the course of chronic diseases and applying psychological interventions as complementary interventions. The aim of this study was to compare the effects of cognitive-behavioral stress management, optimism training and conevntional medical therapy on somatic symptoms, perceived stress, illness perception and quality of life in patients with ulcerative colitis.

Materials and Methods: In this study, thirty female patients were selected accidentally, and randomly assigned into three groups including cognitive-behavioral stress management(n=10), optimism training(n=10) and conventional medical therapy(n=10). All patients completed Perceived Stress Scale, Illness Perception questionnaire, WHO Quality of Life questionnaire, and Lichtiger Colitis Activity Index. Then, experimental groups participated in 9 sessions of  cognitive-behavioral stress management and optimism training group interventions. Thereafter, three groups answered to scales again. This course was repeated in a 6-mounth follow-up. Analysis of covariance (ANCOVA) was used to analyze the data.

Results:  The findings indicated that the effects of two interventions were significant on patient´s illness perception, perceived stress and quality of life however, these interventions did not change somatic symptoms.

Conclusion: Totally, it can be said that psychological interventions such as stress management and optimism training can be effective on improving the psychological symptoms in patients with ulcerative colitis and their cure.

Abstract

Background: Today, the consumption of junk food as a snack among children, especially in elementary school students is on the rise. This research has measured Theory of Planned Behavior constructs in this field.

Materials and Methods: This is a cross-sectional and analytical study that was carried out on 217 primary school students in Arak city in 2015. Students were selected according to cluster sampling method. Data collection instrument was a researcher-developed questionnaire consisting of theory of planned behavior constructs such as attitudes, subjective norms, perceived behavioral control, behavioral intention and performance. Reliability and validity of the questionnaire was studied before the study. Multivariate linear regression model was used to determine the most important predictive constructs of students behavior.

Results: In this study, the mean and standard deviation score of attitude (75.41±12.42), subjective norm (78.25±11.20), perceived behavioral control (63.69±17.06), behavicral intention (76.26±15.44), and performance (48.84±24.44) were observed. Based on the results, perceived behavioral control could predict 23 percent change in behavior. There was a direct correlation between the score of constructs and performance(p<0.05). Moreover, there were not any significant differences(p>0.05) between students nutrition and age(r=0.019), weight (r=0.07), height (r=0.12) and BMI (r=0.07).

Conclusion: The results of this study suggested that positive attitude and self-esteem must taken into account to control students diet, in the design of interventions to prevent consuming junk food.

Abstract

Background: Diabetes is one of the most common disease resulted from metabolism disorders. Thus, Studying medication adherence in patients to explain educational interventions to improve the health of these patients is necessary. Therefore, the present study aimed to assess the factors affecting medication adherence in diabetics patients based on health belief model in Arak 2014.

Materials and Methods: In this analytical study, 366 diabetic patients in Arak in 2014 were selected and data were collected through a researcher made questionnaire about patients behavior in the field of medication adherence and its influencing factors based on the health belief model. Linear regression model was used to determine the predictive power of structures.

Results: The mean knowledge score and the medication adherence was 31.34 and 84.34, respectively. In this study, perceived susceptibility, perceived barriers and internal action guide were as the strongest predictors of medication adherence behavior in patients and predicted their behavior variance 0.36. Between function and susceptibility and severity and perceived benefits and self- efficacy of manual internal and external action guides in the field of medication adherence with diabetes, there was a direct correlation that the highest correlation was related to the perceived susceptibility structure (r=0.42 , p<0.001).

Conclusion: The results showed that the educational interventions with an emphasis on building the perceived sensitivity of patients with diabetes for medication adherence and emphasis on internal incentives as motivators and incentives for patients to achieve better results in the field of medication adherence should be at the heart of educational interventions.

Abstract

Abstract

Background: Bariatric surgery is the most effective intervention for treating severe obesity and patient's adherence to self-management behaviors is essential to reduce complications after surgery. The purpose of this study was to investigate the psychometric properties of BSSQ in Iranian population.

Materials and Methods: The statistical society was all of the obese patients that underwent laparoscopic obesity surgery in Shiraz Ghadir Mother & Child Hospital from December 2016 till June 2016, and 201 of them (149 females and 52 males) were selected by using available sampling method. They responded to demographic characteristics, post bariatric surgery self-management behaviors questionnaire and general and specific adherence scales. For validity, methods such as content validity, structural validity (factor analysis and correlation analysis), simultaneous validity were used. The reliability of the questionnaire was determined by using bisection and internal stability methods by Cronbach s alpha.

Results: According to the results of explatory factor analysis, sisx factors including eating behaviors, physical activity, fruits, vegitables, grain and protein intake, fluid intake and dumping syndrome management were elicited, that these 6 factors explained 61.54% of variance of self-management behaviors. Total score of correlation matrix BSSQ with GAS & SAS were 0.363 and 0.702. For reliability, the Cronbach's alpha coefficient of this instrument was found 0.90 and guttman split-half coefficient was 0.78.

Conclusion: Results show that BSSQ has an acceptable validity and reliability and it can be used for assessing the post bariatric surgery self-management behaviors in Iranian population.

Abstract

Abstract

Background: The aim of this study was to determine the effectiveness of cognitive-behavioral group therapy on quality of life of patients with cardiovascalar diseases.

Materials and Methods: This semi-experimental study was done based on a pretest -posttest design with control group. Statistical population included all of the patients with cardiovascular diseases referred to Amir-Kabir educational and medical center of Arak, and sample included 30 persons who selected by screening and simple random sampling and assigned into two control and experimental groups. Research instrument included Mcnew quality of life scales which participants answered it during two pre-test and post-test steps; also, experimental group participated in 9 cognitive-behavioral therapy meetings.

Results: There was a significant difference between mean quality of life and its components in both experimental and control groups.

Conclusion: Results analysis showed that group-based cognition- behavior theray has an influence on life quality (emotional, physical and social domains)(p>0.05).

Abstract

Abstract
Background: The present study aims to provide a comparative analysis of studies on Acceptance-Commitment and Cognitive-Behavioral interferences effectiveness on generalized Anxiety disorder.
Materials and Methods: This is a quasi-experimental research. Students at the Islamic Azad University of Toyserkan, Iran, who have been in contact with the counseling office in the educational year 2016-2017, were selected as statistical population. These students, in a diagnostic interview with a psychiatrist, met the DSM5 standards for generalized anxiety disorder and were approved by a trusted psychiatrist. Then, these selected subjects gave average or more score in beck anxiety test and were randomly divided into two groups of 20. After executing interferences for each group, data were analyzed by t-test and covariance.
Results: The findings indicated that the average acceptance and commitment interference was 12.3 (t= 16.01 and p < 0.001) and the average cognitive-behavioral interference was 11.25 (t= 10.60 and p < 0.001) on generalized anxiety disorder. Also, covariance analysis showed that ACT was more effective than CBT and f (0.03) was significant. Therefore, all hypotheses turned out to be approved.
Conclusion: The research demonstrated that both interferences are effective in treating generalized anxiety disorder, although Acceptance and Commitment interference is more effective than Cognitive and Behavioral interference.

Abstract

Background and Aim: Iron deficiency is one of the most common nutritional problems during pregnancy. The purpose of this study was to assess the theory of planned behavior structures on nutritional prophylactic treatment of anemia in pregnant women of Arak city in 2018.
Materials and Methods: This is a cross-sectional study that was carried out on 199 pregnant women in Arak city using simple random sampling in 2018. The data were gathered with valid and reliable researcher-made questionnaire including demographic information and the theory of planned behavior structures. The study was conducted to determine the most important predictive constructs of pregnant womens’ behavior using a multivariate linear regression model.
Findings: The finding showed that mean age of mothers was 28.81 ± 5.1 years and the mean weekly gestation was 28.04 ± 8.9 weeks and the gestational interval with previous pregnancy was 5.18 ± 3.9 years. Mean±standard deviation of attitude score (3.61 ± 0.62), subjective norm (3.60 ± 0.74), perceived behavior control (3.50 ± 0.58), behavioral intention (3.49 ± 0.61), performance (3.55 ± 0.76) and awareness (0.57±0.2) were observed. Linear regression model showed that the performance score of pregnant women improved for each unit of attitude and subjective norms and perceived behavioral control, respectively, by 0.463, 0.343 and 0.347, respectively.
Conclusion: The results emphasize the provision of educational programs on the prevention of anemia in pregnant women with an emphasis on attitudinal structures, mental norm, and perceived behavioral control. It is suggested that at the time of designing interventions to improve the prevention of anemia in pregnancy, the structures of the theory of planned behavior should be considered.

Abstract

Background and Aim Psychological factors play a significant role in the exacerbation of cardiovascular diseases. This study aimed to investigate the effectiveness of teaching emotion regulation skill (based on dialectical behavioral therapy) on cognitive emotion regulation and Quality of Life (QoL) of patients with cardiovascular diseases.
Methods and Materials The present study has a quasi-experimental design with pretest-posttest and the control group. The statistical population of the study included all patients with cardiovascular diseases in Kermanshah City, Iran in 2018. Among them, 30 participants were selected by simple random sampling method. Then, they were randomly assigned to the two groups of experimental and control (each group 15 patients). We performed the intervention of excited ordering education based on dialectical behavioral therapy for the experimental group in eight 90-minute sessions, while the control group did not receive any intervention during this period. To collect study information, we used a short form of the emotional cognitive regulation questionnaire and MacNew Quality of Life Questionnaire. To analyze the data, we used the multivariate analysis of covariance in SPSS V. 20.
Ethical Considerations The Research Ethics Committee of Kermanshah University of Medical Sciences approved this study (Code: IR.KUMS.REC.1397.39).
Results The results showed that the scores of Positive Cognitive-Emotional Strategies and quality of life of cardiovascular patients after training of emotional regulation skills based on dialectical behavioral therapy in experimental group significantly increased in comparison with control group, and also Negative Strategies Scores were decreased (P<0.01). 
Conclusion Based on our positive study results, we recommend the therapists to use psychological treatments as complementary therapies for these patients.

Abstract

Background and Aim: Parental acceptance is an important parameter in selecting the proper behavior management technique during pediatric dental treatment. The present study aimed to compare the effects of different informing methods on acceptance of parents about behavior management techniques used in pediatric dentistry.
Methods & Materials: This quasi-experimental study was conducted on one 162 parents of 3-7-year-old uncooperative children who were candidates for dental treatment. They were randomly divided into three groups. Their acceptance about six behavior management techniques including general anesthesia, voice control, hand-over-mouth, parental separation, active and passive physical restraint were assessed by a questionnaire. After determining the level of acceptance, the information about the techniques was presented through three ways include writing, verbal and visual methods. Finally, their acceptance of each technique and the reasons for non-acceptance were re-evaluated. Data were analyzed in SPSS v.23 software using ANOVA and t-test.
Ethical Considerations: This study obtained its ethical approval from the Research Ethics Committee of Arak University of Medical Sciences under code: IR.ARAKMU.REC.1398.102.
Results: The informing methods significantly increased the parents’ acceptance about behavior management techniques. According to the ANOVA results, the effect of visual presentation method in general anesthesia technique was significantly lower than that of other methods (P<0.05), while its effect in active physical restraint technique was significantly greater than that of other methods (P<0.05). There was no statistically significant difference between the effects of informing methods in other behavior management techniques.
Conclusion: Informing parents about pediatric behavior management techniques increase their acceptance. Informing by visual presentation is the most effective method for increasing their acceptance of physical restraint technique, while in their acceptance of general anesthesia technique other informing methods are more effective

Abstract

Background and Aim: One of the problems of children with Autism Spectrum Disorder (ASD) is behavioral excesses resistance to many educational and rehabilitation programs. Parent-mediated behavioral interventions seem to be effective in overcoming these problems. However, these methods are not well-introduced. The purpose of this study is to systematically review these studies and their primary and secondary outcomes, and finally analyze the components. 
Methods & Materials: This study was a systematic review. The search included SID, Magiran, Medline, PubMed, Springer, Science Direct, Online Library, and PsycINFO. We reviewed The articles published between 2000 and 2017 about parent-mediated behavioral interventions on behavioral excesses in autistic children.
Ethical Considerations: This study with ethical code IR.SBU.ICBS.97/1013 was approved by the Biological Research Department of Shahid Beheshti University.
Results: The findings of 9 studies indicated positive effects of parent-mediated behavioral intervention on behavioral excesses, including repetitive behaviors, irritability (including tantrums, aggression, and self-injurious behaviors), echolalia, and destructive behaviors (preliminary results). Also, these interventions improved the adaptive behaviors in autistic children, and parental functions such as self-efficacy, parental style, psychological problems (secondary results). Components of the therapeutic program included the type of consequence-based interventions (such as response interruption and redirection), antecedent-based interventions A (visual cue and daily schedules), antecedent-based interventions B (such as enriching environment with play). Three articles had medium certainty of the evidence, and 6 had high certainty of evidence.
Conclusion: The results of this study indicated the positive effect of parent-mediated behavioral intervention on behavioral excesses in children with ASD. Future studies should emphasize the comprehensiveness of all the effective components in the parent-mediated behavioral intervention and the feasibility of the intervention in various contexts. It is suggested that parent-mediated interventions be implemented on behavioral excesses in children with ASD in Iran.
