Abstract
Introduction: Production of free radicals and reactive oxygen species is an integral part of aerobic metabolism. Disturbance of the balance between the production of free radicals and antioxidant system against them produces oxidative stress. Paraquat toxicity is induction of oxidative stress. However, the aim of our study was to evaluate oxidative stress in blood samples of workers in a pesticide factory formulating paraquat.
Materials and Methods: In this study we measured lipid peroxidation (LPO) with thiobarbituric assay (TBARS), total antioxident power (FRAP) and SH groups between workers in pesticide factories (case group) and health subjects (control group).
Results: Oxidative stress was significantly higher (p<0.001) in LPO, significantly lower (p<0.001) in TBARS and significantly lower (p<0.001) in SH groups in the case group compared to control group
Conclusion: It is concluded that chronic exposure of workers in their workplace to paraquat producess oxidative stress that could be prevented by well known nutritional antioxidant supplements.

Abstract
Introduction: Adenosarcoma is a rare uterine tumor composed of benign epithelial and malignant stromal elements. In 20% of cases stroma contains heterologous elements (usually from stratified muscle type). Association between Tamoxifen usage (exogenous strogen) or ovarian thecoma (endogenous strogen) and occurance of this tumor has been reported. This is a case report of a uterine Adenosarcoma. Case: The patient was a 45 years old woman with history of 16 years OCP use who was admitted with a uterine mass and undergone total hysterectomy. Microscopic assessment of slides, stained by H&A, was indicator of Adenosarcoma with cartilage heterologous elements. Conclusion: Uterine Adenosarcoma is a rare tumor. Although there is a few reports of cartilage heterologous elements in uterine Adenosarcoma, this case had multiple cartilage points in stroma. Also in this case, the coexistance of this tumor with long- term OCP use was considerable.

Abstract
Introduction: World health organization has proclaimed the prevalence of smoking among 15 years old European region students, more than 24 percent. The cigarette smoking indicator in our country among over 15 years old individuals is 11.9 percent and in the province under survey is 15.6 percent. This research has been performed in order to study the effect of health education program based on health belief model (H.B.M) on preventive actions of smoking in middle school students. Materials and Methods: This is a quasi-experimental study, done on 248 students of grade one in middle schools of Boukan city (119 boys and 129 girls) that were randomly divided into experimental and control groups. Data collection tool was a questionnaire including 77 questions based on health belief model (perceived susceptibility, perceived severity, perceived benefits and perceived barriers to smoking, and cues to action) and preventive actions of smoking. After data collection in the first stage, action was taken with regard to educational intervention and then in the second stage (after the intervention), data was collected again. Data was analyzed using T test. Results: Results showed that the mean score of all parts of health belief model in experimental group(except for perceived barriers) after educational intervention compared to before intervention was increased significantly and also the mean score of all parts of HBM after intervention was significantly increased in experimental group compared to control group. Conclusion: The results of this survey show that by increasing the score of health belief model parts including, perceived susceptibility, perceived severity and perceived benefits, adopting preventive actions of smoking also increases. Therefore, the results of this survey confirm the efficiency of health belief model in adopting preventive actions of smoking.

Abstract
Background: Pyelonephrities is the urinary tract infection. Using supplemental dugs may decrease duration of treatment and hospitalization. We studied the effect of vitamin C on the recovery of symptoms due to pyelonephrities Methods and Materials: In this clinical trial, double blind, study, 64 patients with uncomplicated pyelonephrities were assessed in 2 groups randomly. Case group took vitamin C capsules, and control group took placebo capsules. In both groups, primary treatment was ceftriaxon capsules during hospitalization and they took ciprofloxacin capsules for 14 days after discharging. Symptoms and Urinary analysis were assessed in the first day of administration and then on the 7th and 14th days after discharging. Data were analyzed with Chi-Square test. Results: Mean of fever duration in vitamin C group (1.130.34 SD day) and placebo group (1.560.62 SD day) significantly decrease (p=0.001). There was a significant difference in dysuria duration in vitamin C group 2.191.06 SD day with placebo group (2.971.06 SD day) (p =0.007). In flank pain, nausea and vomiting duration difference was not significantly (p≥ 0.05). Conclusion: It seems vitamin C, as a supplemental drug improves some pyelonephrities symptoms, such as fever and dysuria, decreases of the hospitalization period

Abstract
Background: Microbial infections are important challenges to health, and health care officials have major difficulties dealing with them especially regarding their antibiotic resistance. The main aim of this study was to evaluate drug synergistic and anti-bacterial effects of cuminum cyminum essential oil on some pathogen bacteria. Materials and Methods: Cuminum cyminum essential oil was extracted from its fruit. For evaluation of antibacterial effects of the essential oil, disc diffusion method through the measurement of the inhibitory zone diameter and micro-broth dilution for determining the minimum inhibitory concentration (MIC) on several standard bacteria were used. In evaluation of the synergistic and antagonistic effects, standard bacteria were cultured in the media containing essential oil, and antibiotic discs were, then, placed on it. Results: According to the results of disc diffusion test in Agar, cuminum cyminum essential oil, with a 44 mm inhibition zone, had the greatest antibacterial activity against bacillus cereus. It had the greatest positive co-action with gentamicin (10 µg) on escherichia coli. Conclusion: The results of this study indicate that cuminum cyminum essential oil alone or in combination with antimicrobial agents may be useful in treatment of bacterial infections. Additionally, this component can enhance the effect of some antibiotics this implies its application, especially in drug resistance cases.

Abstract
Background: Correct use of cuminum cyminum and finding its different applications in medicine and industry necessitates a more clear understanding of this medicinal plant. This study deals with identifying the effects of the essential oil of this herbal plant on macrophages and tumor cell lines. Materials and Methods:Cuminum cyminum essential oil was extracted from its fruit, and its effects on peritoneal macrophages and LPS stimulated macrophages were examined. MTT assay was done for evaluation of macrophages viability and the amount of nitric oxide (NO) in culture supernant was measured by Griess Reagent. WEHI-164 mice fibrosarcoma cell line was cultured with different concentrations of cuminum cyminum and cytotoxicity level was evaluated by MTT assay. Results: The viability of macrophages and also, the amount of NO production in 50 and 500 μg/ml cuminum cyminum essential oil was lower than that of the control group (p<0.001). MTT assay showed that cuminum cyminum essential oil in 50 and 500 μg/ml concentrations significantly inhibits tumor cells growth (p<0.001). Conclusion: Cuminum cyminum essential oil by having immune-modulatory properties can be used in treatment of many inflammatory and immunologic disorders. Also, it can be used as a therapeutic or complementary agent in tumor therapy.

Abstract
Background: Programmed death 1 (PDCD1), a negative T-cell regulator which induces peripheral tolerance, belongs to Ig super and CD28/CTLA-4 families. PD-1 gene induces negative signals in T-cells during interaction with its ligands. Thus the aim of this study was to investigate the relationship between PD-1 polymorphism and the risk of rheumatoid arthritis (RA) in Iranian patients and healthy controls.
Materials and Methods: In this case-control study, genomic DNA was extracted from the whole blood samples using DNA purification kit (DNG-plus, Cinnagen, Iran). PD1.1G/A as a SNP located on promoter with position -536 were genotyped for 120 RA patients and 188 healthy controls through PCR-RFLP method. Association of genotypes and alleles frequency in the patients was compared with controls and analyzed using Chi-square test and 2×2 contingency table in SPSS software version 15.0. The diagnosis of RA patients and provision of their clinical information was done in Rheumatology Research Center of Tehran University of Medical Sciences, Tehran, Iran.
Results: The A allele of the PD1.1 polymorphism located on the promoter of PD-1 gene was significantly more frequent in Iranian RA patients than the controls (p=0.04). There were no significant differences in PD1.1G/G genotype (p=0.08), PD1.1A/A genotype (p=0.39), and PD1.1G/A genotype (p=0.16) between RA cases and controls.
Conclusion: The findings of this study showed the presence of a significant relationship between the A allele of the PD1.1 (-536) of the promoter and susceptibility to RA in Iranian patients.

Abstract
Background: Live attenuated Shigella vaccines have shown promise in inducing protective immune responses in human clinical trials. The aim of this study was to design and construct pDS132::∆icsA as a suicide plasmid for targeted deletion of a region of icsA gene in Shigella. Materials and Methods: In this experimental study, species and serotypes of Shigella isolated from diarrhea samples of children at Firozabadi and Milad Hospitals of Tehran were confirmed by using serological and PCR tests. Identification primers of icsA gene were designed and then cloned to the pGEM-5zf vector and sequenced. According to icsA restriction enzyme map, 1751 bp of icsA gene was deleted by HincП restriction enzyme and the ∆icsA was constructed successfully. The pGEM∆icsA vector was digested by use of SphI and SalI enzymes and was then cloned to a suicide vector (pSD132). Precision of the process was confirmed by phenotype and genotype tests. Results: The Shigella dysenteriae type 1 strain was verified by serological tests and PCR. Sequence of the icsA gene in the native strain was identical to the strains submitted in the gene-bank database. Since the pDS132::∆icsA contains 1484 bp derived from icsA gene, it can be used to disrupt icsA gene as a specific suicide vector. Conclusion: Application of suicide systems facilitated mutant construction in more specific and effective methods in comparison with the other primary techniques such as serial passage.

Abstract
Background: Food insecurity is defined as the limited or uncertain availability of enough food for an always active and healthy life. Diabetes mellitus, one of the most common metabolic diseases, has been associated with significant morbidity and mortality and it is considered a health priority in Iran. Recent studies indicate the widespread prevalence of food insecurity and diabetes mellitus in Iranian society. This study reviews the status of food insecurity and effective socio-economic factors in people with type 2 diabetes.
Materials and Methods: This cross-sectional study was performed on 135 patients, aged between 30 and 55, who had recently been diagnosed with type 2 diabetes. Demographic and socioeconomic characteristics and food insecurity status were assessed using demographic and the 18-item USDA household food security questionnaires, respectively. Physical activity also was evaluated by MET, physical activity questionnaire. Data were analyzed by Chi-square, t-test, and logistic regression.
Results: Prevalence of food insecurity was 66.7%. Food insecurity was significantly associated with economic status, education level, income, having child under 18 years of age, family size, and number of children (p<0.05) however, there was not a significant relationship between food insecurity and occupation, marital status, family history of diabetes, high blood pressure and heart disease, age, and the amount of physical activity.
Conclusion: Considering the higher prevalence of food insecurity in diabetic patients, it can be said that it is likely that the incidence of diabetes is higher in food insecure people.

Abstract
Background: IFN-&lambda3 (IL-28B) is a new cytokine from the type III interferons. Its role in several biologic activities, such as regulation of immune responses, tumor and metastasis, or its functions in HBV and HCV diseases, has been shown. The present study evaluated the frequency of rs8099917 allele polymorphism in IFN-&lambda3 gene in Iranian population.
Materials and Methods: In this laboratory-experimental study, after collecting blood samples from 118 unrelated Iranian subjects, DNA extraction was performed according to the kit protocols. Evaluation of rs8099917 allele polymorphism was done by Nested-PCR and RFLP method using BsrDI and Tsp451 restriction enzymes.
Results: After running the tests and data analysis, the frequencies of GG genotype (2.5%), GT genotype (31.4%), and TT genotype (66.1%) were obtained. Allele frequencies were 71.22% for G allele and 81.78% for T allele.
Conclusion: The results of this study showed that TT genotype and T allele are the dominant frequencies of rs8099917 allele of IFN-&lambda3 gene in Iranian population. Regarding the different established functions of IFN-&lambda3, frequency determination of this variant can be helpful in different studies, designs, evaluations, and treatment procedures in some diseases, such as tumors and hepatitis, in Iranian patients.

Abstract
Background: Saffron (Crocus Sativus) petals consist of, glycosides, flavonoids, and anthocyanins. Due to the increasing use of medicinal plants in treatment of diseases and identifying their side effects on various organs, this study is to evaluate the effects of Crocus Sativus petals on biochemical blood parameters in male rats.
Materials and Methods: In this experimental study, 30 male Wistar rats with the mean body weight of 215±15 g were randomly assigned into five groups of 6 animals each. Control group received normal saline (10 ml/kg) and treatment groups received 75, 150, 220 and 450 mg/kg body weight of saffron petals extract, for 14 days. Body weight measured in days 0, 7, and 14. At the end of experiment, serum levels of liver function marker enzymes such as AST and ALT, total proteins, albumin, creatinine, cholesterol and triglyceride were assessed.
Results: Compared to control group, ethanol extract of saffron petals significantly increased serum albumin and also significantly decreased levels of serum cholesterol and AST (p<0.05). No significant changes were observed on serum total proteins, creatinine, ALT, triglyceride levels and body weight in control and saffron groups (p>0.05).
Conclusion: Increase in levels of albumin and decrease in liver enzymes may indicate the lack of liver damage. Reduction in cholesterol which is probably due to flavonoid extracts of saffron and no effect on body weight can be beneficial in the use of saffron petals.

Abstract
Background: Celiac disease (CD) is an immune-mediated inflammation of the small intestine which caused by sensitivity to dietary gluten and related proteins in genetically sensitive individuals. This research aimed to investigate of comparison big five factors of personality and early maladaptive schemas among celiac people and normal people.
Materials and Methods: The research method was ex post facto and statistical population included all people with Celiac whose 108 patients were selected by convenient sampling and 108 normal people were selected by convenient in 4 universities of nursing, psychological, literature and accounting of Azad University in Karaj city. Both groups responded brief five factors questionnaire of NEO personality characteristics and Yang’s early maladaptive schemas. Data were analyzed by using independent t-test, analysis of variance and Multivariate Analysis of Variance, Ben Frooni test, Landa test Wickelz and SPSS version 19.
Results: Results indicated that from five main factors of personality, score of psycho -lesion of patient with celiac were more than normal people. Also, patient’s scores in early maladaptive schemas (emotional baffling, lack of trust, fault/shameless, social recluse/ strangeness, attachment/inefficiency, vulnerability toward loss or sickness, not self –maturing/ engaged, failure, fitness/modesty, self-denial, insufficient self-discipline, servitude, self –sacrifice/ aggressive criteria, extreme criticize and emotional inhibition) were higher in comparison with normal people.
Conclusion: This study revealed that according to influence of early maladaptive schemas on celiac sickness, psychological training courses and schema therapy in order to improve emotional and cognitive patterns of these patients.

Abstract
Background: Staphylococcus aureus is one of the important pathogens can cause infections in hospitals specially in intensive care units (ICU). It seems that nasal carriage is important risk factor for developing l infection at ICU units. This study was designed to investigate the frequency of S. aureus nasal carriage and its virulence in patients admitted to ICU units in Vali-Asr hospital at Arak university of medical sciences.
Materials and Methods: In this cross-sectional study, nasal swab samples were obtained from all the patients hospitalized in ICU unit of Vali-Asr hospital from July-December 2014 at admission. After identification, methicillin resistance and the frequency of pvl and acme genes determined by PCR. Demographic data were collected using questionnaire and were analysed using SPSS 20 software.
Results: Out of 390 patients, 81(20.8%) and 31(12.9) patients had been colonized with MSSA and MRSA, respectively. The result of culture was negative for 278 patients (71.3%). 77.4% of MRSA and 54.3% of MSSA isolates were positive for acme gene. Also, 11.11% of and 6.45% of MSSA MRSA isolates were positive for pvl gene.
Conclusion: The application of medical supportive devices like cv line, ventilator, history of surgery operation and antibiotic use significantly was associated with S. aureus nasal carriage. High prevalence of S.aureus shows that these bacteria settled in the hospital. Hence, infection control measures must be performed to reduce the risk of hospital infection.

Background: 4cure models are a model to analyze survival data that these models exist for long term survivors. Cure models are a special type of survival model where it is assumed that there are a proportion of subjects who had never event, thus, survival curve will eventually reach a plateau. Therefore, standard survival models are not appropriate because they do not account for the possibility of cure.The aim of the present research is to apply non-mixture cure model to analyze survival of patients with colorectal cancer.
Materials and Methods: We studied 232 patients with colorectal cancer who were visited and treated at Taleghani Hospital Research Center for Gastroenterology and Liver Disease in Tehran. These patients were diagnosed from 1987 to 2012 and followed up until 2013. The Effect of age, gender, family history, body mass index and site of infection were studied. Kaplan-Meier and Non-Mixture cure Model were used for analzing data.
Results: The ten-year survival rate after diagnosis in the studied patients was 64 % .A total of 60 (25.8 %) deaths due to colorectal cancer were observed. The mean of age at the time of diagnosis was 51.6 years. Based on non-mixed cure model, the rangs of age was 45-65 years old and BMI were significant.
Conclusion: When the population is divided into two groups (susceptible and non- susceptible individuals), using Cox semi-parametric model is not appropriate. Therefore, we should use cure models.

Background: Tympanometry is one of the most important audiological tests for the assessment of neonatal hearing screening programs. This test is usually done using a 226 Hertz probe tone; however, in infants less than 6 months of age, using this probe tone frequency is not reliable due to the different characteristics of the middle ear compared to adults. The purpose of this study was to assess the obtained tympanometric data in neonates 1 to 14 days with normal hearing using 226, 678, 800 and 1000 Hertz frequency probes tones.
Materials and Methods: In this cross-sectional study, 30 neonates aged 1 to 14 days who were diagnosed as normal in hearing aspect with using otoacoustic emissions test, were assessed by tympanometry. Tympanometric tests were performed using 226, 678, 800 and 1000 Hertz frequency probe tones for both ears. For data analysis, the T-paired method has been used.
Results: With using 226 Hertz frequency probe tone, incidence of unusual double peak tympanograms was observed in 65% of cases, which with increasing frequency of the probe tone, this percentage was decreased. There was also a statistically significant difference between the static admittance peak compensated in two 226 and 1000 Hertz frequency probes tones.
Conclusion: In neonates with normal hearing and without any risk factors for hearing loss, the 1000-Hertz Tympanogram shows a better middle ear status than the 226-Hertz tympanogram.

Abstract
Abstract
Background: Today, discovery and production of new antimicrobial drugs has been emphasized due to the growing of antimicrobial resistance. The purpose of this study was to screen out antimicrobial producing bacteria among halophilic or halotolerant Gram-positive endospore-forming bacteria isolated from different areas of Iran.
Materials and Methods: 62 strains were isolated from salin lakes of Iran, endospore-forming ability was evaluated and further identification of strains was done using 16S rRNA gene sequencing. Screening test was performed using two-layer agar diffusion method in which the indicator strains, Bacillus cereus (ATCC 14579) and Escherichia coli, (PTCC 1330) were inoculated in the seed layer. Finally, the production of antimicrobial active agent during a period of 7 days was studied followed by evaluating the effect of base-layer agar concentration on the dissemination of antibacterial metabolite.
Results: Isolates WT6, R4A19 produced an agent(s) which inhibited the growth of both B.cereuse and E.Coli. The inhibition zone against only E.Coli was observed when R4A20 strain had been cultured in the base layer, while four non-bacillus strains (R4S2, LbS2, RF1 and WT19) could inhibit the growth of B.cereuse. The antibacterial compound production of WT6 against Bacillus cereuse and E.Coli reached to its optimumm leved after 3 and 4 days respectively, while R4A20 produced the active substance, optimally, after 5 days. No significant difference effect on diameter of zone inhibition was observed among various base-layer agar concentrations.
Conclusion: Halophile or halotolerant endospore-forming bacteria isolated from different areas of Iran possess a potential to be considered as interesting microorganisms for further antimicrobial research studies.

Abstract
Abstract
Background: Alzheimer’s disease (AD) is a most common neurodegenerative disorder. The aim of this study was to investigate the effect of probiotic on cognitive function in patients with Alzheimer disease.
Materials and Methods: This clinical trial was conducted among 48 AD patients. The patients were randomly divided into two groups (n=23 in control group and n=25 in probiotic group) treating with capsules 500mg containing maltodextrine (control group) and probiotic supplementation (probiotic group) for 12 weeks. Mini-mental state examination (MMSE) and TYM test score was recorded in all subjects before and after treatment.
Results: After 12 weeks intervention, compared with the control group, the probiotic treated, patients with mild degree of Alzheimer disease showed an improvement in the MMSE, TYM score
(p < 0.0001).
Conclusion: Our current study demonstrated that probiotic consumption for 12 weeks positively affects cognitive function in mild degree of AD.

Abstract
Abstract
Background: Early diagnosis of prenatal illnesses and timely treatment of congenital anomalies has been the mainstay of the health system. In this study, our aim is to provide Roc curve multivariate modeling in detection of fetal abnormalities using associated markers in screening Down syndrome in the first and second periods of pregnancy.
Materials and Methods: This is a descriptive-analytical study that uses information from two sets of data. In the first set, 152 individuals, who had the results of the first- trimester and second screening tests at risk and in the second group, 75 individuals with normal results. The studied variables included the serum markers in the first- trimester and the second- trimester screening, auxiliary variables (includes demographic information). Statistical analysis was performed by using ROC regression, incremental value analysis and Stata 12 software.
Results: In evaluating the value of each diagnostic test in the presence of auxiliary variables using logistic regression and rock curves, the results generally showed that in screening the first- trimester of PAPP-A and in the screening the second-trimester,Inhibin-A can be used alone as a diagnostic test.
Conclusion: Best diagnostic test in the first- trimester, respectively, PPAP-A, NT, FREE B-HCG and in the second- trimester of screening, respectively, Inhibin-A, HGG, UE3 and AFP were based on the area under the ROC curve. In addition, the most significant effect of the predictor variable on the outcome of the diagnostic test was family history.

Abstract
Background and Aim: From the viewpoint of Iranian Traditional Medicine (ITM), hemorrhoidal bleeding is not only an organic disease; it also indicates the quality and quantity of blood and temperaments in the body. According to this view, bleeding from different areas has several causes, two important causes of which are: a. vascular hyperemia and consequent blooding in bleeding-prone areas, and b. changes in blood quality that cause the arteries to open and bleed. This study, by reporting a case of hemorrhoidal bleeding, is an evidence of some therapeutic principles in ITM.
Case Report: Patient was a 39-year-old married woman with four children and warm-wet temperament complaining of heavy menstrual bleeding for one year. After three months of herbal drug administration, menstrual bleeding became normal. One month later, she had hemorrhoidal bleeding. Her bleeding was treated based on ITM method by removing the black bile (Soda) from the body, phlebotomy and leech therapy around the anus.

Conclusion: Based on ITM, it seems that the cause of hemorrhoidal bleeding in this case is vascular hyperemia and poor blood quality following symptomatic treatment of heavy menstrual bleeding.

Abstract
Background and Aim: The success of endodontic treatment depends on the complete elimination of pathogenic bacteria inside the root canal. Enterococcus faecalis, a gram-positive, pathogenic, and anaerobic cocci, are the primary etiology of root canal infection in treated and untreated teeth and are highly associated with endodontic treatment failure. This study aimed to compare the antimicrobial activity of Sodium hypochlorite 2.5%, microemulsion of Myrtus 10%, and microemulsion of Thyme 0.6% on the E. faecalis after root canal filling.
Methods & Materials: In this experimental study, 25 extracted single canal human anterior teeth were collected, and their crowns were cut. Then after washing and cleaning, and filling, the sterilization was done using an autoclave. Then, except 5 teeth (negative control group), the teeth were smeared with E. faecalis and then divided into 5 groups randomly represented the irrigation by Sodium hypochlorite 2.5%, microemulsion of Myrtus 10%, and microemulsion of Thyme 0.6%, with a positive control group and negative control group. The teeth were filled with gutta-percha and then incubated in an incubator for 90 days at a temperature of 37°C. Finally, the comparison of CFU in the groups due to abnormal data distribution was performed using the Kruskal-Wallis test.
Ethical Considerations: The Ethics Committee of Arak University of Medical Sciences approved this study (Code: IR.ARAKMU.REC.1398.297).
Results: According to the results of this study, the correlation between the groups of cultured teeth was not significant(P>0.05). The specimens washed with microemulsion of the Thyme 0.6% had the highest positive number of cultures; the specimens washed by Sodium hypochlorite 2.5% had the lowest number of cultures.
Conclusion: This study showed that no statistically significant difference exists in their antimicrobial activity after root canal therapy after the use of different wash solutions.
