Abstract
Background: Bartter syndrome is renal tubular disorders that inhibit salt transport and increased renal salt wasting. Type II Bartter syndrome is caused by mutations in the KCNJ1 gene which encodes the inwardly rectifying ATP-sensitive potassium channel Kir1.1 (ROMK). They play a vital role in secretion of potassium into the tubule lumen. The effects of mutation at position 338 of ROMK2 (Kir1.1.b) was investigate.
Materials and Methods: Site-directed mutagenesis was used to substitute of threonine for methionine at position 338 of ROMK2 (Kir1.1.b). M338T mutant ROMK2 expressed in oocytes of Xenopus laevis, and in a non-polarized mammalian cell line (MDCK). Two electrode voltage clamp and were used to measure oocyte ROMK-dependent currents. Confocal microscopy of EGFP-tagged ROMK2 determined express and distribution of these channels in MDCK cells.
Results: The M338T mutant ROMK2 protein expressed in oocytes was functionally identical to wild type. Its cellular distribution was different in polarized and non-polarized MDCK cells.
Conclusion: The M338T mutation is altered residue interactions within the carboxyl terminus of ROMK2 channels. Thus mistargeting of ROMK2 in vivo reduces the driving force for potassium secretion in the TAL and reduces salt reabsorption by this nephron segment.

Abstract
Background: The aim of this study is to assess antioxidative and pro-oxidative efficacy of ascorbate on serum albumin during iron-induced oxidative stress. Materials and Methods: In this experimental study, albumin was placed in the oxidative system containing iron ions and different concentrations of ascorbate. To monitor albumin degradation, sodium dodecyl sulfate polyacrylamide gel electrophoresis was performed according to Laemmli procedure. Oxidative modification of albumin was demonstrated using a method for determination of carbonyl groups by 2,4-dinitrophenylhydrazine. Results: By applying the carbonyl assay, ascorbate showed a dual effect: initial pro-oxidative effect on albumin changed to an antioxidant one in a dose-dependent manner. Our findings showed prooxidant effects for ascorbate in low concentrations (0-100 µM) and antioxidant effects in higher concentrations (100-300 µM). Also, electrophoretic pattern of plasma proteins showed significant protein aggregations in the range of 35 to 45 kDa of MW and protein degradations in the range of 115 to 180 kDa. Conclusion: Ascorbate can produce reactive oxygen species and can also inhibit the production of these oxidants in the presence of iron ions as well. These findings may be directly applicable to oxidative states during the administration of ascorbate and may be important in preventing oxidative modifications of proteins in blood circulation and other biological fluids.

Abstract
Background: Mucopolysaccharidoses disease type VI (MPS-6) or Marteaux-Lamy syndrome is an autosomal recessive lysosome storage disease which is caused by aryl sulfatase B enzyme deficiency. Because of the accumulation of abnormal metabolites in various tissues of body due to enzyme deficiency, different clinical manifestations from mild to severe occur in the patients. Case: This study reports a case of mucopolysaccharidoses disease admitted with fever, dyspnea, and pneumonia. The patient had coarse facial features, protruding tongue, dwarfism, and abdominal distension. In the urine sample, a significant amount of drmatan sulfate metabolite was found and peripheral blood smear testing indicated large and abnormal inclusions in peripheral blood neutrophils. Leukocyte aryl sulfatase B activity decreased and spine radiography reported multiple skeletal abnormalities. Differential diagnosis with other mucopolysaccharidoses indicated Marteaux-Lamy.

Abstract
Background: Platelet glue is a human plasma product that is rich in platelets growth factors and fibrinogen and has haemostatic and healing properties. In the present study, the effect of fibrin sealant rich in platelet growth factors on healing burn wounds was evaluated.
Materials and Methods: This randomized clinical trial was done on 50 burn patients divided into two equal groups and followed for one month. Both groups received antibiotics during the treatment. Granulation tissue formation and epithelialization were the tokens of wound recovery and positive response to treatment. Data analysis was done by Fisher’s exact test, Mann-Whitney test, and t-test using SPSS software version 16.
Results: The mean of time to complete healing was significantly different in intervention and control groups (p=0.001). Time to recover 100% was 11.5±2.2 days in the intervention group and 16.2±5.3 days in the control group. The intervention group healed about 1.4 times faster than the control group.
Conclusion: Autologous platelet gel can be used as a new therapeutic approach in healing burn wounds. With this product, burn wounds can be expected to heal more quickly.

Abstract
Background: Fluoxetine or Prozac is a serotonin reuptake inhibitor (SSRI). Considering the importance of this drug for the treatment of neurological disorder, such as anorexia and depression its side effects on the endocrine axis of body are of significance. Hence, the present study was conducted to evaluate the effects of fluoxetine on cortisol and thyroid hormone levels and body weight in male rates.
Materials and Methods: In this experimental study, 30 adult male rats (230±20 gr BW) were randomly divided into 3 equal groups: sham, control and treatment. Rats in the control group were kept in normal conditions in animal house, whereas treatment and sham groups were, respectively, injected 32 mg/kg BW of fluoxetine and 0.9 ml of normal saline (i.p) for 35 days. During this time, body weight of all animal was measured and after 35 days, blood was collected by heart puncture and separation of serums to evaluate T3, T4, fT3, fT4, and cortisol hormones through RIA method. The results were statisticaly evaluated by one-way ANOVA test.
Results: Taking fluoxetine for 35 days significantly decreased the level (p&le0.05) in serum concentrations of Ft3, fT4, T4 and cortisol hormones compared to the control and sham groups. However, no significant differences were observed in the serum concentration of T3 hormone in treatment group compared to the the control group. The drug also caused a significant decrease in the average weight of rats in the treatment group compared to the control group (p&le 0.05).
Conclusion: By affecting the activity of different levels of hypothalamus-pituitary-adrenal hormones axis, fluoxetin decreases the level of cortisol hormones. It also reduces the activity of the thyroid gland this is probably due to the increased prolactin secretion through inhibiting TRH secretion and reducing the production of TSH and thyroid hormones

Abstract
Background: Peripheral nerve injures are common in clinical practice that includes from compression injury to complete disruption of nerve trunk. After injury nerves can grow and repair spontaneously, early end to end anastomosis is first choice for repair but only 50% of nerve fiber would be physiologically accurate and effective, thus several techniques for repair of nerve injury are introduced including combination of nerve transplant with silicon tubes, use of biologic and synthetic tubes for nerve conduit.
Materials and Methods: In this Exprimental study, 20 rats with the same weight were selected and kept in the same condition .The left sciatic nerve of them was cut and were divided in two groups.In the first group the facial flap method for repair of nerve injury was used .In the second group vein conduit method was used as standard method. Histological nerve repair was assessed after 45 days and the intact right sciatic nerve was used as control.
Results: Repair of nerve injury in proximal, mid and distal portion of nerve in facial flap method was more effective than vein conduit (P<0.05). Repair of nerve injury in proximal, mid and distal portion had no difference compared with control group (p>0.05).
Conclusion: Repair of nerve injury with facial flap method is more effective than standard method and can be used as a new method for nerve repair.

Abstract
Background: Wharton's Jelly Mesenchymal Stem Cells (WJMSc) are potential renewable source of cells in replacement therapies of many diseases. Different biomaterials have been used as a scaffold to mimic the stem cell niche, which is important for promoting cellular interactions, cell proliferation and differentiation. Encapsulation involves entrapment of living cells within the semi-permeable membrane for the exchange of nutrients, oxygen and stimuli, whereas antibodies and host immune cells are kept out. In this study, a new approach for culturing and differentiating Wharton's Jelly Mesenchymal Stem Cells to definitive endoderm in a three dimensional environment using alginate capsules is presented.
Materials and Methods: In this experimental study, Wharton's Jelly Mesenchymal Stem Cells were capsulated and Trypan blue exclusion method was applied to determine cells viability. Then, encapsulated cells have been cultured in medium contain differentiating factors and to investigate the expression of definitive endoderm related genes, Real- time PCR was performed.
Results: The encapsulation procedure did not alter the morphology and viability of the encapsulated cells. Post-differentiation analysis confirmed the expression of FOXa2 and SOX17 as definitive endoderm specific markers.
Conclusion: Alginate has potential to be used as a three dimensional scaffold for culturing and differentiation of WJMSCs to definitive endoderm.

Abstract
Background: Brucellosis is a major cause of zoonosis disease and is endemic in hamadan Province in Iran. The purpose of this study was to isolate Brucella species from brucellosis patients and identify different species of this bacterium in order to determine the prevalence of the species.
Materials and Methods: This study was descriptive- cross sectional and fifty blood samples were obtained from brucellosis patients with clinical symptoms. The samples were cultured in BACTEC system and incubated for 14 days. Then, the samples were cultured on Brucella agar and biochemical tests were done for identification of bacteria. Finally, Polymerase Chain Reaction (PCR) applied for confirmation and isolated identification with specific primers.
Results: Seven Brucella strains were isolated from 50 blood samples of the patients with brucellosis by blood culture and PCR. The PCR results on blood specimens showed 4 positive in spite of the negative results of blood culture. PCR and biochemical methods revealed that all the 11 isolated bacteria were Brucella Melitensis.
Conclusion: This study was designed to evaluate PCR technique as a diagnostic tool for brucella spp in comparison to conventional techniques. This study showed a high prevalence of brucellosis due to Brucella Melitensis in Hamadan Province and efforts in this region should be aimed at the eradication of this bacterium.

Abstract
Background: Gentamicin is an aminoglycoside antibiotic that broadly is used to treat gram negative bacteria infections, although it has side effects such as nephrotoxicity. According to antioxidant, anti-inflammatory and vasodilatory properties of Zataria Multiflora, the effects of co-treatment with zataria Multiflora and hydroalcholic extract on gentamicin induced nephrotoxicitj were investigated.
Materials and Methods: In this study, male rats of Vistar race were divided into 4 groups: control group, co-treatment with gentamicin and vehicle group, co-treatment with gentamicin and zataria Multifiora extract group, and co-treatment with zataria Multiflora extract and normal saline solution group. Zataria Multiflora hydroalcoholic extract was added to drinking water as 800 PPm concentration. They, systolic blood pressure and renal blood flow (RBF) were measured. Also, the amounts of urea, creatinine, sodium, potassium and osmolarity were measured in plasma and urine samples
Results: In co-treatment group with zataria Multiflora extract, the amounts of urea, creatinine, absolute sodium excretion and relative sodium and potassium excretion and malondialdehyde (MDA) that have been inceased in treatment with gentamicin, significantly were reduced. Creatinine clearance, urine osmolarity, RBF and FRAP that was decreased in gentamicin group in compare to control group, significantly increased.
Conclusion: Co-treatment prevents nephrotoxicity induced by gentamicin and attenuates oxidative-stress associated renal injury by reducing oxygen free radicals and lipid peroxidation, So it can be effective to cure rats receiving gentamicin.

Abstract
Background: The prevalence of constipation in childhood is 0.7% to 29.6%. Inorganic causes are considered as the most common cause of constipation in children. Polyethylene glycol (PEG) is the most effective and safest laxatives which is usable in children in the long-term. This study was conducted to evaluate and compare the effect of PEG treatment and probiotic bacilluscoagulans and bifidobacterium and probiotic to enhance the growth of probiotics.
Materials and Methods: In this double blind clinical trial study(interventional study), 120 children aged 2-16 years with chronic functional constipation enrolled the study based on ROME III criteria. The bowels became empty by using liquid paraffin. Then, children were randomly divided into three groups of polyethylene glycol+placebo and Probiotics+placebo and polyethylene glycol+probiotics recipients. After 6 weeks of treatment, treatment success rates between the three groups were compared before and after the study Data analyzed by SPSS 20 software.
Results: The results of the study conducted on the three groups for treatment of constipation showed that the effect of polyethylene glycol and probiotic on treatment was considerable before and after treatment but the levels of effect was similar in the comparison of groups. The simultaneous use of two drugs was more effective than a single drug.
Conclusion: Overall, the results of our study showed that concurrent use of polyethylene glycol and probiotics is much more effective in the treatment of constipation.

Abstract
Background: Many of key problems in students with attention deficit disorder are related to executive dysfunction that in this field neurofeedback training and computer games are effective. This study aimed to compare the effectiveness of neurofeedback training and computer games on continuous attention and planning ability in students with attention deficit disorder.
Materials and Methods: This study was a quasi-experimental with a pre-test and post-test design and with a control group. The statistical population was included all elementary students with attention deficit disorder that referred to counseling centers of Mashhad city in 2013. Totally, 45 students were selected through available sampling method and randomly assigned to three groups. The experimental groups were educated 12 sessions of 60 minutes by neurofeedback and computer games methods. To assess continuous attention and planning, the CPT and Tower of London computerized tests were used, respectively. Data were analyzed by using the SPSS-19 software and multivariate analysis of covariance (MONCOVA) methods.
Results: The findings showed that both methods of neurofeedback training and computer games significantly lead to increase continuous attention and planning in students with attention deficit disorder. Also, there wasn't any significant difference between two methods in continuous attention and planning (P<0/05).
Conclusion: The results showed that neurofeedback training and computer games methods were effective in improving continuous attention and planning for students with attention deficit disorder. Therefore, counselors and therapists can use mentioned methods in treatment of students with attention deficit disorder.

Abstract
Background: The efflux pumps are one of the main mechanisms of resistance to antibiotics in E. coli and K. pneumoniae strains. The aim of this study is to study the association between genes coding efflux pumps AcrAB and Qep A and ciprofloxacin resistance among E. coli and K. pneumoniae clinical strains
Materials and Methods: This study was done on 100 strains of E. coli isolated from Taleghani and Labbafinejad Hospitals and 100 strains of K. pneumoniae isolated from Taleghani and Mofid Children Hospitals. Antimicrobial susceptibility tests were performed by disk diffusion method based on CLSI guidelines. Identification of genes encoding efflux pumps Acr AB and Qep A was done by PCR technique.
Results: In this study, fosfomycin and imipenem had the best effect against E. coli clinical isolates and fosfomycin and tigecycline had the best effect against K. pneumoniae clinical isolates. PCR assay demonstrated that the prevalence of AcrA, AcrB and QepA genes among E. coli isolates were 92%, 84% and 0%, respectively and among K. pneumoniae isolates were 94%, 87% and 4%, respectively.
Conclusion: The prevalence of genes encoding efflux pumps in E. coli and K. pneumoniae clinical strains, which causes resistance to fluoroquinolones, is cause for concern. Therefore, controlling infection and preventing the spread of drug-resistant bacteria needs to manage medication carefully and identify resistant isolates.

Abstract
Background: Gentamicin (GM) is one the aminoglycoside antibiotics which isroutinelyused to treatinfections gram-negative, either alone or insynergistic withbeta-lactamantibioticsused. However, frequent useleads toserious side effectssuch asrenal toxicity, ototoxicity. Coenzyme Q10 has antioxidant, anti-inflammatory and vasodilatory properties. According to these properties of Coenzyme Q10 and tissue damage mechanism in GM induced-nephrotoxicity, in this study, the effects of these two substances for the co-treatment and post -treatment on renal injury induced by gentamicin were investigated.
Materials and Methods: Experiments has been done on 77 male Wistar rats in weight range of 200 to 250 g. Animals were divided randomly into 5 groups of 7 numbers. Renal nephrotoxicity induced by i.p injection of gentamicin (100mg/kg) Therapeutic effect of coenzyme Q10 (10mg/kg)in the two protocols co-treatment and post-treatmentwas investigated.The animals after the last injectionon the ninth day of co-treatment andthe seventeenth day of post-treatmentwere placed into individual metabolic cages so as to collection urine and urine volume was measured gravimetrically. Afteranesthesia, systolic blood pressure and renal blood flow was measured. Then blood sampling was done. Amount of urea, creatinin, sodium, potassium and osmolarity was measured in plasma and urine samples. Left kidney, for doing histological experiments in 10% buffered formaldehyde and right kidney for biochemical experiments in fluid nitrogen was preserved.
Results: Co-treatment with Coenzyme Q10 significantly decreased fractional excretion of sodium (6.37±1.33 %; p<0.001) and decreased fractional excretion of potassium(219.14±83.8 %; p<0.001) MDA levels (2.13 ±0.24µmol/gkw; p<0.001), and significantly increased renal blood flow (6.38 ±0.1ml/min: p<0.01) and FRAP levels (24.44±0.42mmol/gkw; p<0.001). Post-treatment with coenzyme Q10 significantly decreased fractional excretion of sodium (3.58 ±0.57 %; p<0.001), potassium (111.77±29.4%; p<0.001) and MDA levels (3.08 ±0.12µmol/gkw; p<0.001) and significantly increased renal blood flow (6.74±0.15ml/min: p<0.001) and FRAP levels (24.34±0.75mmol/gkw; p<0.001) that is reduced by gentamicin.
Conclusion: According to the results, this study showed thatpost- treatment with coenzyme Q10more protective effect on the kidney tissue andAnda greater increase inantioxidant defensecreated.

Abstract
Abstract
Background: Polycystic ovary syndrome(PCOS) is the most common endocrine aberration in women. PCOS is characterized by ovarian hyperandrogenism and anovulation resulted from a disorder of follicular maturation. Apoptosis is a regulatory mechanism for oocyte maturation and survival. Several studies have shown a possible role of Fas in ovarian apoptosis. The present study is the first investigation to examine the possible association of Fas rs1800682gene polymorphism with PCOS risk in Iranian women.
Materials and Methods: This case-control study was conducted on 251 patients with PCOS and 213 healthy control women. The Fas rs1800682 gene polymorphism genotypes were analyzed using the Tetra-ARMS-PCR method. Also, logistic regression analysis was used to investigate the association between genotypes and PCOS risk.
Results: There was a significant association between A allele and susceptibility to PCOS(OR =1.4, %95CI=1.08-1.83, p=0.011). Moreover, in the recessive genetic model for A allele, the AA genotype increased the risk of PCOS after adjusting age and body mass index(OR=1.6, %95CI=1.02-2.51, p=0.041).
Conclusion: For the first time, this study showed that Fas rs1800682 polymorphism is associated with PCOS risk in Iranian women and the A allele may act as a recessive allele for increasing the risk of PCOS.

Background: The present research aimed to measure the effectiveness of Eye Movement Desensitization and Reprocessing (EMDR) on reducing the severity of symptoms of post-traumatic Stress Disorder (PTSD) and Stuttering in a four-year-old child post-traumatic stress disorder in children is one of the disorders relating to trauma and stressful factors, also, stutterring or word fluency disorder is a nerve growth disorder.
Materials and Methods: This research design was conducted as a case study with a multiple baseline design. Participant in this research was a four-year- old child with Diagnostic Posttraumatic Stress Disorder and Psychogenic Stuttering. He had been placed under the intervention with EMDR at Shahid Birjand Counseling Center. The instruments used for this research included demographic researcher-made questionnaire, the third edition of stuttering severity test, the scale of parents’ report of post-traumatic Symptoms and child’s report of posttraumatic Symptoms. Data analysis was done through graphic and descriptive analysis. The data was collected as base line and during the treatment as well as after the treatment and follow-up (in terms of 3 and 24 months).
Results: Means percentage improvement (MPI) to reducing the severity of post-traumatic symptoms was achieved as %74.66 and it was %56.06 for reduction of the severity of stuttering and they continued to maintain in the follow-up period.
Conclusion: Results showed that EMDR method had affected on reduction of the severity of post-traumatic stress disorder symptoms and stuttering intensity.

Abstract
Abstract
Bakcground: Application of Talc powder, Bleomycin and Tetracycline is among conventional treatments in recurrent malignant pleural effusion caused by malignant pleurodesis. The aim of this research is to estimate the effect and safety in application of betadine as a cheap and available chemical factor in treatment of pleurodesis.
Materials and Methods: In this semi-empirical study, all patients suffering from recurrent pleural effusion enrolled the study. In order to perform pleurodesis, after chest tube insertion, solution containing iodopovidon, lidocaine, and normal saline was injected into the pleural cavity through chest tube. All the patients were examined for the pleural effusion and pleurodesis recurrence during a
6-month follow-up.
Results: In this study, a total number of patients was 50 among whom 23 were male (46%) and 27 were female (54%). In the 6-month follow-up, 40 patients (80%) were completely recovered and the recurrence of the pleural effusion was noticed in 10 patients (20%). Comparison of the success rate of recovery in terms of the gender of the patients showed no significant relation between gender and response to the recovery process (p=0.219). Of the total of 32 patients (64%) with chest pain after pleurodesis, 23 patients had complete recovery, and the incidence of chest pain in patients with no improvement was significantly higher (p=0.018).
Conclusion: Iodopovidone is a cheap, highly available material and of high efficacy in comparison to other sclerosant chemical substances, that provides an appropriate option in treatment of malignant pleural effusion.

Abstract
Background and Aim: The aim of this study was to evaluate the success rate and complications of Ahmed Glaucoma Valve (AGV) insertion during a follow up of at least 5 years.
Materials and Methods: In this retrospective case series study, patients with 5 years of follow up after AGV insertion were enrolled. Success was defined as intraocular pressure (IOP) < 21 mm Hg (criterion A) and IOP < 16 mm Hg (criterion B), with at least 20% reduction in IOP, either with no medication (complete success) or with no more than preoperative medication (qualified success). Cumulative success was defined as sum of qualified and complete success.
Findings: This study included 40 eyes of 40 patients with mean age of 40.63± 22.91years, (range 1 to 88). Cumulative survival success rates were 90%, 73% at 1 and 5 years after surgery according to criterion A and 42%, 25% based on criterion B with the median survival time of 60 months and 22.36 months, respectively. Complications were observed in 4 eyes of 4 patients (10%) and included wound dehiscence, choroidal detachment, encapsulated bleb and lid retraction. There were not any early complications such as hypotonia or hyphema.
Conclusion: The present study was one of the largest series that reported the long-term outcome of AGV implantation in Asian patients with refractory glaucoma, and showed the wide difference of success rate between criterion A and B.

Abstract
Background and Aim: The aim of this study was to present a conceptual model of spiritual well-being in prevention of substance abuse in adolescent.
Materials and Methods: The present study was done with a qualitative meta-synthesis with systematic review of the studies present in the foreign and Iranian journals about spiritual well-being and the role of them in prevention of substance abuse in adolescents. The population of this study consisted of non-interventional archive studies that were done during 1986 -2016 in Oxford Journal, Science Direct, Wiley Online Library, Taylor&Francis, ProQuest, Springer, Iranmedex, Irandoc, SID and Magiran. Keywords were including of spiritual well-being, spirituality, prevention of substance abuse and adolescents.
Ethical Considerations: This study with ethics code 8797/1129 has been approved by Farabi Campus of University of Tehran.
Findings: After reviewing and evaluating the quality of the research, eventually 73 papers and theses included 8 Persian articles, 65 English papers including 11 theses and 54 articles were found and dissertations and finally conceptual model based on fisher (1998) theory's, wree presented. On this ground, subcategory of spiritual well-being was consisted of three theme of relationship with others, relationship with self and relationship with God. Each of them consisted of two subtheme of psychological health (interpersonal and intimate) and psychological well-being(interpersonal and intimate).
Conclusion: The results showed that spiritual well-being influenced on multiple dimensions of adolescent psychological health and well-being and prevention of adolescent substance abuse from this way.

Abstract
Background and Aim Gentamicin antibiotic has some side effects such as nephrotoxicity. The aim of this study was to evaluate the post-treatment effects of using hydroethanolic extract of Origanum Vulgare (OV) on nephrotoxicity caused by gentamicin.
Methods & Materials In this study, 32 male Wistar rats were divided into four groups of control (n=8), gentamicin (n=8; 100 mg/kg/day intraperitoneally for 8 days and gavage of distilled water for 2 days), OV extract group (intraperitoneal injection of normal saline for 8 days and using 40 mg/kg OV extract by gavage for 2 days), and gentamicin+ OV extract (intraperitoneal injection of gentamicin 100 mg/kg/day for 8 days and using 40 mg/kg OV extract by gavage for 2 days). The concentration of urea, creatinine, sodium, potassium and osmolarity were measured in plasma and urine samples. The right kidney was used to measure Malondialdehyde (MDA) and Ferric Reducing Antioxidant Power (FRAP).

Results Post-treatment administration of hydroethanolic extract of OV significantly decreased the concentration of urea, creatinine, absolute sodium excretion, relative sodium and potassium excretion, and MDA levels but significantly increased creatinine, urine osmolality and FRAP levels.
Conclusion Oral administration of OV extract as post-treatment method improved nephrotoxicity caused by gentamicin use by reducing oxidative stress of oxygen free radicals and lipid peroxidation in the affected kidneys.
