Abstract
Background and Aim: Gene mutation in Staphylococcus aureus is one of the most important causes of antibiotic-resistant strains. The High Resolution Melting Curve (HRM) analysis of DNA method can detect these mutations very high quality. The purpose of this study was to evaluate the role of clinical sample type in the occurrence of nucleotide mutations in the mecA gene of S. aureus by HRM method.
Materials and Methods: In this experimental study, 43 clinical isolates of S. aureus were used. To detect possible mutations, isolates with mecA gene were replicated and sequenced. Then, analysis was performed using StepOne Software v2.3 and HRM v3.0.1 software. Sequencing results were used as gold-standard.
Ethical Considerations: This study with research ethics code IR.UMSHA.REC.1396.637 has been approved by research ethics committee at Hamadan University of Medical Sciences.
Findings: Of 43 clinical isolates of S. aureus, 11 isolates (25.58%) had mecA gene and 32 isolates (47.41%) lacked the mecA gene. According to different clinical samples, 3 isolates (27.27%) were resistant to methicillin from blood samples, 2 isolates (18.18%) from urine specimens, 2 isolates (18.18%) from wound samples, 2 isolates (18.18%) of the catheter samples, 1 isolate (9.09%) of the abscess and 1 isolate (9.09%) were separated from the nose swab. In the meanwhile, isolates from the wound and urine had the highest mutation in the adenine amino acid as A → T, A → G, A → C, and
A → X. Isolates taken from blood have mutations in Guanine amino acid as
G → A.
Conclusion: There was a significant relationship between type of mutation and type of clinical specimen in methicillin-resistant Staphylococcus aureus isolates.

Abstract
Background and Aim: In recent years, Multidrug resistance has been increasing among Klebsiella isolates. The aim of this study was to survey existence of integrons and its relation with antibiotic resistance among clinical isolates of Klebsiella.
Materials and Methods: From Jun 2015 to May 2016, 129 Klebsiella isolates collected from Karaj hospitals and laboratories. Statistical population included 80.6% female and 19.4% male. Antimicrobial susceptibility was performed using Kirby-Bauer disk diffusion and ESBLs producer were screened. Integrons were detected using PCR.
Ethical Considerations: This study with research ethics code IR.IAU.K.REC.1396.16 has been approved by research ethics committee at Islamic Azad University of Karaj, Iran.
Findings: The highest and lowest percentage of sensivity were found to ofloxacin (89.1%) and amoxcicillin (6.2%), respectively. 82.9% of isolates were resistant to more than two antibiotics from different classes. Among 129 isolates, 19.3% of the isolates harbour integrons. Frequencies of MDR among integron-positive isolates were 100%. Also, 71.3% and 28.7% of isolates were ESBLs positive and negative respectively.
Conclusion: Results showed integron elements were prevalent among MDR isolates. Integron-associated resistance genes can be served as reservoirs of multi drug resistance within clinical isolates and presence of integron can be used as a marker to identify MDR isolates. Prevalence of ESBLs among clinical isolates of Klebsiella showed that antibiotics like ampicillin or amoxicillin-clavulanic acid are not effecvtive anymore in treatment of UTIs.

Abstract
Background and Aim: Physical activity causes the releases of bone resorption indices in the bloodstream by the mechanical load on bone, which in interaction with adipokines reduces obesity and prevent its complications. So, the present study aims to compare the effects of aerobic and resistance exercise on Anthropometric Indices and osteocalcin, leptin, adiponectin levels in overweight men.
Materials and Methods: A total of 40 overweight young healthy men (BMI 28.67±0.96 and age 31.50 ± 2.23) were randomly assigned to control (n = 14), aerobic exercise (n = 13) and resistance exercise (n = 13) groups. Subjects in the exercise group were on 8-week supervised exercise training programme for three sessions per week (aerobic exercise were performed at 60-85% of HRR, and resistance exercise were performed at 55-75% of 1RM). Osteocalcin and adipocytokines (leptin, adiponectin) were assessed from fasting blood samples before and after the 8-week exercise programme. Data were analyzed by t-test and ANOVA.
Ethical Considerations: This study with research ethics code IR.IAU.RASHT.REC.1396.124 has been approved by research ethics committee at Islamic Azad University, Rasht branch, Iran and also has been registered with code IRCT20180226038876N1 at Iranian Registry of Clinical Trials (IRCT).
Findings: Aerobic and resistance training led to decrease in anthropometric indices (p<0.05), leptin levels (p<0.001, p=0.001) and increase in osteocalcin (p=0.001, p<0.001) and adiponectin (p=0.001, p<0.001), respectively. In addition, the difference between the two training groups was not statistically significant (p>0.05).
Conclusion: Considering to the effects of aerobic and resistance exercise on osteocalcin and adipocytokines level, it seems that both exercise methods can be increasing mechanical load on bone mass and cause to change in energy metabolism and body weight and can be an important factor in decrease of obesity complications.

Abstract
Background and Aim: Polycystic Ovary Syndrome (PCOS) is one of the most common endocrine abnormalities that insulin resistance and obesity are among its main causes. The purpose of this study was to investigate the effect of chronic aerobic and resistive exercises along with supplementation of cinnamon on insulin resistance in women with polycystic ovary syndrome in Ahwaz in 2017.
Materials and Methods: In this semi-experimental study, 40 women with aged 18 to 30 years (BMI ≤24) were randomly divided into 4 groups. The intervention group performed 6 weeks, 3 sessions per week with intensity 50 to 75% of maximum heart rate for 50 to 85 minutes of exercise aerobic-resistance. Serum levels of insulin, glucose, and body composition indexes were evaluated. The t-test was correlated and ANCOVA was used.
Ethical Considerations: This study with research ethics code EE/ 96.24.3.77886/Scu.ac.ir has been approved by research ethics committee at Shahid Chamran University of Ahvaz, Iran.
Findings: Data analysis showed that body mass index, fat percentage and insulin resistance were decreased in exercise and supplementation exercises (p≤0.05). Cinnamon was also effective in combination with combined exercises in body composition and insulin resistance index (p≤0.05). Changes in fat free mass were not significant in any of the groups (p≤0.05).
Conclusion: It seems that this decrease in the difference in insulin resistance is more due to the combined exercise, and given the positive effect of cinnamon on reducing BMI, its use may be effective in the long term on insulin resistance.

Abstract
Background and Aim: Regarding the controversy about the effects of atorvastatin on blood glucose control and diabetes, we evaluated the effects of atorvastatin on blood glucose and insulin resistance in the present study.
Materials and Methods: 88 type two diabetic patients (T2DP), were treated with anti-diabetes oral agents were enrolled in a double blind randomized clinical trial, placebo control. Then they were randomly classified into two 44 cases groups. The intervention group took atorvastatin 40 mg daily and control group took placebo for three months and fasting blood glucose (FBS), insulin resistance, HbA1c and lipid profile were measured and compared at the beginning and the end of the study. Data were analyzed using student t-test and paired t-test.
Ethical Considerations: This study with research ethics code IR.ARAKMU.REC.90.108.10 has been approved by research ethics committee at Arak University of Medical Sciences.
Findings: After three months, insulin, insulin resistance and HbA1c decreased significantly in the intervention group than control group, while FBS non-significantly decreased. Total cholesterol and LDL cholesterol (Low density Lipoprotein) were also decreased.
Conclusion: It seems atorvastatin is effective to decrease FBS and insulin resistance in diabetic patients.

Abstract
Background and Aim Exercise improves sensitivity to insulin by reducing some adipokines including chemerin. However, the effect of endurance exercise and high-intensity intermittent exercise on chemerin level and insulin resistance in obese male rats is still unknown.
Methods & Materials In this study 27 male rats were randomly divided into 4 groups of sedentary control (n=6), endurance exercise (n=8), high-intensity intermittent exercise (n=8) and untreated obese control (n=5). Both endurance and high intensity intermittent exercise groups performed exercises for 8 weeks. The plasma level of chemerin was measured by ELISA method.
Ethical Considerations ???
Results The chemerin level was significantly reduced in the endurance group compared to the untreated obese control group (P<0.01), but it had no significant change compared to the sedentary control group. Moreover, the plasma level of chemerin in the high-intensity intermittent group was not significantly different from that of untreated obese control group and sedentary control group (P>0.05) and chemerin level reduction in the endurance group was significant compared to the high-intensity intermittent group (P<0.05). Furthermore, there was no significant difference in the insulin resistance reported in high-intensity intermittent group compared to that of endurance group (P>0.05), but its difference in high-intensity intermittent and endurance training groups was significant compared to that of untreated obese control and sedentary control groups (P<0.05).
Conclusion Both endurance and high-intensity intermittent exercises can reduce insulin resistance which can be due to the reduction of chemerin plasma level and weight loss.

Abstract
Background and Aim: recent years, increase in extended-spectrum β-lactamases (ESBLs) producing Escherichia coli has led to limitations of treatment options. This study aimed to find the frequency of blaTEM and blaPER genes among ESBL producing urinary isolates of E. coli and detect their resistance pattern.
Methods & Materials: From January 2016 to February 2017, 972 urine samples from patients suspected of having urinary tract infections in three main hospitals and laboratories in Karaj were collected. Bacterial identification, antimicrobial susceptibility and ESBL production were performed according to the standard guidelines. Polymerase chain reaction was used for the detection of TEM and PER -lactamases.
Ethical Considerations: This study was approved by the Research Ethics Committee of Islamic Azad University of Tehran Medical Branch (Code: IR.IAU.TMU.REC.1396.274).
Results: Out of 972 samples, 500 cases were culture-positive for E. coli. Thirty-six percent (n =180) of the isolates were determined as ESBL-producer. Among ESBL positive isolates, the most susceptibility was observed in amikacin and imipenem (80 and 60% respectively). Resistance to trimethoprim/sulfamethoxazole,ciprofloxacin, tetracycline and gentamicin was 92.7%, 78.9%, 66.1% and 57.8%, respectively. All ESBL producers exhibited multidrug resistance. Among the ESBL-positive isolates, blaTEM gene was detected in 44.72% (n=85) of the isolates, but the blaPER gene was not found in any of the isolates.
Conclusion: The prevalence of multidrug resistant ESBLs producing uropatogenic E. coli is high. Continues monitoring of ESBL producers and their resistance patterns can help to reduce the spread of such resistant strains in the community.

Abstract
Background and Aim: The present study aimed to investigate the effect of a 10-week resistance training on the serum levels of insulin resistance index (HOMA-IR), the serum levels of liver enzymes, chemerin, and resistin in healthy and type 2 diabetic rats.
Methods & Materials: In this experimental study, 40 Wistar rats with the mean±SD weight of 200±48 gr were randomly divided into 4 groups (normal control, diabetic control, diabetic resistance training, and resistance training). The training groups performed regular resistance exercises for 10 weeks by ladders. Twenty-four hours after the last training session, the blood sample of rats was collected for resistin, chemerin, insulin, and liver enzymes. The obtained data were analyzed using one-way Analysis of Variance (ANOVA), Tukey posthoc test, Analysis of Covariance (ANCOVA), and Bonferroni posthoc test at a significance level of 0.05.
Ethical Considerations: This study was approved by the Research Ethics Committee of Arak University of Medical Sciences, Iran (Code: IR.ARAKMU.REC.1394.329).
Results: Diabetes induction significantly HOMA-IR, the serum levels of liver enzymes, resistin, and chemerin in the diabetic control group, compared to the healthy control group (P<0.05). Resistance training also significantly decreased insulin HOMA-IR, the serum levels of liver enzymes, resistin, and chemerin in the diabetic resistance training group, compared to the diabetic control group (P<0.05).
Conclusion: The obtained data suggested that resistance training improved liver enzymes in type 2 diabetic rats by decreasing the insulin HOMA-IR, the serum levels of chamirin, and resistin.

Abstract
Background and Aim: Tendon is the extracellular matrix of the muscle that mechanically and structurally adapts to the mechanical load. However, the cellular and molecular mechanisms of this adaptation are not known yet. The purpose of this study was to investigate the effect of 6 weeks of resistance training on expression of two Transforming Growth Factor Beta 1 (TGF-β1) and myostatin genes in the tendon of fast- and slow-twitch muscles including Extensor Digitorum Longus (EDL) and Soleus (SOL).
Methods & Materials: Twelve male Wistar rats with 8 weeks of age were randomly divided into two groups of exercise (n=6) and control (n=6). The exercise group performed resistance training (Carrying weights with 40-160% body weight on the ladder) for 6 weeks, 5 sessions per week. Forty-eight hours after the last training session, all rats were sacrificed and the tendons of SOL and EDL muscles were extracted. The mRNA expression level of TGF-β1 and myostatin genes was assayed using real time polymerase chain reaction. Independent t-test was used for statistical analysis.
Ethical Considerations: All experiments on animals were according to the ethical guidelines of Research Ethics Committee of Islamic Azad University, Central Tehran Branch (ethical code: IR.IAU.PS.REC.1398.296)
Results: The expression of TGF-β1 gene in EDL (P≤0.001) and SOL (P≤0.01) muscle tendons significantly increased, while the expression of myostatin gene in EDL (P≤0.001) and SOL (P≤0.05) tendons were significantly reduced.
Conclusion: Resistance training appears to up-regulate the basal levels of TGF-β1 gene and down-regulate the basal levels of myostatin gene in tendons of fast- and slow-twitch muscles, where these effects are significantly more pronounced in the tendon of fast-twitch muscle.

Abstract
Background and Aim: Coronary Heart Disease (CAD) is one of the most important causes of mortality. Exercise activities after coronary artery bypass surgery increase the density of myocardial capillaries called angiogenesis and improve cardiovascular function. The aim of the present study was to determine the effect of aerobic-resistance training on plasma basic fibroblast grown factor levels in post-coronary artery bypass grafting patients.
Methods & Materials: The sample consisted of 30 men aged 45-60 years who underwent bypass surgery. They were randomly divided into experimental (n=15) and control (n=15) groups. The experimental group performed aerobic resistance training for eight weeks (3 sessions per week) and the control group did not. Blood samples were taken 24 hours before the start of the first training session and 48 hours after the last training session while all subjects were fasting. In vitro ELISA method was used to measure BFGF. Independent and paired t-test and Shapiro-Wilk test were used to analyze the data at the significant level of 0.05 in SPSS v. 21.
Ethical Considerations: This study was registered (Ethics Code: IR.IAU.NEYSHABUR.REC.1398.01) in the Ethics Committee of Islamic Azad University, Neishabour Branch, and with Clinical Trial Code: IRCTID:IRCT20191228045919N1.
Results: Eight weeks of aerobic-resistance training significantly increased bFGF levels in the exercise group compared to the control group (P=0.002). Also, pre and post changes in the experimental group showed a significant increase in plasma bFGF levels (P=0.002), which was not significant in the control group (P=0.758).
Conclusion: According to the results, it may be concluded that combined (aerobic resistance) exercise may increase angiogenesis and capillary density in post-CABG patients by increasing plasma bFGF levels

Abstract
Background and Aim: Coronary Artery Disease (CAD) is one of the leading causes of death and mortality in today's societies. Physical activity increases some of the influential factors for this disease. The purpose of this study was to investigate the effect of 8 weeks of aerobic and resistance training on endostatin in patients with Coronary Artery Bypass Graft (CABG).
Methods & Materials: The study participants were 24 male patients who were randomly divided into the experimental (n=12) and control (n=12) groups with Mean±SD age of 55.37±6.90 years, weight 75.45±5.87 kg, height 173.27±3.36 cm and body mass index of 25.11±1.55 kg/m2. The experimental group performed 8 weeks of aerobic and resistance training (3 sessions per week and 1.5 hours per session) based on the measurements, while the control group did not exercise during this period. To measure endostatin concentrations, blood samples were taken 48 hours before and 48 hours after the last training session while all subjects were fasting. Data were analyzed using the Shapiro-Wilk test to normalize the data and Student t-test in independent and correlated groups at the significant level of 0.05.
Ethical Considerations: This article was ethically approved by Azad University of Neyshabur (Ethics Code IR.IAU.NEYSHABUR.REC.1398.018) and with the Clinical Trial Code IRCT20191228045916N1 in the Iranian Registry of Clinical Trials.
Results: The present study showed that the aerobic and resistance training group significantly decreased endostatin concentrations (P=0.001) (t=1.672) compared with the control group.
Conclusion: Based on the findings, aerobic and resistance training decreases endostatin concentrations, known as an anti-angiogenic factor.

Abstract
Background and Aim: Coronavirus Disease 2019 (COVID-19) pandemic has reduced the physical activity of people worldwide. The purpose of this study is to provide a practical guide for performing physical activities at home during the COVID-19 pandemic.
Methods & Materials: The study was conducted by using library method and reviewing the books and articles on home-based physical activity, to provide a scientific basis for performing physical activity at home. In this regard, various physical activities with and without equipment at homes were examined.
Ethical Considerations: This article is a review article with no human or animal sample.
Results: Home-based practical suggestions for staying active and cope with its physical and psychological consequences of the COVID-19 pandemic were reported walking or running on a treadmill, the use of stationary bikes, and body weight resistance exercise, in addition to aerobic exercise.
Conclusion: This proposed physical activity programs can be used during the pandemic at home by people with sedentary lifestyle.

Abstract
Background and Aim: Fungal infections are among the most critical and common issues for hospitalized patients, especially in intensive care units. This study aimed to determine the fungal contamination of indoor air and surfaces in sensitive wards of the Arak University of Medical Sciences educational hospitals and determine the drug susceptibility pattern of isolated species.
Methods & Materials: In this descriptive cross-sectional study, 63 air samples were taken from sensitive hospital wards using the one-stage Anderson method, and 63 surfaces samples were taken using wet cotton swabs and cultured in saprodextrose agar medium containing chloramphenicol. Identification of the genus and, as far as possible, the species of fungi was performed using the culture method on the slide. Drug susceptibility testing was performed on isolated species by broth microdilution method (CLSI-M38A2 standard).
Ethical Considerations: This study was approved by the Research Ethics Committee at Arak University of Medical Sciences (Code: IR.ARAKMU.REC.1395.315).
Results: From the total samples, 18 species of fungi were isolated. These included: Aspergillus niger (8), Aspergillus flavus (4), Aspergillus fumigatus (2), Rhizopus spp. (2), Mucor spp. (1) and Fusarium spp. (1). In the drug sensitivity assay, instances of resistance included: Partial sensitivity of Aspergillus fumigatus to Itraconazole (1), Partial sensitivity of Aspergillus niger to Ketoconazole (1), and Resistance of Aspergillus niger to Itraconazole (1).
Conclusion: The pattern of nosocomial fungal infection with pathogenic fungi and the drug susceptibility pattern of these organisms in other regions of Iran and the world is relatively consistent with the present study results. And drugs listed in global guidelines for treating these infections, such as voriconazole and caspofungin in the treatment of invasive aspergillosis and amphotericin B in the treatment of invasive mucormycosis and Fusarium wilt, are now effective drugs.

Abstract
Background and Aim: Resistance training with blood flow restriction can have the same effects as traditional resistance training. The current study aimed to determine the effect of six weeks of selected resistance training with active and passive rest, with and without blood flow restriction, on young men’s C Reactive Protein (CRP), Lactate Dehydrogenase (LDH), and muscle endurance.
Methods & Materials: From the available and voluntary samples, 24 healthy young soldiers of the Islamic Republic of Iran Air Force were divided into 3 groups of 8 subjects: traditional resistance training, resistance training with blood flow restriction, and passive rest and resistance training with blood flow restriction and active rest. Exercise programs were performed for 6 weeks, 3 sessions per week with an intensity of 70-80% One-repetition maximum (1RM) for the traditional resistance group (3 sets of 10), 20-30% 1RM for the passive rest group (30-15-15-15), and 20 -30% 1RM was administered for the active rest group (30-7-15-7-15). Before and after six weeks, physiological and anthropometric characteristics, muscle endurance, and hormonal levels were measured, and ELISA measured blood samples. Data were analyzed using covariance and Bonferroni post hoc tests and paired t-test for comparison within groups.
Ethical Considerations: All experimental procedures were approved by the Ethics committee of the Sport Sciences Research Institute of Iran (Code: I.R.SSRC.REC.1398.129), Clinical Trial (Code: ID IRCT20191207045644N1) from the Iran Clinical Trial Registration Center and were conducted under the Declaration of Helsinki.
Results: Results showed that 6 weeks of exercise had a significant effect on Muscle endurance (P=0.001) but on the levels of C-reactive protein (P=0.43) and Lactate dehydrogenase (P=0.44) had no significant difference.
Conclusion: According to the results of this study, it seems that a combination of resistance training with restriction of blood flow and Interval training (active rest) can be a good alternative to traditional training and, in some cases, replace resistance training with occlusion and passive rest.

Abstract
Background and Aim Recently, a myokine called irisin has been discovered that affects glucose level, obesity, metabolism and glucose homeostasis by browning of white adipose tissue and heat production. This study aims to evaluate the effect of four weeks of ginger supplementation followed by acute eccentric and concentric training on irisin and insulin levels, insulin resistance index, and lipid profile of young women with obesity.
Methods & Materials This is a randomized clinical trial. Participants were 50 young women (BMI >30 kg/m2, age: 24.86±2.87 years, height: 159.66±4.20 cm, and weight: 87.42±4.61 kg). After measuring their maximum oxygen consumption, they were randomly divided into five groups, two groups of supplementation, two groups of placebo, and one control group. The training protocol consisted of two sessions of acute eccentric and concentric activity. The supplementation groups consumed 2 g of ginger powder daily for 4 weeks. Blood samples were collected before and immediately after each activity. Data were analyzed using dependent t-test and one-way analysis of variance (ANOVA).
Ethical Considerations This study was registered by Iranian Registry of Clinical Trials (Code: IRCT201104246178N1) and was approved by the ethics committee of Ilam University of Medical Sciences (Code: IR.MEDILAM.REC.1395.192).
Results The irisin and insulin levels between increased and the glucose level decreased significantly in the supplementation groups (P<0.05). The change in insulin resistance index was not significantly different among the study groups (P>0.05). Based on the results of one-way ANOVA, triglyceride and low-density lipoprotein levels decreased significantly (P<0.05), but the high-density lipoprotein level showed no significant difference (P<0.05). The total cholesterol level was not significantly different in terms of exercise type (P> 0.05) but was significantly different in terms of supplementation type (P<0.05).
Conclusion Long-term consumption of ginger along with regular exercise can be an effective method for lowering glucose level and causing favorable changes in irisin level, insulin resistance, and lipid profile in young women with obesity.

Abstract
Introduction: Considering the importance of urinary tract infections caused by uropathogenic Escherichia coli (UPEC) in the medical field, this study aimed to investigate serogroups O25 and O16 and the pattern of antibiotic resistance among UPEC isolates obtained from hospitalized patients with urinary tract infections (UTIs) in Rasht hospitals.
Methods: A total of 110 urine samples were collected from patients with UTIs referred to selected hospitals in Rasht. The disk diffusion method, as recommended by the CLSI, was used to determine the pattern of antibiotic susceptibility. Serogroups O25 and O16 were detected using specific primers.
Results: Among the studied samples, 36.4% (40/110) were men and 63.6% (70/110) were women. Based on the antibiotic susceptibility pattern, a high level of antibiotic resistance was observed against nalidixic acid (81.8%) and co-trimoxazole (78.2%), while the most effective antibiotics were amikacin (85.5%) and nitrofurantoin (83.6%). In addition, multi-drug resistant phenotype was found in 72.7% (110/80) of UPEC isolates. According to PCR results, the frequency of serogroups O25 and O16 was 36.4% and 17.3%, respectively. Both serogroups had the highest resistance to nalidixic acid and co-trimoxazole, while the lowest resistance in serogroup O25 to nitrofurantoin (20%) and amikacin (14.3%) and in serogroup O16 to imipenem (5.3%) and nitrofurantoin (10.5%).
Conclusions: This study showed that the high prevalence of MDR strains among UPEC strains is very worrying and professionals should be very careful in prescribing antibiotics for patients. Like most studies, the frequency of serogroup O25 was high, and probably, this serogroup can play a role in causing urinary tract infections and antibiotic resistance of UPEC strains.

Abstract
Introduction: Fetuin-A is a secreted protein from the liver, which plays a role in microvascular disorders of diabetes through insulin resistance and inflammation. The aim of this study is to investigate the amount of fetuin-A in diabetic patients without and with microvascular disorders.
Methods: 90 people in 4 groups, diabetic control, nephropathy, retinopathy and retinopathy-nephropathy participated in this research. Data were analyzed using Kruskal-Wallis and Pearson correlation tests. Measurement of parameters including VEGF, interleukin 8 (IL-8), insulin, interleukin 6 (IL- 6) and Fatuin-A was done by ELISA method. The concentration of C-reactive protein (CRP) was measured by nephrometric method.
Ethical considerations: This study was conducted after the approval of the protocol in the research ethics committee and receiving the ethical code number IR.ARAKMU.REC.1400.250 and according to the Declaration of Helsinki.
Results: The obtained results showed that the correlation between fetuin-A and VEGF in the control group was not significant (p=0.234), in the nephropathy group it was significant (p<0.01), in the retinopathy group it was significant (p<0.01) and in the retinopathy group - Nephropathy was significant (p=0.032). Also, the correlation between fetuin-A and interleukin-6 in the nephropathy group is not significant (p=0.285), in the retinopathy group is not significant (p<.075), in the retinopathy group is not significant (p<0.059) and in the retinopathy-nephropathy group. It was insignificant (p=0.113). The correlation between fetuin-A interleukin 8 in the control group is not significant (p=0.592), in the nephropathy group (p=0.592), in the retinopathy group (p=0.314) and in the retinopathy-nephropathy group (p= 0.362). The correlation between Fetuin-A and the homeostatic model of insulin resistance was significant in all groups (p<0.01). Fetuin-A and VEGF levels in the three groups of nephropathy and retinopathy increased significantly compared to the control group. The amount of interleukin 6, interleukin 8 and homeostatic model of insulin resistance in nephropathy, retinopathy and retinopathy-nephropathy groups has increased significantly compared to the control group.
Conclusions: Because fetuin-A is directly related to insulin resistance and VEGF production, its control can have an effect in preventing and controlling the development of microvascular disorders, especially in the early stages of diabetes. Although the results showed that fetuin-A level has no significant relationship with inflammatory factors such as interleukin 6 and interleukin 8, especially in the retinopathy-nephropathy group, but a significant increase of interleukin 6, interleukin 8 and CRP in patients with microvascular disorders to the diabetic control group, it shows the importance of inflammation in the development and progression of microvascular disorders and the importance of its control in diabetic patients.

Abstract
Introduction: The purpose of this research was to investigate the effect of a period of resistance training and melatonin consumption on sex hormone levels, pain intensity, and sleep quality in girls with primary dysmenorrhea.
Methods: For this purpose, 60 girls with moderate primary dysmenorrhea disorder (score 4-7) were randomly placed in 4 groups: resistance training + melatonin, resistance training + placebo, melatonin supplement group, and control group. Pain intensity, and sleep quality were assessed by the McGill questionnaire, and the Pittsburgh questionnaire respectively. 10 mg of melatonin was taken daily in two 5 mg capsules and the placebo group received the same amount of carbohydrates in the same capsule. Weight training, three days a week for eight weeks. This circuit exercise training was in 9 stations and with a maximum of 10-12 repetitions at 30-65% of a maximum repetition in each station. Each set and repetition was separated by 2-3 minutes and 90 seconds of rest, respectively. The blood sample was taken in the morning, fasting and 5 cc from the brachial vein from the left hand while sitting in the sitting position, and was taken by special kits. Data were analyzed by ANOVA with repeated measures and Bonferroni post hoc test at level P<0.05. All experimental procedures were approved by the Ethics committee of the Sport Sciences Research Institute of Iran (Code: ID IR.IAU.NEYSHABUR.REC.1401.008), Clinical Trial (Code: ID IRCT20230703058653N1) from the Iran Clinical Registration Center and were conducted under the Declaration of Helsinki.
Results: A significant increase in Estrogen (P<0.001) and progesterone (P<0.001) levels in the exercise + supplement group compared to all groups, and in the exercise + placebo group and the supplement group compared to the placebo group was reported. Also, The decrease in Pain intensity (P<0.001) and sleep quality (A decrease in the sleep score means an increase in the sleep quality in the output of the questionnaire) (P<0.001) in the supplement group compared to all groups and in the exercise + placebo group (P<0.05) and the exercise + supplement group compared to the placebo group showed a significant difference.
Conclusions: It seems that synergy of exercise and melatonin has affected the sex hormones level. Also, the melatonin probably helped to improve the sleep quality and pain intensity of the subjects through the adjustment of sex hormones following dysmenorrhea. Also, exercise probably has an effect on the pain intensity and sleep quality through the release of beta-endorphins and the effects of exercise on the menstrual cycle, although the finding are ambiguous.

Introduction: Resistance training (RT) is an important part of athletes' preparation exercises with the aim of strengthening physical fitness, creating neuro-muscular adaptations and improving sports performance. The aim of this study was to investigate the effect of resistance training with weight on NCV of tibial and peroneal nerves, muscle strength, agility, flexibility and performance of half-back, half-turn and angel balance skills in seven- to ten-year-old gymnasts.
Methods: In this semi-experimental study, 24 children with two years of experience in gymnastics (age: 8.5±1.5 years, weight: 32.10±9.40 kg, height: 135±13 cm) voluntarily Participated. Subjects were randomly divided into two groups including gymnastics training and gymnastics training + resistance training with weights. And they trained for 10 weeks. During the research, one group did general gymnastics exercises, and the other group did resistance training with weights three times a week in addition to gymnastics exercises. Before and after the exercises, NCV test, Sargent's jump muscle power tests, agility of 9x4m back-and-forth run, Wells' flexibility, and performing half-back, half-turn and balance skills of Angel were performed. Independent t-test was used to analyze the data. And a significance level of P<0.05 was considered.
Results: The results showed that the NCV in the tibial and peroneal nerves increased significantly in the weight resistance training group compared to the general gymnastics training group. Also, muscle strength in Sargent's jump, agility in 4x9m back and forth running, and the performance of half-back and half-turn skills increased significantly (P<0.05). However, no significant change was observed in the flexibility with the Wells test (P=0.870) and execution of Angel's balance skill (P=0.552).
conclusion: 10 weeks of resistance training with weights significantly increases neuromuscular adaptations, NCV, muscle power, agility, and implementation of half-back and half-varus skills in gymnast children. Therefore, it is suggested to use resistance exercises with weights to improve the performance of children athletes in the field of gymnastics and fields where nerve conduction speed, muscle power and agility are effective in the success of athletes.

Abstract
Introduction: Considering the synthesis and release of myokines from muscle tissue during exercise and the relationship of myokines with the development of strength and increase in muscle volume, the purpose of this study is to investigate 12 weeks of resistance training on the serum levels of myonectin and FGF- There were 21 middle-aged men.
Methods: In a semi-experimental study, 40 middle-aged men of Arak city (mean age 38.27 ± 6.02 years, weight 77.12 ± 11.23 kg, height 174.05 ± 7.22 cm) were selected and randomly placed in two resistance training groups (20 people) and control (20 people). The training group performed 12 weeks of resistance training 3 times a week with an intensity of 65-80% of maximum strength. Blood was taken from all the subjects 48 hours before and after the intervention, and serum levels of myonectin and FGF-21 were checked by the ELISA method. After checking the normality of the data, they were analyzed by the correlation t-test and independent t-test at a significance level of less than 0.05.
Results: Resistance training caused a significant increase in the serum level of myonectin (P = 0.001) and the serum level of FGF-21 (P = 0.001) and also decreased the percentage of fat (P = 0.417) in middle-aged men.
Conclusions: According to the results of this study, it seems that resistance training can have positive adaptation by increasing the serum levels of myonectin and FGF-21 in the regulation of muscle mass, hypertrophy process, angiogenesis process, and Improve the body's metabolism.
