Abstract
Background and Aim: one of the key concepts in physical education and sport science is the process of talent identification. The purpose of this research was to investigate the effect of PPARα gene polymorphism on some of the athletic performances of non-athlete 10-12-year-old children.
Materials and Methods: The present project was carried out in the form of semi-experimental and field-based research with salivary sampling. To determine the polymorphism of the genes, the methods used included saliva sampling, salvary DNA extraction and PCR-RFLP method and exercise tests included the Shuttle run, standing broad jump and 20m sprint. Our subjects consisted of 118 non-athletic healthy boys of Marand from 10 to 12 years old. After comparison with Hardy-Weinberg equilibrium, frequency of genotype was tested with Leven, Fisher and Kolmogorov-Smirnov tests. Using one-way covariance analysis, the mean group phenotypes was compared with each other. Type of polymorphism as a predestine variable and the athletic performances of 20m shuttle run, standing broad jump and 20m sprint test were considered as the criterion variable. All analyzes were performed by SPSS 22.
Findings: The results showed that the subjects with PPARα gene GG polymorphism had better performance in the endurance tests than subjects with CC and GC polymorphism.
Conclusion: It can be concluded that GG polymorphism is related to the endurance activities, but CC and GC polymorphisms do not have a particular predominance in the endurance, speed and power activities. |

Abstract
Background and Aim: Enterococci are among the microbial flora in human and animals digestive tract. The nosocomial pathogenicity of enterococci has emerged in recent years. The purpose of the study was to evaluate the effect of a diode laser with a wavelength of 810nm and a power of 30mw at 180s on the survival of an antibiotic resistant enterococci.
Materials and Methods: 30 clinical isolates of enterococcus bacteria were tested for sensitivity to antibiotics according to diffusion method. 10 isolates with the highest antibiotic resistance were selected and diode laser with a wavelength of 810nm and a power of 30mw for 180s were beamed on them and investigated.
Findings: The Results of our antibiotic susceptibility test showed that among 30 isolates resistant to antibiotic enterococci, 27 isolates (90%) were resistant to Oxacillin antibiotics. The results of the diode laser radiation showed that the lowest survival rate of the intended isolates was at 810nm and 30mw for 180s (0.58%).
Conclusion: Based on the results obtained, the antibacterial effect of the diode laser is at 810nm and 30mw for 180s. Therefore, it is recommended to use a 810nm diode laser to eliminate clinical strains of antibiotic-resistant enterococci.

Abstract
Background and Aim: There are two philosophies about the origin of the immune system, defense and homeostasis. In this article, the existential philosophy of the immune system was reviewed, in which the defense role is only part of its homeostasis functions.
Materials and Methods: The Medline was searched for terms such as “Homeostasis”، “Immunculus”، “Natural autoantibody” “Evaluation” and “Immune system”.
Findings: Hemostasis is the most important part of immune regulation and the existential philosophy of this system, so that its defensive role is only part of homeostasis functions. Natural antibodies play this role as immune reflection by showing qualitative and quantitative changes in the antigenic composition of the body. Therefore, the level of natural antibodies can also serve as a prognostic marker.
Conclusion: Although many improvements have been made in explaining the hemostatic role of the immune system, many studies have to be conducted to identify its particular aspects.

Abstract
Background and Aim: Type 2 diabetes exerts an anti-angiogenesis effects on the cardiac tissue. The aim of present study was to investigate the effects of endurance training on gene expression of VEGF and VEGFR2 of cardiac tissue in type 2 diabetic male wistar rats.
Materials and Methods: 36 male wistar rats were randomly divided into three groups, Diabetic Endurance Training (DET), Diabetic Control (DC) and Healthy Control (HC). Type 2 diabetes was induced by intraperitoneal injection of STZ. The endurance training included 10 weeks, 5 sessions per week running at speed of 27 m/min and intensity of 75% VO2max for 20-30min in 1st week and reached to 27 m/min and intensity of 75% VO2max for 60 min/day in 10th weeks. The gene expression of VEGF and VEGFR2 were examined by Real-Time PCR AND Western Blotting.
Findings: The results of Real-Time PCR showed that the endurance training caused increase in VEGF mRNA (p<0.05) and VEGFR2 mRNA (p<0.05) compared to DC group. The results of Western Blotting also indicated that the endurance training induced increase in VEGF protein expression (p<0.05) compared to DC group and there was no significant change in expression of VEGFR2 protein (p>0.05).
Conclusion: Type 2 diabetes impairs the angiogenesis process in diabetic cardiac. It appears that endurance training has positive impact on angiogenesis and it can cause the improvement of diabetic cardiac.

Abstract
Background and Aim: Recent findings in cell therapy have presented new perspectives and opportunities for the treatment of neurodegenerative disease. The experimental research with intranasal (IN) administration of Stem Cells in Parkinson’s disease (PD) mouse model can work and in some cases induce major, long-lasting improvement. Adult Human endometrial derived stem cells (HEDSCs), a readily obtainable type of mesenchymal stem-like cell were used to generate dopaminergic cells and for cell therapy. In this study, we investigated the therapeutic effects of IN-delivered HEDSCs in mice model of Parkinson.
Materials and Methods: In this experimental research, 35 male mouse weighting 25-30 g were divided into 5 groups. On day 120 post cell administration, the rotational behavior was measured. Immunohistochemistry staining was used to detect HEDSCs in mice brain.
Findings: IN application of HEDSCs resulted in the appearance of cells in the substantia nigra (SN) and decrease in the rotational behavior of case group.
Conclusion: HEDSCs are a highly inducible source of allogenic stem cells that improve Parkinson’s disease.

Abstract
Background and Aim: The eNOS gene that is responsible for the production of nitric oxide and angiostatin is an inhibitor of angiogenesis. The aim of present study was to investigate the effect of endurance training on angiostatin and eNOS gene expression of cardiac tissue in type 2 diabetic male wistar rats.
Materials and Methods: In an experimental study, 36 male wistar rats were randomly divided into three groups, Diabetic Endurance Training (DET, n=12), Diabetic Control (DC, n=12) and Healthy Control (HC, n=12). Type 2 diabetes was induced by intraperitoneal injection of STZ. The endurance training included 10 weeks, 5 sessions per week running at speed of 27 m/min for 15 minutes in 1st week and reached to 27 m/min for 60 min/day in 9th weeks. The animals were sacrificed 24 h after last training session and the samples were taken from cardiac tissue. The gene expression of angiostatin and eNOS were examined by Real-Time PCR. The one-way ANOVA was used to analysis the data. The significant level was set at p<0.05.
Ethical Considerations: This study was approved in Research Ethics Committee of Arak university of medical sciences with the code IR.ARAKMU.REC.1394.329.
Findings: The gene expression of angiostatin and eNOS of DC group showed significant increase compared to HC group (p =0.000). The endurance training induced significant decrease in the gene expression of angiostatin and eNOS compared to DC group (p =0.000).
Conclusion: It appears that gene expression of angiostatin and eNOS of diabetic cardiac tissue are affected by positive effect of endurance training.

Abstract
Background and Aim: Obsessive-compulsive disorder is a disabling disorder that affects one's performance. So, the aim of this study was to investigate the effectiveness of mindfulness training on reduction of obsessive compulsive disorder symptoms and increase in marital satisfaction of married women.
Materials and Methods: This study was a quasi-experimental design with pretest-posttest and control group. The statistical population included married women with obsessive compulsive disorder of Tehran city in year 2017. 30 married women with the age range of 20-45 years old were chosen as samples and randomly were divided into two groups of individuals as control and mindfulness training. The experimental groups practiced the mindfulness training for a month, 8 sessions and 60 minutes in each session. Then, post-test was performed for them. For data collecting, the obsessive-compulsive inventory, obsessive-compulsive and marital satisfaction were used. Data analysis was done by using multivariate analysis of covariance by SPSS 24 software at a significant level less than 0.05.
Ethical Considerations: In this study, patients’ consent and providing results were considered as ethical principles.
Findings: Statistical data analysis illustrated that the amounts of marital satisfaction were significantly increased in experimental than control group by mindfulness training (p<0.05). Also, of mindfulness training was significant reduction of obsessive compulsive disorder symptoms (p<0.05).
Conclusion: According the findings of this study, mindfulness training can be a good approach to improve of obsessive compulsive disorder symptoms and marital satisfaction of married women. |

Abstract
Background and Aim: The purpose of this study was to determine the effect of a hind limb immobilization on the expression of PGC-1α, NRF1, Mfn2, PINK1 and Drp1 genes as the main regulators of mitochondrial quality and function in soleus muscle of endurance trained rats.
Materials and Methods: In this study, 15 male Sprague-Dawley rats were divided into three groups (control, exercise +immobilization and immobilization). The exercise + immobilization group run on the treadmill for 12 weeks and five times per week. The hind limb of the animal was immobilized for seven days with the casting method. Soleus muscle was extracted and the expression of the genes was measured by RT-PCR method. Univariate ANOVA and Tukey post hoc test were used to determine the differences (α = 0.05).
Ethical Considerations: This study with research ethics code IR.SUMS.REC.1396.S444 has been approved by research ethics committee at Shiraz university of medical sciences, Iran.
Findings: Results showed that immobilization in both immobilization and exercise +immobilization groups, compared to the conterl group, reduced the expression of PGC-1α gene (p = 0.001 and p = 0.045), NRF1 (p = 0.001 and p = 0.006), Mfn2 (p = 0.001, p = 0.001) and increased the expression of PINK1 (p = 0.001 and p= 0.001), but the expression of Drp1 gene didn't change significantly (p = 0.069 and p = 0.223). Also, studies showed that the expression of PGC-1α (p = 0.013), NRF1 (p = 0.001) and Mfn2 (p = 0.001) in the exercise + immobilization was lower in compare with the immobilization group. The expression of PINK1 was lower than immobilization group as well (p = 0.001).
Conclusion: This study shows that endurance training has a protective effect on mitochondrial quality during the immobilization period, but it can't prevent mitochondrial dysfunction.

Abstract
Background and Aim: Physical activity causes the releases of bone resorption indices in the bloodstream by the mechanical load on bone, which in interaction with adipokines reduces obesity and prevent its complications. So, the present study aims to compare the effects of aerobic and resistance exercise on Anthropometric Indices and osteocalcin, leptin, adiponectin levels in overweight men.
Materials and Methods: A total of 40 overweight young healthy men (BMI 28.67±0.96 and age 31.50 ± 2.23) were randomly assigned to control (n = 14), aerobic exercise (n = 13) and resistance exercise (n = 13) groups. Subjects in the exercise group were on 8-week supervised exercise training programme for three sessions per week (aerobic exercise were performed at 60-85% of HRR, and resistance exercise were performed at 55-75% of 1RM). Osteocalcin and adipocytokines (leptin, adiponectin) were assessed from fasting blood samples before and after the 8-week exercise programme. Data were analyzed by t-test and ANOVA.
Ethical Considerations: This study with research ethics code IR.IAU.RASHT.REC.1396.124 has been approved by research ethics committee at Islamic Azad University, Rasht branch, Iran and also has been registered with code IRCT20180226038876N1 at Iranian Registry of Clinical Trials (IRCT).
Findings: Aerobic and resistance training led to decrease in anthropometric indices (p<0.05), leptin levels (p<0.001, p=0.001) and increase in osteocalcin (p=0.001, p<0.001) and adiponectin (p=0.001, p<0.001), respectively. In addition, the difference between the two training groups was not statistically significant (p>0.05).
Conclusion: Considering to the effects of aerobic and resistance exercise on osteocalcin and adipocytokines level, it seems that both exercise methods can be increasing mechanical load on bone mass and cause to change in energy metabolism and body weight and can be an important factor in decrease of obesity complications.

Abstract
Background and Aim: Curcumin is one of the most important nutritional polyphenols that is included in daily supplements diet highly and plays a role in moderating some of the intracellular messenger pathways associated with the regulation of pathologic hypertrophy. The purpose of the present study is to survey the effect of curcumin supplementation on the expression of some genes regulators of the pathological processes of the heart muscle in rats.
Materials and Methods: In this eight-week experimental study, 12 male Sprague Dawley rats were divided into two groups: obesity control (n=6) and curcumin supplement (n=6). Curcumin supplementation was conducted for eight weeks. 24 hours after the completion of the curcumin supplement protocol, the rats were dissected and their heart muscle was removed. The expression of the genes (AMPK, mTOR, S6K, 4EBP, COL1, COL3, and Ang) was performed using Real-Time PCR technique. The expression of the genes was calculated by the 2-∆∆CT method. One way ANOVA was applied to determine the significance of the variables among the study groups.
Ethical Considerations: This study with research ethics code IR.SUMS.REC.1396.S446 has been approved by research ethics committee at Shiraz University of Medical Sciences.
Findings:The results has showed that supplement group of curcumin reduced the expression of mTOR (p < 0.001), S6K (p < 0.011), 4EBP (p > 0.005) collagen 1 (p > 0.002), Collagen 3 (p < 0.001) and Ang (p < 0.003) compared to the placebo group. There was also an increase in the expression of AMPK gene (p < 0.001) which was statistically significant.
Conclusion: it seems that the supplementation of 10 mg curcumin moderate the pathological pathway of cardiac muscle hypertrophy by reducing or keeping up the expression of mTOR gene in obese rats and increasing the expression of AMPK gene. Moreover, this supplement can affect on reducing the pathological hypertrophy during the consumption of curcumin supplementation

Abstract
Background and Aim: Colon cancer is one of the most common cancers in the gastrointestinal tract. Colon cancer is the third death cause among cancers. The aim of this study was to estimate the survival rate and determine the effective factors in colon cancer patients.
Materials and Methods: In this study, 193 colon cancer patients referring to Hamadan Imam Khomeini Clinic during the years 2003-2017 in a retrospective cohort study were used. Follow up of all patients was done by referral and phone call up to 2017. The Kaplan -Meyer model was used to estimate the survival of patients. Also, the effect of prognostic factors on the survival of patients was obtained by Cox regression model. The software used to analyze the data was STATA 11 and the significance level was 0.05.
Ethical Considerations: This study with research ethics code IR.UMSHA.REC.1396.144 was approved in Research Ethics Committee of Hamadan University of medical sciences, Iran.
Findings: The mean age of the patients at diagnosis was 57.09 ± 12.9 years. The probability of survival of one-, three- and five-year was 0.82, 0.61 and 0.48 percent, respectively. Also, the cancer stage has a significant effect on survival time of the patients.
Conclusion: Based on the Cox model, only the stage of cancer was effective on the survival time of patients with colon cancer. Therefore, timely diagnosis also helps prevent disease progression, as well as increase the survival time of the patient, especially at an advanced age.

Abstract
Background and Aim: The aim of this study was to present a conceptual model of spiritual well-being in prevention of substance abuse in adolescent.
Materials and Methods: The present study was done with a qualitative meta-synthesis with systematic review of the studies present in the foreign and Iranian journals about spiritual well-being and the role of them in prevention of substance abuse in adolescents. The population of this study consisted of non-interventional archive studies that were done during 1986 -2016 in Oxford Journal, Science Direct, Wiley Online Library, Taylor&Francis, ProQuest, Springer, Iranmedex, Irandoc, SID and Magiran. Keywords were including of spiritual well-being, spirituality, prevention of substance abuse and adolescents.
Ethical Considerations: This study with ethics code 8797/1129 has been approved by Farabi Campus of University of Tehran.
Findings: After reviewing and evaluating the quality of the research, eventually 73 papers and theses included 8 Persian articles, 65 English papers including 11 theses and 54 articles were found and dissertations and finally conceptual model based on fisher (1998) theory's, wree presented. On this ground, subcategory of spiritual well-being was consisted of three theme of relationship with others, relationship with self and relationship with God. Each of them consisted of two subtheme of psychological health (interpersonal and intimate) and psychological well-being(interpersonal and intimate).
Conclusion: The results showed that spiritual well-being influenced on multiple dimensions of adolescent psychological health and well-being and prevention of adolescent substance abuse from this way.

Abstract
Background and Aim: Promoting spiritual intelligence and adherence to ethics leads to higher quality service, efficiency and effectiveness. The present study was conducted to investigate the relationship between the spiritual intelligence of librarians and the quality of services in the libraries of Shahid Beheshti University of Medical Sciences with a professional ethics approach.
Materials and Methods: The research method was descriptive-correlational and with an objective purpose. The statistical population included 180 librarians working in the library of Shahid Beheshti University of Medical Sciences. Data collection was done by two questionnaires of King and Radad. Data analysis was done by inferential methods and Kolmogrov-Smirnov test. Data were analyzed by SPSS version 22 software.
Ethical Considerations: In this study, all principles of research ethics were considered.
Findings: The results showed that there is a positive and significant relationship between the dimensions of spiritual intelligence(critical existential thinking, production of personal meaning, transcendental consciousness, and extension of consciousness) and the quality of services in the libraries.
Conclusion: The result of the research showed that there is a relationship between the spiritual intelligence of librarians and the provision of quality services in the libraries of Shahid Beheshti University of Medical Sciences with the professional ethics approach. Therefore, using the spiritual intelligence, service quality in the studied libraries could be increased.

Abstract
Background and Aim: Background and Aim: Photodynamic therapy is a minimally invasive approach to cancer, which is used to combine non-toxic photosensitizer and visible light to produce reactive oxygen species and destroy tumors. This study aimed to investigate the cytotoxicity effect of Graphene Oxide (GO)as an organic matter with many oxygen groups on photodynamic on destroying cancer cells.
Materials and Methods: This study was performed on breast cancer cells (MCF-7) in vitro. The study groups included the first group of drug with different concentrations of graphene oxide (333.3, 285.7, 230.7, 166.6, 90.9,
47.6 µg/ml), the second group co-drug and laser light irradiation and the control group consisted of cells treated only with laser irradiation and the control group was treated with no treatment. Cells were exposed to visible laser irradiation (405 nm) at 0.1 W / cm2. Cell viability was determined by MTT assay.
Ethical Considerations: This study with research ethics code SBU/S.1397.46A has been approved by research ethics committee at Shahid Behesht University of Tehran, Iran.
Findings: The results of in vitro experiments showed that the dark toxicity of graphene oxide at concentrations of less than the 90.9 μg / ml concentration had no significant effect on cancer cells. Also, laser light alone don’t has toxic effect on cells. But graphene oxide-mediated dynamic light therapy has reduced the bioavailability of cancer cells on average by 21% over dark toxicity. Results are presented as mean of three independent replications, standard error, and p< 0.05 was considered significant.
Conclusion: In this study, graphene oxide is fully biodegradable at concentrations below a certain value, but with increasing concentration, the toxicity effect increases. With exposure to light and graphene oxide, viability decreses that it is more effective for in vivio studies.

Abstract
Background and Aim: Endurance training causes cardiac remodeling, one of the factors that adjusting expression of more genes of heart is miR-133. The aim of this study was to evaluate the effect of endurance training on miR-133 expression in wistar rats’ heart.
Materials and Methods: In this experimental study, 14 rats were housed under controlled conditions for 4 weeks, after familiarization they were randomly assigned to control (7 rats) and experimental (7 rats) groups. The experimental group performed 14 weeks, 6 session per week an endurance training program (that gradually reached to 60 min and 30 m/min) on treadmill. 48 hours after the end of the last session, all animals were anesthetized and sacrificed. Then, the their heart were removed and after tissue homogenization of left ventricle, and RNA extraction, and cDNA synthesis, the expression level of left ventricle miR-133 were measured by using Real-Time PCR. The rate of miR-133 expression was evaluated by using t-test at p≤ 0.05 level.
Ethical Considerations: This study was approved in Research Ethics Committee of Lorestan University with the code 1396345.52
Findings: After 14 weeks endurance training, the expression of heart miR-133 in experimental group was significantly increased (p=0.007) than control group which coincided with increase of the rate of left ventricular mass to weight body (p=0.012).
Conclusion: Regarded to structural changes of heart, it seems the part of heart adaptation to endurance exercise caused by increase in miR-133 expression.

Abstract
Background and Aim Oppositional Defiant Disorder (ODD) with hyperactivity is one of the behavioral disorders in childhood and adolescence. The mobile health is an economical, available, and useful tool for training the mothers with ODD and hyperactive child. The research aimed to examine the effectiveness of parent training behavioral management program by mobile health to mothers to reduce oppositional and aggression symptoms in children with ODD and hyperactivity.
Methods and Materials The method of this research was a single case experiment by the A-B model. The participants of this study ranged from 7 to 12 years old children and their mothers in Tehran who referred to Rooyesh-e-Talaee Institute and diagnosed with ODD and hyperactivity in 2018. Four children and their mothers were selected with simple random sampling. The intervention implemented in ten 60- 90-minute sessions. For data analysis using the charts, improve percent, and RCI.
Ethical Considerations The Research Ethics Committee of Khatam University, Iran approved this study (Code: Kh.96.S.100.3123).
Results Children with ODD and hyperactivity showed improvement in comparison with baseline measures. The overall improvement in their defiant was 61.43% and in aggression was 55.33%, and the RCI was higher than 1.96 for all three children. The results showed that the two hypotheses were clinically significant.
Conclusion The mobile health training with its related apps was a useful, low cost, available, and suitable method for mothers with ODD and hyperactive children. It can also be complementary to counseling services for mothers with the ODD and hyperactive children. Besides, this technology can be helpful for the country's health system and sustainable health development.

Abstract
Background and Aim Exercise improves sensitivity to insulin by reducing some adipokines including chemerin. However, the effect of endurance exercise and high-intensity intermittent exercise on chemerin level and insulin resistance in obese male rats is still unknown.
Methods & Materials In this study 27 male rats were randomly divided into 4 groups of sedentary control (n=6), endurance exercise (n=8), high-intensity intermittent exercise (n=8) and untreated obese control (n=5). Both endurance and high intensity intermittent exercise groups performed exercises for 8 weeks. The plasma level of chemerin was measured by ELISA method.
Ethical Considerations ???
Results The chemerin level was significantly reduced in the endurance group compared to the untreated obese control group (P<0.01), but it had no significant change compared to the sedentary control group. Moreover, the plasma level of chemerin in the high-intensity intermittent group was not significantly different from that of untreated obese control group and sedentary control group (P>0.05) and chemerin level reduction in the endurance group was significant compared to the high-intensity intermittent group (P<0.05). Furthermore, there was no significant difference in the insulin resistance reported in high-intensity intermittent group compared to that of endurance group (P>0.05), but its difference in high-intensity intermittent and endurance training groups was significant compared to that of untreated obese control and sedentary control groups (P<0.05).
Conclusion Both endurance and high-intensity intermittent exercises can reduce insulin resistance which can be due to the reduction of chemerin plasma level and weight loss.

Abstract
Background and Aim Among the plants with multiple healing properties and a long and common history of use in Iranian traditional medicine, is dog-rose plant with the scientific name of “Rosa canina” that belongs to the Rosaceae family. This study aimed to review the effectiveness of Rosa canina in treatment of different body organs and tissues and emphasizes its therapeutic mechanism.
Methods & Materials This is a review study. About 176 papers published from 2010 to 2018 were extracted from PubMed, Science Direct, Scopus, Google Scholar, Ovid, MagIran, and SID databases using related keywords and entered into study.
Ethical Considerations Ethical issues (including plagiarism, misconduct, data fabrication, falsification, double publication or submission, redundancy) have been completely observed by the authors.
Results Different studies had indicated the importance of Rosa canina as a medicinal plant protecting liver, lowering glucose and lipid levels, and having antioxidant and anti-inflammatory properties. The highest flavonoid activity in the Rosaceae family is related to Rosa canina. The use of herbal extracts in the treatment of common diseases, such as diabetes, fatty liver, urinary tract and Alzheimer's, has been recommended.
Conclusion High qualitative and quantitative diversity of effective ingredients, ease of access, absence of adverse effects of consumption and endemic nature of Rosa canina plant make it necessary to use it for the control and treatment of common human diseases.

Abstract
Background and Aim: recent years, increase in extended-spectrum β-lactamases (ESBLs) producing Escherichia coli has led to limitations of treatment options. This study aimed to find the frequency of blaTEM and blaPER genes among ESBL producing urinary isolates of E. coli and detect their resistance pattern.
Methods & Materials: From January 2016 to February 2017, 972 urine samples from patients suspected of having urinary tract infections in three main hospitals and laboratories in Karaj were collected. Bacterial identification, antimicrobial susceptibility and ESBL production were performed according to the standard guidelines. Polymerase chain reaction was used for the detection of TEM and PER -lactamases.
Ethical Considerations: This study was approved by the Research Ethics Committee of Islamic Azad University of Tehran Medical Branch (Code: IR.IAU.TMU.REC.1396.274).
Results: Out of 972 samples, 500 cases were culture-positive for E. coli. Thirty-six percent (n =180) of the isolates were determined as ESBL-producer. Among ESBL positive isolates, the most susceptibility was observed in amikacin and imipenem (80 and 60% respectively). Resistance to trimethoprim/sulfamethoxazole,ciprofloxacin, tetracycline and gentamicin was 92.7%, 78.9%, 66.1% and 57.8%, respectively. All ESBL producers exhibited multidrug resistance. Among the ESBL-positive isolates, blaTEM gene was detected in 44.72% (n=85) of the isolates, but the blaPER gene was not found in any of the isolates.
Conclusion: The prevalence of multidrug resistant ESBLs producing uropatogenic E. coli is high. Continues monitoring of ESBL producers and their resistance patterns can help to reduce the spread of such resistant strains in the community.

Abstract
Background and Aim: From the viewpoint of Iranian Traditional Medicine (ITM), hemorrhoidal bleeding is not only an organic disease; it also indicates the quality and quantity of blood and temperaments in the body. According to this view, bleeding from different areas has several causes, two important causes of which are: a. vascular hyperemia and consequent blooding in bleeding-prone areas, and b. changes in blood quality that cause the arteries to open and bleed. This study, by reporting a case of hemorrhoidal bleeding, is an evidence of some therapeutic principles in ITM.
Case Report: Patient was a 39-year-old married woman with four children and warm-wet temperament complaining of heavy menstrual bleeding for one year. After three months of herbal drug administration, menstrual bleeding became normal. One month later, she had hemorrhoidal bleeding. Her bleeding was treated based on ITM method by removing the black bile (Soda) from the body, phlebotomy and leech therapy around the anus.
Ethical Considerations: This study has been approved by the Research Ethics Committee of Arak University of Medical Sciences with code: IR.MUQ.REC.1396.110.
Conclusion: Based on ITM, it seems that the cause of hemorrhoidal bleeding in this case is vascular hyperemia and poor blood quality following symptomatic treatment of heavy menstrual bleeding.
