Abstract

Abstract

Background: In the modern societies, the seniors are as one of the growing social groups that according to the age pyramid have increasingly become important. Accordingly, this study was conducted with the purpose of investigating the effectiveness of protocol based on integrated view on reducing psychological distress in the elderly.

Materials and Methods: The type of this study was semi-experimental with pre-test post-test plan along with the control group. The statistical population includes all elderly that referred to Karaj Takht-e Jamshid hospital in the second half of the year 2016. Among them, 30 people were selected in form of available; then, randomly were placed in two 15 people experimental and control groups. Control group were treated based on integrated view protocol (three combination therapies consisting of cognitive- behavioral therapy, spiritual therapy and acceptance and commitment therapy). In this research, the short form questionnaire of psychological distress (DASS) was used for gathering the data in two stages of pre-test and post-test. The data were analyzed by using SPSS19 software and was analyzed by the covariance analysis method.

Results: The results of covariance analysis showed that the protocol based on integrated view has led to the statistical significant difference (p<0.05) between the pre-test and post-test stages in the psychological distress score.

Conclusion: According to the obtained results, the protocol-based on integrated view can be used as a proper and effective treatment method for the reduction of psychological distress in seniors.

Abstract

Abstract
Background: In this study, the effects of zinc oxide nanoparticles (ZnO NPs) and thiamine on the blood biochemical markers and kidney histopathological changes after experimental diabetes in mice was investigated.
Materials and Methods: For this purpose, 56 mice were randomly divided into 8 groups of 7 each. Two groups of animals as controls (A) and thiamine (G) were considered. Other groups were diabetic by alloxan at a dose of 180 mg/kg. Group B mice were considered as diabetic group. To diabetic mice into Group C and D , ZnO NPs in concentrations of 0.1 and 0.5 mg/kg were intraperitoneally injected. Groups E and F; to these groups of diabetic mice, ZnO NPs in concentration of 0.1 and 0.5 mg/kg along with thiamin (30 mg/l) was injected. ZnO NPs in concentration of 0.1 was injected to group H mice. Changes in renal tissue along with some biochemical parameters were measured.
Results: The results showed that diabetes induced changes in some of the serum biochemical factors (GGT, BUN and creatinine) in rats (p<0.05). However, the administration of nanoparticles and thiamine reduced these negative effects. Exposure to diabetes causes changes in the kidney tissue of the mouse, in the disturbance of scaffolds for tissue integrity clutter, fragmentation of some convoluted tubules and congestion within the connective tissue.
Conclusion: Treatment of the diabetes mice by ZnO NPs and thiamine improves renal histopathologic structure and blood biochemistry levels.

Abstract

Abstract
Background: Candida glabrata is the fourth most common cause of blood infection in America and, due to its resistance to amphotericin B, we have to look for new therapies. The aim of this study was to evaluate the antifungal properties of silver nanoparticles, copper and combine them together with amphotericin B in order to produce an effective drug.
Materials and Methods: Minimum inhibitory concentration (MIC) and minimum fungicidal concentration (MFC) for nano silver copper and combining them together and amphotericin B was conducted by macro dilution. By intraperitoneal injection, anti-fungal effect of nano silver, copper and combining them together and amphotericin B was evaluated in a mouse model.
Results: The amounts of MIC and MFC for silver nanoparticles, copper nanoparticles and their combination were 31 and 62.50 ppm, 31 and 62.50 ppm ,and 31and 15.50 ppm, respectively. The amounts of MIC and MFC for the combination of silver nanoparticles and amphotericin B were 8 and 15.5 ppm, also, 15.5 and 31 ppm for the combination of copper nanoparticles and amphotericin B. Mouse model study confirmed the effect of silver nanoparticles, copper nanoparticles and combining them together and amphotericin B against Candida glabrata. The combination of silver and copper nanoparticles had maximum effect and the combination of silver and copper nanoparticles with amphotericin B had minimum effect in animal model.
Conclusion: The combination of silver and copper nanoparticles has a better effect than other groups.

Abstract

Background and Aim: Dust was considered by researchers as one of the most important forms of air pollution. The aim of this study was to identify the physical and chemical characteristics of dust storm sources in Tehran.
Materials and methods: This study was conducted in selected air sampling stations of Tehran, in the days of dust and ordinary from January 2016 for one year. The concentration of dust particles was determined by gravimetric. Heavy metal concentrations in samples after digestion were determined with graphite furnace atomic absorption spectrometry (AAS-GF). Mineralogy and morphology of the dust fall particles were inspected using X-ray diffraction and scanning electron microscopy and sources particles using Enrichment Factor Analysis and principal component analysis (PCA) were performed. For statistical analysis, R software was used.
Ethical Considerations: This study with research ethics code IR.IUMS.REC.1397.313 has been approved by research ethics committee at Iran University of Medical Sciences.
Findngs: The highest average particle concentration in normal days was related to the central station (122.63 µg / m3), and the lowest for the East Station (67.13 µg / m3). The highest concentrations of the elements measured in the particles are related to iron and aluminum and the lowest concentrations of chromium and nickel. Lowest amount of Enrichment Factor was obtained for iron, less than one.
Conclusion: The results of this study indicated that the concentration of dust in center was higher than other station. Natural and human resources both play a significant role in the release of metals. By conducting further studies on the source of dust in Tehran and using the results, effective control measures can be designed and implemented.

Abstract

Background and Aim: Enterotoxigenic Escherichia coli (ETEC) is one of the most common bacterial causes of diarrhea deaths among children and travelers in developing countries. The ETEC colonization factors, such as CFA/I and CS2 play an important role in the development of the disease. In this study, to produce the CFaE fusion recombinant protein, the tip subunits CFA/I(CfaE) and sub structural unit of CS2 (CotD) from ETEC, were used. Since mucosal immune responses to CFs can prevent disease, the aim of this study was to develop a chimeric antigen for developing the effective vaccine.
Materials and Methods: In order to amplify the cfae-cotd gene, a dual gene construct consisting of cfae and cotd, the PCR reaction was performed by designed primers. The propagated gene was cloned in the expression vector pET28a. Following the induction of a recombinant gene construct with IPTG, the recombinant protein was expressed and purified by Ni-NTA chromatography column and confirmed by western blotting by Anti-Histag.
Ethical Considerations: This study with research ethics code IR.IAU.SRB.REC.1397.066 has been approved by research ethics committee at Islamic Azad University, Science and Research Branch of Tehran, Iran.
Findings: Cloning accuracy was confirmed by PCR and enzyme digestion reaction. The presence of the band in the SDS-PAGE 10% gel in the 68 kDa region, the expression of the recombinant protein, and the presence of the band on the nitrocellulose paper in the Western blotting test confirmed the production of recombinant protein.
Conclusion: Optimization of codon and expression in heterologous hosts is a useful method for the production of recombinant proteins. The production of ETEC antigens as a candidate for vaccination against this bacterium is also prominent.

Abstract

Background and Aim: Diabetes mellitus is one of the leading causes of death, and its prevalence is increasing annually because of the change in lifestyle. Increased blood glucose level and dyslipidemia are the major symptoms of this metabolic disease. Currently, the main and effective treatment for diabetes is the use of medication such as insulin. Its control by using herbal products has received a lot of attention in the world. The aim of this study is to evaluate the effects of bioflora (a probiotic supplement) and aqueous extract of cinnamon in improvement of blood glucose level, dyslipidemia, and reduction of cardiovascular diseases in diabetic rats. 
Methods & Materials: Thirty-five male Wistar rats were prepared and randomly divided into five groups of negative control, positive control, probiotic (treated with bioflora 3.2×108 CFUs/day for 30 days), cinnamon (treated with 200 mg/kg of cinnamon aqueous extract for 30 days), and probiotic + cinnamon. Diabetes was induced by intra-peritoneally injection of streptozotocin. The rats’ weight, blood glucose level, lipid profile, high sensitivity C-Reactive Protein (hs-CRP) and Atherogenic Index (AI) were measured at the end of experiment to predict the risk of cardiovascular diseases.  
Ethical Considerations: This study ethically approved in Research Centre of Islamic Azad University of Arak (Code: IR.IAU.ARAKREC1397.005). All interventions performed in accordance with the Guide for Care and Use of Laboratory Animals prepared by the Institute of Laboratory Animal Resources.
Results: Induction of diabetes caused severe weight lost in rats, but the weight loss was lower in groups treated with probiotic. The blood glucose level in probiotic + cinnamon group was significantly reduced. Bioflora reduced triglyceride, cholesterol, and Low-Density Lipoprotein Cholesterol (LDL-C) levels, while cinnamon extract significantly reduced triglyceride and LDL-C levels compared to the positive control group. AI and hs-CRP values were reduced in the probiotic group compared to control groups. There was no synergistic effect by combined use of bioflora and cinnamon extract. 
Conclusion: Bioflora effectively prevented dyslipidemia by improving intestinal microbiota, lowering blood glucose level, and improving lipid profile and, therefore, reduced the risk of cardiovascular diseases.

Abstract

Background and Aim: Since late 2019, with the emergence of a new type of coronavirus that causes a new respiratory disease called COVID-19, there have been many concerns about the spread of this disease and how to deal with it. Due to the ability of the virus to be transmitted rapidly, diagnosing the infected individuals in the early stages for isolating them is critical. This study  aims to evaluate the reliability of Computed Tomography (CT) scan in diagnosing COVID-19.
Methods & Materials: Participants were 212 patients admitted to hospital with confirmed diagnosis of COVID-19. Demographic information, medical history, symptoms, and the chest CT scan results were collected and analyzed. Finally, the power of CT scans in the diagnosis of this disease was compared with the Real-Time Polymerase Chain Reaction (RT-PCR) molecular test.
Ethical Considerations: This study received ethical approval from the ethics committee of AJA University of Medical Sciences (Code: IR.AJAUMS.REC.1399.091).
Results: The sensitivity of CT scan in the diagnosis of COVID-19 was relatively high, but its false-positive results were also high.
Conclusion: CT scan is a relatively sensitive method for diagnosing COVID-19, but caution should be made due to its high false-positive results which can lead to increased financial burden on the health system.

Abstract

Background and Aim: The human coronavirus is a member of the Coronaviridae family and causes upper respiratory tract infections. Despite repeated severe epidemics and the lack of appropriate antiviral drugs, not much progress has been made on the epitope-based vaccine designed for HCoV. 
Methods & Materials: The method of this study was to select the spike corona virus protein sequence from NCBI, retrieve the protein sequence and determine the T, B epitopes required to produce the chimer vaccine, evaluate the antigenicity and allergenicity and toxicity of the selected epitopes, respectively. Different servers were designed to configure the primary chimer composition of the epitope vaccine. Then, the chimer vaccine was evaluated in terms of structure and connectivity to B cells and MHCI and II compounds, and the two-dimensional structure and position of amino acids and bonds in the immunogenic model were studied, as well as the physicochemical and stability of the model vaccine by some other servers. Finally, it was tested for binding against HLA molecules using silico docking techniques to investigate the interaction with the epitope.
Ethical Considerations: All ethical principles are considered in this article. Participants were informed about the research objective and its implementation stages. They also made sure their information was confidential. The principles of the Helsinki Convention were also observed.
Results: The results showed that the immunogenic construct created in terms of two-dimensional and three-dimensional structure and the position of amino acids and bonds in the model of immunogenic structure, toxicity and allergenicity and antigenicity were in good condition. And had stability (instability index 33.93) and favorable half-life and suitable physicochemical conditions.
Conclusion: In general, the immunogenic structure that was prepared in this research process could have a favorable interaction with some components of the immune system (HLA) in the docking process, which indicates the optimal identification of this structure by the humoral and cellular immune system and stimulation in In order to produce immunity in the body of the host, of course, more reliable proof of it requires clinical phase processes.

Abstract

Background:  Rotator cuff injuries are one of the most common causes of shoulder pain.
There are different therapeutic approaches in this disease. The evidence of the positive effect of exercise therapy in this pathology is inconclusive. This study aimed to investigate the effect of a complete exercise protocol on improvement of the patients with rotator cuff injuries.
Methods: The patients with shoulder pain received the 12-week exercise protocol in 4 phases if they had confirmed rotator cuff injuries in clinical examination and shoulder MRI. 
Pain was measured by visual analog scale (VAS) and function was measured by two questionnaires; disabilities of the arm, shoulder and hand (DASH) and western Ontario rotator cuff index (WORC). Range of shoulder joint motion (ROM) was measured in five directions by goniometry. All of the outcomes were evaluated before intervention and in 6, 12, 24 weeks after starting the intervention.
Ethic: This study was registered in Iranian Registry of Clinical Trials (IRCT20100718004409N13). A written consent was taken from All participants before the study. 
Results: In this study Twenty-four patients with rotator cuff tendinopathy and tear were analyzed finally in terms of effectiveness of exercise therapy. The average of their age was 51.7 years old. 14 patients (53.8%) with Supraspinatus tendinopathy, 6 patients (23%) with Supraspinatus tear, 3 patients (12.5%) with Supraspinatus plus biceps tendinopathy, and one patient (4.16%) with Subscapularis plus Biceps tendinopathy were assessed. The pain score (Visual Analog Scale) decreased at all fallow-ups specially after 24 weeks (3±0.1) versus before intervention (8±0.1) (P value < 0.001). Shoulder range of motions by goniometer and also the patients function based on WORC and DASH Questionnaires were improved after 24 weeks versus before intervention that was significant statistically and clinically (P value < 0.001). 
Conclusion: A 12- week exercise protocol including strengthening, stretching, PNF and dynamic stabilizer exercises was effective on pain reduction and improvement of function and shoulder joint range of motion improvement in the patients with shoulder rotator cuff tendinopathy and partial tear. The improvement of the variables lasted 6 months.

Abstract

Background: and Aim Acinetobacter baumannii causes various nosocomial infections and has a high antibiotic resistance. Probiotics can produce metabolites with antimicrobial properties. This study aims to evaluate the antimicrobial ability of probiotics against nosocomial pathogens by inhibiting the ompA gene expression effective in biofilm formation in Acinetobacter baumannii.
Methods & Materials: The antimicrobial properties of probiotics against nosocomial pathogens were evaluated phenotypically. The polymerase chain reaction (PCR) technique was used to identify the ompA gene in Acinetobacter baumannii. After treatment with Bacillus licheniformis supernatant, the ompA gene expression was compared before and after treatment with real-time PCR technique.
Ethical Considerations This study was approved by the ethics committee of Islamic Azad University, Qom branch (Code: IR.IAU.QOM.REC.1398.004).
Results: Among the study probiotics, Bacillus licheniformis supernatant had the best antimicrobial properties against nosocomial isolates of Acinetobacter baumannii A52, Acinetobacter baumannii ATCC19606, Kelebsiella pneumonia ATCC70063, and Pseudomonas aeruginosa PAO1. Bacillus licheniformis supernatant also significantly reduced the biofilm formation and ompA gene expression in Acinetobacter baumannii.
Conclusion: Bacillus licheniformis can produce substances with antimicrobial and antibiofilm properties. It can be used for controlling the causative agents of nosocomial infections.

Abstract

Background and Aim The use of allograft alone or in combination with other bone materials has significantly improved bone-augmentation procedures. Therefore, this study aimed to evaluate the effect of topical gentamicin solution (80 mg) with allograft on guided bone regeneration in defects in rat skull. 
Methods & Materials Twelve Wistar mice were randomly divided into two groups, with 6 in each group. Three defects were created in their skulls, one filled with demineralized freeze-dried bone (DFDBA) allograft alone, the other with allograft with gentamicin 80 mg solution, and the third defect were left empty as a control group. After 4 and 10 weeks, the mice were killed for histological specimens. Histological and histomorphometric evaluations were performed to qualitatively and quantitatively assess bone formation, a new bone type made, new bone vitality, inflammatory response, connective tissue type, and angiogenesis.
Ethical Considerations This study was approved by the ethics committee of Arak University of Medical Sciences (Code: IR.REC.ARAKMU.1397.30).
Results At the end of 4 and 10 weeks after surgery, the average percentage of ossification in the area of ​​defects was higher in the group that received allografts with gentamicin, and during this period, a significant difference was seen between these groups and the control group.
Conclusion According to the present study, the use of gentamicin in combination with DFDBA has a significant effect on bone regeneration in defects in the rat skull.

Abstract

Introduction: Depression is an injury to the central nervous system that is caused by several factors that affect a large number of people around the world. Emotion regulation refers to the processes by which we influence what emotions we experience, when we experience them, and how we express them. The aim of this study was to investigate the role of emotion regulation in predicting depressive symptoms in students of Kermanshah University of Medical Sciences.
Methods: The present study is a descriptive correlational study. Participants were all students of the School of Nursing and Midwifery of Kermanshah University of Medical Sciences during 2020-2021. Three hundred and five participants were selected by available sampling method and randomly. Informed consent forms got from the participants, then they completed the Beck Depression Inventory - Second Edition (BDI-II) and the Cognitive Emotion Regulation Scale (CERQ-18). This research was reviewed in the ethics committee of Kermanshah University of Medical Sciences and approved with the ID IR.KUMS.REC.1400.623.
Results: In this study, the study population was 305 people, 9 of whom were excluded from statistical analysis due to incomplete answers. Total mean age was 50.08 ± 191.30 Of these, 198 (66.9%) were female and 98 (33.1%) were male. Self-blame strategies (β = 0.31, P≤0.001), positive reassessment (β = -0.26, P≤0.001), catastrophic (β = 0.26, P≤0.001), numerical importance (β = -0.12, P≤ 0.001) and positive refocus (β = -0.10, P≤0.001) in five steps predict 40% of the variance of depression. Inconsistent strategies such as self-blame and catastrophe directly and positive reassessment, insignificance and positive refocus inversely predict depression. Mahalanobis distance was used to investigate multivariate throw values. Statistical assumptions were tested by performing an initial regression analysis and evaluating the scatter plot of residues. After confirming the regression assumptions, the data were analyzed using Pearson correlation and stepwise regression. The findings of our study showed that there is a significant direct relationship between self-blame and catastrophic thinking as maladaptive strategies of cognitive emotion regulation with depression. Also in our study among adaptive emotion regulation strategies; Positive refocusing, positive reassessment, and a small number had a significant negative correlation with depression.
Conclusions: The results of our study showed that emotion regulation strategies play an important role in predicting the occurrence of depression. Therefore, it is necessary for health policy makers to strengthen adaptive emotion regulation strategies in designing and modifying interventions related to depression.

Abstract

Introduction: The fundamental role of marital satisfaction is vital in family formation and marriage survival. Marital dissatisfaction can make couples fail to solve their conflicts. Marital discontent, combined with job-related stress such as pre-hospital emergencies, can lead to heightened conflict in job responsibilities. It leads to the effect on personnel, patients, and their families. Therefore, this study aimed to investigate the marital satisfaction of pre-hospital emergency personnel at Qom University of Medical Sciences in 2023.
Methods: This cross-sectional study was conducted on 200 pre-hospital personnel of Qom University of Medical Sciences in May 2023. The sampling method was a stratified cluster. The data collection tool included the "Demographic Information" checklist and Enrich's "Marital Satisfaction" questionnaire. Data analysis was done using descriptive tests, Independent T-test, and Regression.
Results: The average age of the participants was 37.3 ± 7.06. More than half of the participants had a bachelor's degree. Most people had rotating shifts. The average score of marital satisfaction was 145.94 ± 22.21. Based on the results of the multivariate regression test, the variables of education, employment status, work experience, type of shift, having a second job, and having a job other than medical staff explain 63% of the variance of marital satisfaction of pre-hospital personnel.
Conclusions: According to the findings of the present study, in general, the marital satisfaction of pre-hospital emergency personnel of Qom University of Medical Sciences is at an average to high level. People with a fixed shift, with a second job, high work experience, without a fixed job status, and bachelor's and associate's degrees had less marital satisfaction than others. Therefore, managers and policymakers in the health field must pay more attention to the quality of family life, especially the married life of pre-hospital emergency workers with the mentioned conditions.

Abstract

Introduction: The purpose of the "Health Communication" discussion plan, the most serious sparks of which were struck in the minds with the emergence of the COVID-19 pandemic in 2019, revealed the importance of paying attention to improving health communication more than ever. For this purpose, in the current study, the formulation of a conceptual model of health communication capacities in pandemic conditions was examined from the perspective of health system experts.
Methods: This research was conducted using a qualitative approach and the foundation's data theory method. Sampling was done with a purposeful snowball method, which included 20 communication and media professors and health professionals. Data collection was done through semi-structured interviews and document reviews based on the systematic approach of Strauss and Corbin (open, central, and selective coding), which includes causal conditions, central idea, context, mediating conditions (intervenor), strategies, and consequences of analysis. Validation was done through the review of participants and experts in the health system.
Results: The findings of the research in the grounded theory section show that the dimensions of the paradigm model include the central phenomenon: health communication (effective and informative communication), causal conditions: health communication harms (infodemic), the context of harms (widespread and popular media, lack of authority of the health unit, mass information, politicization), strategies (holding officials accountable, reliable sources and producing practical content, anxiety management, avoiding exaggeration of people's participation, teamwork and combination of experts, using the experiences of other countries) and consequences including (attracting trust and forming beneficial relationships) took place.
Conclusions: According to the dimensions achieved in presenting the model for improving health communication capacities in pandemic conditions in the country, it can be said that on the one hand, "transparency and immediacy of information" and the other hand, "empathy and trust between people and officials" in such More than one situation should be considered.
