Abstract

Introduction: Optimum management of medical equipment in hospital settings has tremendous effects on decreasing costs and providing desired health and treatment services. This research has been undertaken to design a medical equipment management model for public hospitals of Iran. Materials and Methods: In this adaptive research, medical equipment management among United States of America, United Kingdom, Germany, Canada, Australia, Sweden and Italy, which have successful experiences and determined patterns for medical equipment management, are assessed with respect to 6 dimensions as structure, purchase, control, maintenance, training and distribution and according to the shared strategies in the mentioned countries, a model was designed for Iran's public hospitals. This model was validated using expert's survey in DELFI method. Results: In all of the studied countries, the medical equipment engineering unit is established for effective management of medical equipments and in direction with the main purpose of ensuring safety and improving patient care, clinical engineers are being trained. Trainings are designed according to international federation course plan for clinical engineers “which are combination of necessary engineering and managing courses for activities of hospital medical equipment management”. Under study issues include: precise need assessment for medical devices, balancing, use of life span expenditure, stock management, location finding, and needed physical space prediction, selection and purchase committee establishment, entering controls plus reception test, performance and safety control, activities documentation, using computerized systems for preventive maintenance and semi-centered distribution, are among necessary processes in the medical equipment management cycle of under studied countries' hospitals. Conclusion: Management and medical equipment engineering unit establishment in hospitals, selection and purchase committee formation, continuous equipment performance control during calibration process, equipment preventive maintenance according to workforce training schedule, training needed experts, and with respect to the topics of international federation courses for clinical engineering training in universities, will improve medical equipment efficiency in hospitals.

Abstract

Background: Medical equipment improvement and tremendous expenditure is allocated costs annually, so that optimum and proper maintenance management of equipment would have outstanding effects on health and treatment of medical training and research economies of the country. Present research was implemented aiming to examine effect of the medical equipment maintenance management systems on the hospital setting expenditures. Methods and Materials: In an interventional research, the model of medical equipment maintenance management system implemented in Imam Khomeini Hospital was examined precisely and based on appropriate software for Vali-e-Asr Hospital was designed. According to the model for all available medical instruments in Vali-e-Asr hospital with usage of more than 5 years (240 medical equipment) all costs were recorded in 2006 and compared with previous year (2005). Results: There wasn't any training for medical equipment operators and for its implementation necessity on 21.7% of equipment in the installation and start up phase with for 83.7% of equipments the operator was not fixed using of medical equipment maintenance management system, the repair events decreased from 78 events in 2005 to 58 events in 2006. Vali-e-Asr hospital recorded costs for maintenance and repair of medical equipment in 2006 was 801765375 Rials(Iran)which decreased to 513212912 Rials (Iran) according to accurate calculation of costs and auditing by medical equipment engineer and shows 36% saving in expenditures by medical equipment maintenance management system. Conclusion: Medical equipment computer–based maintenance, management system implementation and also using of medical engineers potentialities in hospital are necessary.

Abstract

Background: Delay in treatment of acute myocardial infarction has an important effect on developing the myocardial damage and the prognosis of the patients. In this study, interval between the onsets of symptoms until hospitalization was studied on patients who reffered to fatemeh Zahra hospital of Bushehr port. Methods and Materials: This is a cross-sectional analytic study, carried out on 120 patients, with acute heart attack in Bushehr hospital by using a sampling during 9 month period. A questionnaire was used consisted of three sections: first demographic information second, questions about time Interval between the Beginning symptoms to hospitalization and third questions about cause of delay that it was completed by inteviwe method. Results: Results showed that the total length of time between the onset of symptoms and hospitalization EU was 273/33±259 minutes. There was significant relation between positive family history, past history of CAD, having insurance, chief complain, going to physician clinic before hospitalization and age with the delay timed (P<0.05). The most common cause of delay was lack of knowledge about symptoms and inattention toward their importance. Conclusion: Time Interval between the onsets of symptoms to hospitalization approximately was 4.5 hours that was not acceptable. Findings indicate developing educational approaches to increasing patient’s knowledge is necessary for better understanding of symptoms in the onset of MI that hereby with the delay time reduces.

Abstract

Background: Hospitals need performance evaluation systems for development and competition in order to evaluate the effectiveness and efficiency of their plans, processes and human resources. The objective of this research is to design an evaluation system at hospitals through the use of the Balanced Scorecard. Methods and Materials: In this comparative study, the selected countries consisted of Australia, USA, Canada and New Zealand, and 34 public hospitals of Iran which were applying strategic planning. Performance evaluations were examined in terms of 6 aspects including mission, vision, strategies, perspectives, objectives and measures .A pattern was designed for performance evaluation of public hospitals in Iran and was validated using experts’ survey of DELFI method. Results: All countries had mission and vision as important aspects of their performance evaluation model while health effectiveness promotion was the most important strategy. High quality health services, resource development, innovation and guaranty in continuous services were other strategies. Perspectives of Balanced Scorecard included customer, internal processes, learning and development of staff and financial affairs. All of the countries had objectives and measures for each of the perspectives. Iranian hospitals had mission, vision, strategies and objectives but for them perspectives had not been defined and measures were not congruent with strategies and objectives. In the model, 4 perspectives were determined to describe hospital strategies and to use competition benefits as well. Conclusion: Applying measures of 4 perspectives including "Patients and Community", "Internal process and Productivity", "Growth and innovation" and "financial aspects" to doevaluation of Iranian public hospitals collates their performance, improves productivity of their resources and enhances patients and community satisfaction.

Abstract

Background: Job satisfaction is regarded as an individual's opinion of his or her job. Noticing the key role of hospital personnel in giving health services to patients, this study was designed to determine job satisfaction of hospital personnel in Arak. Materials and Methods: This cross-sectional study was carried out on 702 hospital personnel through simple sampling method. The means of gathering data were demographic and Herzberg's job satisfaction questionnaires. Job satisfaction was categorized in three levels. Results: Results in eight domains showed that most personnel's satisfaction of job environment (%75.2), relationship with co-workers (%49), salary and benefits(%75.1), job position(%60.5), job security (%64.6), supervision(%64),management policy(%59.2)and personal life relations (%65.7)were at average levels. Overall, %82.2 of the personnel had an average satisfaction with their jobs. The greatest level of satisfaction was with relationship with co-workers and the lowest level of satisfaction was related to job difficulty benefits, establishment of justice and fairness, discrimination and lack of facilities. X2 statistical test revealed significant differences amolay sex, level of education, type of employment, type of job and work shift. Conclusion: Overall, Job satisfaction in this study was at an average level. Increasing salaries, job difficulty benefits, on time payment and providing welfare facilities are essential for the improvement of job satisfaction in this group.

Abstract

Background: This study was designed to investigate the quality of life in staff and hospital personnel of Arak University of Medical Sciences and to gain basic information for carrying out interventions for improving their quality of life. Materials and Methods: This cross-sectional study was done to determine quality of life of 300 staff and hospital personnel in Arak University of Medical Sciences. Data gathering tools were demographic and standard quality of life (SF-36) questionnaires. Results: Of the 275 participants who had completed the questionnaires, 127 (%46.2) were staff and 148 (%53.8) were hospital personnel. The means of age and years of experience were 36.797.02 and 12.297.39 years, respectively. The mean of different dimensions of quality of life in the staff was significantly much higher than that in the hospital personnel (p<0.05). In terms of physical performance, vitality, social performance, general health, physical and mental health domains, significant differences were found among personnel with different employment status. There was also a significant difference between men and women in terms of physical restriction factors, physical agony, vitality, sanity, mental and physical health. Conclusion: The mean of different dimensions of quality of life in hospital personnel was lower than the staff while this value was lower in women than men. Specific longitudinal studies are needed for investigating the causes of these differences.

Abstract

Background: Hospital wastewater usually contains pathogenic micro-organisms, drugs, radio active materials, and toxic chemical elements that in case of being disposed without filtration, will result in pollution and spread of diseases. Therefore, evaluating the status of wastewater disposal in hospitals is necessary for proper planning. Materials and Methods: In a cross-sectional descriptive study in 2008, data collection was done by means of interviews, observations, and questionnaires in all hospitals of Markazi Province. Noticing the 33 items in the questionnaire, the overall conditions of the hospitals and the methods of using the sewage system were studied, and at the same time, filtration installations and wastewater disposal systems were inspected. Results: Of all the 19 hospitals, 10 used a conventional method of direct disposal of sewage to subterranean shafts, and of the 4 hospitals with filtration installations, three were practically out of order or were not efficient. Five of the hospitals were directly linked to the city sewage system without any pre-filtration. None of the hospitals had a specialized operator of sewage filtration and disposal system. Conclusion: Wastewater disposal in hospitals of Markazi Province is not desirable therefore, such acts as conducting more specific surveys, creating a positive attitude in planners, and providing sufficient funds for filtering wastewater in some of the hospitals are suggested.

Abstract

Background and aim: Schizophrenia is a chronic illness, and one of the major health problems in the world. So the select of different treatment methods that is appropriate for community culture is very important and necessary issue. The purpose of this study was to investigate the effect of religious training (prayer) on quality of life of schizophrenic inpatients. Materials and methods: 62 schizophrenic inpatients were assigned randomly to two groups: experimental and control. Data were collected via Schizophrenia Quality of Life Scale (SQLS). After two months religious training (prayer) for experimental group (three times a week), all of the subjects were assessed again. Findings: The findings showed that, before intervention there was no significant difference between two groups (t=-1/78, p=0/08). After intervention (religious training), there was significant difference in experimental group (t=2/09, p=0/04). Despite the difference in experimental group, there was no significant difference in control group (t= 1/50, p= 0/14). Conclusion: This study showed that, religious and spiritual training can be increase the satisfaction of quality of life schizophrenic patients.

Abstract

Background: The holy Quran is the curative that after its cure, there is no fear of disease anymore. One of the miracles of Quran is its effect on service and medical care duties. Noticing the key role of nurses in offering health service to patients and the importance of the nurses’ mental health, this study was conducted. Materials and Methods: This educational trial was done on 55 nurses at educational hospitals of Arak University of Medical Sciences through randomized simple-sampling. Before and after intervention, the GHQ-28 questionnaire was completed by the participants. Data were analyzed through descriptive statistics, paired t-test, and Chi square. Results: The mean age of the nurses was 34.45±6.3 years, and 68.2% (30 participants) of them were married and the rest were single. The mean mental health scores before and after intervention were 27.75±9.76 and 22.34±7.83 which indicated a significant difference (p<0.014). Conclusion: The findings of this study suggest that this intervention has been effective in reducing mental health scores and somehow, in improving the nurses’ mental health status. On the other hand, participation in the familiarity with Quran workshop can improve the nurses’ mental health.

Abstract

Background: Measurement of service quality is a basic prerequisite for improving quality. Studies show that consumers are in close contact with services quality better than any other group. This study was conducted with the aim of measuring service quality based on patients’ expectations and perceptions teaching hospitals in Arak, Iran. Materials and Methods: This cross-sectional study was done on 260 patients at under auspices of Arak University of Medical Sciences. For data collection SERVQUL questionnaire was employed to measure quality of services. The reliability and validity of this questionnaire had already been confirmed by studies both in Iran and other countries. Results: The results showed that there were significant differences between patients’ expectations and perceptions in all dimensions of quality. The accessibility dimension constituted the most serious problem at hospitals. Conclusion: The negative scores show that it is necessary to promote all quality services dimensions at hospitals. For reducing the qualitative gap among all dimensions, it is recommended that hospital managers take patients’ needs into account and provide desirable services for them.

Abstract

Background: Hospitalization of a child in pediatric intensive care unit (PICU) is a very stressful experience for parents. High stress in father can product decline in psychological adaptation and associated problems. The present study aimed to determine effect of fathersʼ participation in the care of hospitalized child in PICU on fathersʼ stress and coping strategies.

Materials and Methods: This is a clinical trial with 60 fathers with children admitted to the PICU in intervention and control groups. Fathers in intervention group, participated in the care of the child in 5 times, during a 5-days course. Fathersʼ stress and coping strategies were surveyed in intervention group before and after participation course and in control group before and after a 5-days course.

Results: Fathersʼ stress in intervention group decreased significantly after participation (p˂0.001) but in control group difference was not significant after time course. Fathers in both groups applied similar coping strategies that majority were emotion focused.

Conclusion: Fathers having children in PICU would experience a lot of stress. Their participation in the care of the children would be very effective to decline their stress and reach most adaptation with disease, cure and cares in children.

Abstract

Background: Drugs residual discharge into the environment through municipal and hospital wastewaters is one of the emergent environmental problems. Imipenem as a professional hospital antibiotic is widely used against gram- positive and negative bacteria and with entrance to the aquatic environments could prompt a lot of risks such as bacteria resistance, allergies, spoiling alga and daphnia and interrupting in wastewater treatment processes. Therefore there is a command to develop a method for extraction and determination of Imipenem from hospital sewage.

Materials and Methods: Solid phase extraction (SPE) was used to extract Imipenem from samples. Recovery percentage calculated at different pH of 3 and 7. The extracted samples analyzed by High Performance Liquid Chromatography (HPLC) equipped with UV detector. HPLC operated using borate buffer/methanol as mobile phase at flow rate of 0.7 ml/min, column temperature of 20 °C, and UV wavelength of 280-300 nm.

Results: Maximum recovery percentage was obtained 68% at pH=7. The best condition for HPLC was 80:20 ratio of borate buffer/methanol with pH=7.5 and at UV wavelength of 300 nm. Linearity calculated 0.9829, primary and intermediate precision both were more than 95%. Limit of detection (LOD) and limit of quantification (LOQ) were 3 and 10 µg/l respectively.

Conclusion: The method could simply and with significant reliability be applied to extract and determinate Imipenem in complex hospital wastewater matrixes.

Abstract

Background: Under-nutrition has multiple negative impacts on child's growth and development and it has a high prevalence among hospitalized children. Assessment of the food insecurity status is one of the most important indicators of malnutrition among communities, therefore the aim of the current study is to evaluate the prevalence of food insecurity and its social, economical and nutritional determinants among children hospitalized in Tabriz pediatric hospital.

Materials and Methods: The current cross-sectional study was carried out among 236 hospitalized children in Tabriz pediatric hospital. Demographic information including age, weight, height, household size, child order and household income were gathered by face to face interviews with parents. Food security was assessed by a Household Food Security questionnaire. Statistical analysis was performed by Excel 2007, Epi-info and IBM SPSS Statistics 21 software.

Results: The prevalence of food insecurity among hospitalized children in Tabriz pediatric hospital was 44.9%, while 26.3% and 18.6% had moderately and severely food insecurity, respectively. Its prevalence in females was more than in males. Low household income, low educational attainment of parents, rural residency and household size were potent determinants of food insecurity(p<0.05). All indices of nutritional status in food insecure group were lower than in secure group. Although this difference did not achieved significant threshold.

Conclusion: In the current study, a high prevalence of food insecurity in hospitalized children in hospital has been reported. Therefore the need for suitable interventions to reduce its prevalence has been warranted.

Abstract

Abstract
Background: Nanotechnology offers a great chance to treat drug-resistant microbial infections. The purpose of this study was to synthesize amoxicillin-encapsulated polymeric nanoparticles and compare the antibacterial activity of this nanoformulation with pure amoxicillin.
Materials and Methods: Amoxicillin-encapsulated polymeric nanoparticles were prepared using chitosan as a polymer and alginate as cross linking agent. The obtained nanoparticles were characterized by Fourier transform infrared, dynamic light scattering and scanning electron microscopy. The antibacterial effects of the nanoparticles were evaluated using broth microdilution and well diffusion methods against some common bacterial strains involved in hospital-acquired infections.
Results: The drug-encapsulated nanoparticles were found to be spherical in shape with average size of 96 nm. These nanoparticles had a significant antibacterial effect on all tested bacteria, except for Pseudomonas aeruginosa. They also displayed stronger antibacterial activity than the nanocarrier alone or free antibiotic. The highest mean zones of growth inhibition (23.7 mm) for methicillin-sensitive Staphylococcus aureus and the smallest zone (12.5 mm) for the resistant species of this bacterium were determined. MIC of the nanoformulation against these two strains was respectively determined at 1.5 and 48 µg/ml and for Escherichia coli and Pseudomonas aeruginosa at 6.6 and 256 µg/ml.
Conclusion: The results suggest that amoxicillin encapsulation in polymer nanoparticles has the potential to increase its antibacterial activity against bacteria causing nosocomial infections.

Abstract

Background and Aim: Training clinical students who learn the theoretical lessons in hospitals in practice will play an important role in improving treatment processes and providing better services to patients. Therefore, the purpose of this study is to identify the factors affecting the quality of clinical education in hospitals in Saveh City.
Methods & Materials: This research is a combination of exploratory studies and exploratory projects. In the qualitative stage, the qualitative method of Grand Strauss and Corbyn 1998 was used to explain the quality of clinical training. Since the students who have taken clinical courses in the Faculty of Medical Sciences of Saveh Hospitals in 1398-1397 are about 200 people and the professors and educators who teach these people are about 20 people, which according to Karajsi and Morgan, the sample size should be 140, of which 20 were interviewed and 120 were filled out with paper and electronic questionnaires. In this study, semi-structured interviews, and a researcher-made questionnaire with Cronbach’s alpha coefficient of 0.598 were selected as the data collection method. This research was performed using the qualitative method and using MAZQDA 10 and SPSS 26 software, the items obtained from qualitative research were collected and exploratory factor analysis was used to identify the factors.
Ethical Considerations: Considering that this study is one of the research priorities of Saveh School of Medical Sciences, it has a research license letter (No. 24.127. D.99).
Results: The results showed that the factors affecting the quality of clinical education include five factors. The first factor is space and facilities, the second factor is the improvement of educational processes, the third factor is educational and managerial deficiencies, the fourth factor is the evaluation of the teacher and the fifth factor is the qualitative improvement of education.
Conclusion: According to the quantitative and qualitative results of this study and by interviewing twenty experienced professors and student representatives, the factors affecting the quality of clinical education in Saveh hospitals were identified.

Abstract

Background and Aim Crisis management is an important field of management, especially in the field of health. This study aims to investigate the factors affecting the development of disaster management mechanisms in hospitals of Arak, Iran.
Methods & Materials In this regard, 240 questionnaires were distributed among hospital and nursing managers in Arak. The questionnaire had acceptable reliability with Cronbach’ alpha  value of 0.897. 
Ethical Considerations This study was approved by the ethics committee of Arak University of Medical Sciences (code: 1274.1401.REC.ARAKMU.IR). 
Results Then, by using structural equation modeling in LISREL software, the designed model was analyzed. Finally, by using statistical tests, the research hypotheses were examined. 
Conclusion The results showed the significant effect of three factors of “lack of stability in business environment”, “risk taking”, and “organizational culture” on the development of the mechanism of crisis management.

Abstract

Background and Aim This study aims to investigate the effect of nurses’ autonomy on the quality of patient care based on Kramer and Schmalenberg’s model
Methods & Materials This is a causal-comparative study. Participants were 101 nurses of Imam Khomeini Hospital in Arak, Iran from the intensive care unit and internal medicine, surgery, pediatrics, gynecology, orthopedics, and emergency departments. The data collection tools were Moghimi and Ramazan’s patent care quality questionnaire and an autonomy assessment questionnaire based on Kramer and Schmalenberg’s autonomy model. The data were analyzed in SPSS software.
Ethical Considerations This study was approved by the ethics committee of Islamic Azad University of Arak Branch (Code: IR.IAU.ARAK.REC.1397.66).
Results The mean autonomy score of nurses was 3.16 ±0.35, indicating a desirable level. Nurses of all departments had limited autonomy. In the internal department, nurses had unsanctioned autonomy, while in the surgical department, they had autonomous nursing care action. The regression models were significant for all departments except for the internal department. With an increase of one unit in limited autonomy in the intensive care unit, autonomous nursing care in the surgical department, and limited autonomy in pediatrics, gynecology, orthopedics, and emergency departments among the nurses, the quality of care can be increased by 0.253, 0.187, 0.522, 0.205, 0.466 and 0.129, respectively.
Conclusion Nurses’ autonomy can increase the quality of patient care. The nursing managers need to empower nurses and provide an environment for them to apply their knowledge in practicing autonomy.

Abstract

Introduction: Patients with COVID-19 can have various and complex coagulation disorders that cause hypercoagulable conditions. Investigating the role of anticoagulants with different doses in the outcome of patients with COVID-19 seems to be needed. Therefore, the present study was conducted with the aim of predicting factors of severe disease, including the number of lymphocytes, LDH, and the recovery status of patients. 
Methods: A cross-sectional study was conducted in patients with COVID-19 hospitalized in Hazrat Rasool Akram (PBUH) Hospital. Study entry includes acute respiratory symptoms caused by COVID-19 disease (uncontrollable cough, dyspnea, prolonged medicine and blood saturation drop) along with lung involvement in chest spiral CT scan without injection, SARS RT-PCR–CO virus test was positive and transfer to intensive care unit (ICU) was indicated. Anticoagulant starting time, type and dose of anticoagulant received and duration of use were recorded. Two groups of prophylaxis dose, higher dose than prophylaxis (medium dose and therapeutic dose) were divided.
Results: Out of the total number of patients referred to Rasul Akram Hospital, 90 people (43 women (47.8%) and 47 men (52.22%)) participated in this study. The average hospitalization in the ICU was 13.50 ± 12.39 days and 17.18 ± 13.36 days in the hospital. Also, the duration of coagulant therapy was 17.13 ± 13.38 days. 53 patients (58.9%) received a prophylactic dose, 28 patients (31.1%) received a therapeutic dose, and 9 patients (10%) received a moderate dose of anticoagulant. Anticoagulant in 74 patients (82.2%) was unfractionated heparin (UFH) and in 16 cases (17.8%) low molecular weight heparin (LMWH). The changes of LDH, number of lymphocytes, dimer, fibrinogen, ferritin and FDP according to the dose of anticoagulant assessed in the three groups of prophylaxis, treatment and no statistically significant difference. The mean of ICU admission days according to different doses of anticoagulant did not show a statistically significant difference.
Conclusions: Finally, the findings obtained from the study showed that the use of different doses of anticoagulant does not cause a difference in the level of LDH, number of lymphocytes, ferritin and FDP at different measurement times. Also, the length of stay in the hospital and the time of hospitalization in the intensive care unit did not differ between the groups receiving prophylactic anticoagulant, medium dose and therapeutic dose.

Abstract

Lack of preparation for fires can lead to irreversible damage. Therefore, fire prevention is crucial in effectively managing this risk. It is essential to implement measures to address fire incidents. Fire safety in hospital is particularly critical due to factors like the presence of disabled individuals who may not be familiar with emergency exit points, as well as the specialized medical equipment present. Fires in hospitals can result in extensive harm, including injuries to staff and patients, a loss of trust in local authorities, and the subsequent failure to deliver adequate healthcare services. Adhering to fire safety principles and fundamentals can help mitigate the impact of fires in all locations, especially medical facilities.

Abstract

Introduction: The fundamental role of marital satisfaction is vital in family formation and marriage survival. Marital dissatisfaction can make couples fail to solve their conflicts. Marital discontent, combined with job-related stress such as pre-hospital emergencies, can lead to heightened conflict in job responsibilities. It leads to the effect on personnel, patients, and their families. Therefore, this study aimed to investigate the marital satisfaction of pre-hospital emergency personnel at Qom University of Medical Sciences in 2023.
Methods: This cross-sectional study was conducted on 200 pre-hospital personnel of Qom University of Medical Sciences in May 2023. The sampling method was a stratified cluster. The data collection tool included the "Demographic Information" checklist and Enrich's "Marital Satisfaction" questionnaire. Data analysis was done using descriptive tests, Independent T-test, and Regression.
Results: The average age of the participants was 37.3 ± 7.06. More than half of the participants had a bachelor's degree. Most people had rotating shifts. The average score of marital satisfaction was 145.94 ± 22.21. Based on the results of the multivariate regression test, the variables of education, employment status, work experience, type of shift, having a second job, and having a job other than medical staff explain 63% of the variance of marital satisfaction of pre-hospital personnel.
Conclusions: According to the findings of the present study, in general, the marital satisfaction of pre-hospital emergency personnel of Qom University of Medical Sciences is at an average to high level. People with a fixed shift, with a second job, high work experience, without a fixed job status, and bachelor's and associate's degrees had less marital satisfaction than others. Therefore, managers and policymakers in the health field must pay more attention to the quality of family life, especially the married life of pre-hospital emergency workers with the mentioned conditions.
