Abstract

Background: Vesicoureteral Reflux (VUR) is the most common urologic anomaly in children. In spite of prescribing prophylactic antibiotics in children with VUR, in some cases breakthrough infection (BTI) occurs while receiving prophylactic antibiotics which may cause pyelonephritis that can ultimately lead in renal scarring.

Materials and Methods: In this discreptive-analytic study, 288 children with primary vesicoureteral reflux under Cotrimoxazole(2mg/kg) treatment have been under supervision for duration of at least one year. Based on the collected data, predictive effect of some factors like gender, mean age of VUR diagnosis, VUR grade, unilateral versus bilateral and presence of abnormal renal scar on the breakthrough infection under prophylactic treatment, were examined.

Results: Breakthrough infection while receiving prophylactic antibiotics was observed among 111(38.54%) patients consisting of 88 girls (79.28%) and 23 boys (20.72%). The moderate to high VUR grades (grade III-V), presence of renal scar and female gender were found to be the important risk factors for BTI. However, no statistically significant relationship between mean age of VUR diagnosis and unilateral versus bilateral VUR with BTI was found.

Conclusion: Results of the current study will help improving parents' and physicians' awareness of the risk factors associated with BTI which may potentially lead to renal damage.

Abstract

Background: There have been numerous reports of snake venoms being employed as analgesics in attempts to relieve severe pain associated with cancer, immune dysfunction and viral infections. This study investigates the antinociceptive effects of iranian cobra snake venom (Naja naja oxiana) in comparison with morphine and lidocain on laboratorial femal mice.

Materials and Methods: This study has been done on 48 NMRI female mice of 18-20 g in weight. Antinociceptive activeity of snake venom was evaluated by formalin test. In this test, the animals were divided into 6 groups (each group consisting of 8 mice): Sham, positive Control (receiving morphine at dose of 5 mg/kg, and receiving lidocain at dose of 20 mg/kg), and experimental groups receiving venom at doses of 1, 3 and 4.5 µg/mice. In all groups, the formalin test was recorded for 60 min after administration of venom and drugs in mice. Data were analyzed using one-way ANOVA and Tukey test.

Results: The results showed that the venom of Naja naja oxiana decreased nociception meaningfully in both acute and chronic phases. We also showed that this venom revealed even a better analgesic activity in comparison with morphine and lidocain.

Conclusion: This study showed that the antinociceptive effect of the venom was mediated through central nervous system and peripheral mechanisms. Although details of the mechanism remain unclear, and further studies should be considered to demonstrate its therapeutic effects.

Abstract

Abstract:
Background
CAP ( community acquired pneumonia ) is one of the most common infectious diseases that leads to hospitalization of patients. Also, the effects of vitamin D deficiency on the functioning of the immune system and its association with infectious diseases such as CAP have received much attention in recent years. Therefore, we decided to investigate the relationship between serum vitamin D levels and the incidence and severity of CAP pneumonia in this study.
Methods
In this case-control study, 75 patients with CAP  and 75 healthy individuals were evaluated and serum levels of vitamin D in these two groups were measured and compared.
Results
The present study shows that for each unit increase in serum vitamin D level, the need for hospitalization in the ICU according to the IDSA criterion is reduced by seven percent and according to the CURB65 criterion, the need for hospitalization in the ward and ICU is reduced by fifteen percent. Vitamin D deficiency also increases the chance of developing community-acquired pneumonia by 3.08 times. And for every unit increase in vitamin D, the chance of developing community-acquired pneumonia decreases by 4 percent. And in people with vitamin D deficiency, the chance of being admitted to the ICU according to IDSA is 2.05 times and according to CURB65 is 2.84 times.
Conclusions
Overall, based on the results of this study, it is concluded that vitamin D deficiency is associated with an increased incidence as well as a greater severity of community acquired pneumonia.

Abstract

Introduction: Maternal folate supplementation during pregnancy is associated with reduced risk of several fetal neurodevelopmental disorders. However, it is not well known that excess folate intake from diet and supplements can impair neurodevelopment and behavior in offspring. Therefore, the aim of this study is to investigate the effect of chronic and high doses of folic acid before and during pregnancy in female rats on learning and spatial and avoidance memory in male and female offspring.
Methods: 24 female Wistar rats received doses of 0.5, 1, and 2 mg folic acid by intraperitoneal injection two weeks before and during pregnancy. The control group received normal saline. Male and female offspring were divided into 8 groups. Learning behavior and spatial memory were measured by Morris blue maze test, avoidance memory by shuttle box test. The results showed that taking a dose of 2 mg folic acid before and during pregnancy causes spatial learning deficits in male offspring.
Results: While spatial memory is unchanged compared to the control. This dose of folic acid also causes a disturbance in avoidance memory in both male and female offspring.
Conclusions: Our results suggest that high doses of folic acid supplements during early life (fetal) have the potential to impair neurological functions such as memory. Although the severity of this disorder can depend on the gender of the child.
