Abstract

Introduction: Kikuchi-Fujimoto disease or histiocytic necrotizing lymphadenitis is a rare but important etiology of cervical lymphadenopathy in young women. This disease is self limited and improves during 2-8 months. Diagnostic key is the pathology of involved lymphnode. Association of the disease with viral and autoimmune disease is not proved yet. In this article two cases of Kikuchi-Fujimoto disease are reported. Case: The first case was a 31 years old female patient with sever fever and right axillary lymphodenpathy (size: 57cm) in which one month after the disease, diagnosis was proved by pathology studies. This patient has erythematous patches in her face and nose in several times that were improved spontaneously. The second case was a 20 years old female patient with fever, chill, headache, weigh loss and cervical lymphadenopathy (size: 21cm). She received antibiotic at several times without any improvement. Finally after 5 months since the beginning of symptoms, the disease was diagnosed as Kikuchi-Fujimoto disease by surgical excision of lymhnode and pathology studies. After the surgery two small lymphnodes were appeared around the surgical site. Conclusion: In differential diagnosis of lymphadenpathy especially cervical in young women, Kikuchi-Fujimoto should be considered. Consumption of antibiotic in lymphadenopathy without clear diagnosis should be avoided. After diagnosis of Kikuchi-Fujimoto, long term work up for recurrence or the appearance of autoimmune disease symptoms is necessary.

Abstract

Background: Breast cancer is the most common cancer among women. An important goal for cancer patients is to improve the quality of life (QOL) by maximizing functions affected by the disease and its therapy. This study examined the effects of massage therapy on quality of life in women with breast cancer after surgery.

Materials and Methods: The study was quasi-experimental. Among the 53 women with breast cancer surgery and oncology clinic of Arak, 30 were randomly allocated in two groups of massage therapy (n=15) and 5 weeks (30 min, 2 sessions per week) and control (15 cases). Cortisol levels and quality of life questionnaire (SF36) was redirected and mental health groups. Data analysis software SPSS version 16 using paired t-test and one-way analysis of variance and a confidence level of 95% (p<0.05) was performed.

Results: The results showed a significant increase in the experimental group improved after the massage, physical (p=0.001) and psycho-social (p=0.002) were compared with control subjects. Massage also improves quality of life and mental health in breast cancer patients after surgery was (p<0.05). However, serum cortisol levels between the experimental and control groups were not significant (p>0.05).

Conclusion: The overall results of this study showed that massage therapy can be an effective intervention to improve quality of life and mental health in patients with breast cancer during radiotherapy is used.

Abstract

Introduction: Considering the importance of urinary tract infections caused by uropathogenic Escherichia coli (UPEC) in the medical field, this study aimed to investigate serogroups O25 and O16 and the pattern of antibiotic resistance among UPEC isolates obtained from hospitalized patients with urinary tract infections (UTIs) in Rasht hospitals.
Methods: A total of 110 urine samples were collected from patients with UTIs referred to selected hospitals in Rasht. The disk diffusion method, as recommended by the CLSI, was used to determine the pattern of antibiotic susceptibility. Serogroups O25 and O16 were detected using specific primers.
Results: Among the studied samples, 36.4% (40/110) were men and 63.6% (70/110) were women. Based on the antibiotic susceptibility pattern, a high level of antibiotic resistance was observed against nalidixic acid (81.8%) and co-trimoxazole (78.2%), while the most effective antibiotics were amikacin (85.5%) and nitrofurantoin (83.6%). In addition, multi-drug resistant phenotype was found in 72.7% (110/80) of UPEC isolates. According to PCR results, the frequency of serogroups O25 and O16 was 36.4% and 17.3%, respectively. Both serogroups had the highest resistance to nalidixic acid and co-trimoxazole, while the lowest resistance in serogroup O25 to nitrofurantoin (20%) and amikacin (14.3%) and in serogroup O16 to imipenem (5.3%) and nitrofurantoin (10.5%).
Conclusions: This study showed that the high prevalence of MDR strains among UPEC strains is very worrying and professionals should be very careful in prescribing antibiotics for patients. Like most studies, the frequency of serogroup O25 was high, and probably, this serogroup can play a role in causing urinary tract infections and antibiotic resistance of UPEC strains.
