Abstract

Background: Thymus is essential in creating some of the disorders and diseases. Treatment of these disorders (particularly tumors and myasthenia gravis) is possible with complete removal of the thymus.

Materials and Methods: In this retrospective study over a period of eleven years, all case of thymectomy performed at the Department of Thoracic Surgery of shahid Beheshti University has investigated. Variables examined included age, sex, primary diagnosis, surgical technique, complications and results of surgery.

Results: The total number of patients was 50 (46% male and 54% female) with a mean age of 85.14 ± 46.41 years. Primary diagnosis was myasthenia gravis in 40% of patients and thymoma in 42% of them. Myasthenic symptom was the most common presenting symptom (50%). Radical thymectomy surgery was performed in 64% of patients. Fever and atelectasis (10%) and respiratory failure (12%) were the most common postoperative complication and mortality rate was 4%. Thymoma (64%) and thymus hyperplasia (20%) were the most frequent pathology. Clinical symptoms were gone or greatly reduced in 40% of patients. The majority of them had a primary diagnosis of myasthenia gravis (36%). 50% of patients have been followed up. The mean follow-up time in these patients is 32.20 ± 36.41 months.

Conclusion: Thymectomy surgery with low morbidity and mortality, provides the possibility of treatment of thymic lesions. Especially Thymoma and Myasthenia Gravis

Abstract

Background: Height and weight growth failure are among the most important health disorders affecting children less than two in the developing countries. Failure to treat this disorder can lead to the development of serious conditions including increased mortality and types of disability. The aim of the present study is to investigate the factors affecting the height and weight growth of children less than two years old.

Materials and Methods: This historical cohort study was conducted on a sample of 2030 children less than two in Khorramabad who were selected using stratified and cluster sampling methods. Based on household records, data related to the height and weight were recorded as quantitative variables and finally were modeled using a longitudinal marginal model, identity link functions and the R software.

Results: Based on the marginal model, the effect of such variables as child's age, child's gender and mother's educational attainment were significantly related to the child's height and weight. Moreover the child's birth order had significant relationship with only the child's height. However, the variable of exclusive breastfeeding during the first six months was no significantly related to weight and height growth of the children.

Conclusion: Given the results, it seems that monitoring the growth of the children of the illiterate mothers, specially the girls and the children with first birth orders are the most important approaches to deal with growth disorders among this population.

Abstract

Abstract
Background: It is believed that sports exercises have minimal impacts on the pathological changes in the lung tissue exposed to chemical carcinogens in cigarette smoke. The aim of this study was to determine the effects of 12 weeks of swimming exercises on IL-6 concentration in the lung tissue of rats following the injection of carcinogen NNK.
Materials and Methods: This study is experimental in terms of the design. Forty-six Wistar rats were divided into five groups: A) Exercise (E), B) Exercise-NNK (EN), C) NNK (N), D) Control (C) and E) Vehicle (V). Exercise groups completed the swimming exercises for twelve weeks, five days per week (25-60min). Rats of vehicle and NNK treatment groups were respectively administered subcutaneous injections of distilled water and NNK (12.5mg/kg, once a week, 12weeks). Then, samples of the lung tissue were collected and IL-6 concentration was measured by ELISA technique. One-way analysis of variance and Tukey post hoc test were used for data analysis at the level of p ≤ 0.05.
Results: The results indicated a significant decrease of IL-6 concentration in exercise (p=0.009), exercise-NKK (p=0.006) groups and vehicle (p=0.006) with NKK groups; while there was no significant difference between exercise groups and the control groups. The swimming exercises can reduce IL-6 concentration significantly in lung tissue of rats following the injection of carcinogen NNK.
Conclusion: It seems that endurance exercises, along with other therapeutic methods, can play a role in reducing the effects of carcinogen NNK and negative effects of smoking by decreasing the inflammatory factor of IL-6.

Abstract

Background and Aim: Children with cerebral palsy (CP) may have musculoskeletal disorder due to inappropriate position during sleep. The aim of this study was to design the equipment for sleep positioning of children with CP.
Materials and Methods: This research was a designing device study for sleep positioning of children with CP. The design was carried out in three stages: needs assessment, develop an idea and final design. Seventeen children with CP aged 2-12 year olds at the levels of III to V Gross Motor Function Classification System Expanded and Revised (GMFCSE&R) were recruited from Tehran and Karaj rehabilitation clinics by a convenience sampling strategy. For data collection, some demographic questionnaire and GMFCS E&R were used.
Ethical Considerations: This study was approved in Research Ethics Committee of Faculty of Applied Arts at Tehran University of Art with the code IR. ART U.REC.1394.118.
Findings: In the process of needs assessment, a researcher made questionnaire and pictures of sleep positioning children with CP when they were sleeping were used. In developing an idea stage, the free and creative ideas under the name of black box, as well as, Etudes at the transparency stage were presented as the combination of creative ideas. In the final design stage, by using superior materials, positioning and packaging, we achieved a superior design.
Conclusion: The proposed plan is designed as an interventional tool based on the need of Iranian children with CP and there is no similar national or international device available. The final design of the sleeping equipment includes a set of mattresses and a variety of modular styling devices based on a family centered approach, so that, allow parents to active engagement during sleep and rest of their children and can help the children to assume the proper positions.
