Abstract

Introduction:The membranes used in haemodialysis may be manufactured from cellulose, modified cellulose or synthetic polymers. Such membranes, when in contact with blood will activate the complement system, which entails changes in leukocyte and platelet counts. Polysulfone is a synthetic membrane of high biocompatibility standards, whereas haemophane membranes are modified cellulose-based membranes. The biocompatible profiles of these membranes, has been studied by clinical reactions (i.e. hypotension, nausea, pruritis, …) during dialysis sessions. Both kinds of these membranes are used in Iran. The number and severity of these reactions define the degree of dialysis biocompatibility. Materials and Methods: In a clinical trial study which was carried out in Imam Khomeini hospital in Tehran, 100 hemodialysis patients were enrolled to this investigation. Their clinical reactions were compared during 3 sessions of hemodialysis with polysulfone and 3 sessions of hemodialysis with hemophan membrane. Each patient was the control of him/herself. Data was analyzed using Chi square test. Results:Mean age of patients was 48.85±17.56 years and 39% of them were female. The most common complications were hypotension and muscle cramps (each 21.5%). Hypotension was higher in sessions of polysulfon versus hemophane (18% versus 25%) but was not significantly different. Also other complications such as muscle cramp, nausea, vomiting, dyspnea, headache, chills and fever had no significant difference. Seizure did not occur in any patient. Conclusion: According to our findings the membrane’s type has no role in acute clinical complications during hemodialysis and in most patients, membranes can be used according to their availability. It seems their biocompatibility has no considerable clinical difference.

Abstract

Background: Radiotherapy is one of the cancer treatment methods. Prescribed dose for each fraction is considered based on radiosensitivity of tumoral and normal tissues. Viral agents are the effiectiv factors on tissue sensitivity. This research aimed to determine the effect of ionizing radiation of Cobal 60 on radiosensitivity of Hela cells infected with Measles virus. Methods and Materials: In this study, the radiosensitivity of Hela cells is investigated experimentally and qualitively. The cells have been cultivated in two groups (experimental and blank) and plating efficiency has been obtained. Then 100λ measles virus with serial dilution method was used to induce infection in different ratio for experimental group. After cell growth and passage, the two groups were irradiated with 2Gy gamma radiation of cobalt 60. Results: Results respectively indicated cell death increases up to 5-7%, 15-20% and 50-65%, after 2Gy irradiation by Co 60 for contaminating to Measles in low, moderate and high concentrations. Conclusion: Radiosensitivity of tumoral cells increases when they are infected by viral agent. The result in radiotherapy of cancers showed, in prescribing dose fraction non cancer disease should be considered.

Abstract

Abstract Background: Employed women like men, with their income have many physical and psychological problems and their mental health has been threaded. In our country the most rate of employed women are in Health and Education Ministries. So, we decided to determine the mental health of employed women in this both areas. Materials and Methods: This descriptive-analytic study was compared the mental health of 131 employed women in University of Medical Sciences with Education office in Arak city with General Health Questioner (GHQ) in 2008. Also effective factors on mental health based on age, education, income, number of children, mental and physical diseases history, marital status, habitancy condition, job satisfaction and experience were determined and data were analyzed by descriptive and analytical statistics. Results: There was difference between mental health level in women working in Medical Sciences University and Education office of Arak (p=0.041). Also income, age, mental disease history and job satisfaction had direct relationship with mental health Conclusion: Difference between psychological health level of women working in office of Medical Sciences University and Education were significant and income level, age, experience, mental disease history and job satisfaction are the most important factors for mental disorders.

Abstract

Background: In this research dosimetric parameters of mercury was investigated as a radiotherapy modulator and compared with cerrobend as a common beam attenuator material in most radiotherapy centers. Also several designs for mercury shielding system were evaluated.

Materials and Methods: In this experimental research transmission factor of different thicknesses of cerrobend and mercury at 6MV and 18MV photon beams was measured in order to investigate mercury as a beam modulator. Relative dosimetry was done with ionization chambers and then mercury modulating system for radiotherapy was designed with different cross sections.

Results: The result show that the transmission values increased with field size increment due to increased scatter contribution, for all cerrobend and mercury thicknesses at two energies. The procedure of mercury transmission factor is in agreement with cerrobend transmission factor. The second designed system with hexagonal cross section is in good agreement with patient contour.

Conclusion: The experimental investigation on mercury dosimetric parameters showed that mercury can be proposed as a modifier in radiotherapy because of its profit properties such as being liquid in room temperature, power of attenuation and …. So mercury can be proposed as a beam modifier (at closed system because of its poison vapor) in radiotherapy. Keywords: beam modifiers, Cerrobend, dosimetry, Mercury

Abstract

Background: Listeria monocytogenes is one of the most important causes of abortion and postpartum infection in newborns. Because of the importance of L . monocytogenes in the health of pregnant women and newborn babies, the aim of this study was to determine the prevalence of these bacteria in pregnant women and to compare the level of prevalence between women with a history of abortion and with no a history of abortion.

Materials and Methods: In this study, 540 samples of pregnant women were provided from Arak Taleghani hospital. The samples were cultured in enrichment media, then L .monocytogenesis was isolated in specific media.

Results: Of clinical samples, 14 cases had Listeria monocytogenes. Of these samples, 8 cases in women had a history of abortion, while women with no history of abortion were 6 Most cases of positive culture were related to the age of 25 to 34 years, including 7 cases, the lowest cases were 35 to 44 years old including 3 women and 4 women were between 17 and 24 years old.

Conclusion: The study showed that Listeria monocytogenes can cause infection in pregnant women. The use of Phenotypic methods and specific media can apparently isolate listeria monocytogenes from healthy pregnant women.

Abstract

Abstract

Background: The aim of this study was to investigate the effect of aerobic exercise and GnRH agonist on the levels of apelin and insulin resistance index in girls with central precocious puberty.

Materials and Methods: In this study, twenty-five girls (7.44±0.34 years) with precocious puberty were included. They were randomly divided into three groups (drug, n=10, exercise+drug, n=8, and exercise, n=7) and one control group (healthy, n=10). In the experimental group, aerobic exercise for 12 weeks, 3 days/week was done, duration intensity of exercise gradually increased 5 minutes and 5% respectively every 2 week. Also, the GnRH agonists were used as medicine. BMI, the apelin and insulin resistance index were assessed before and after exercise.

Results: Insulin resistance index and apelin serum levels in experimental group (exercise and exercise+ drug) were significantly decreased(p=0.05), while these factors were unchanged in control group (p=0.05). Additionally, BMI in exercie groups was significantly decreased (p=0.000), while it was increased in experimental group (drug group) (p=0.06).

Conclusion: Considering to current data, it could demonstrate that aerobic training and GNRH agonists at the same time can lead to a decrease in BMI and apelin and an improvement in insulin resistance index.

Abstract

Abstract
Background: H2S plays a key role in the pathogenesis of the Alzheimer’s disease. The aim of the present study was to investigate the effects of 4 weeks of the special aerobic training after induction of Alzheimer’s disease by Aβ1-42 injection on CBS and SAM levels in hippocampus of Wistar male rats.
Materials and Methods: Twenty male Wistar rats (8 weeks old and weight 195 ± 20 g) were divided into four groups including: healthy control, Alzheimer’s control, Alzheimer’s + training and sham. To induce Alzheimer’s disease, Aβ1-42 was infused into the hippocampus of rats. Training group trained for 4-week. For data analysis, one-way ANOVA was used and Eta and Omega squared tests were used to determine the effect size (p<0.05).
Results: Findings revealed that 4 weeks of special aerobic training increased significantly the CBS and SAM levels in hippocampus of Alzheimer’s rats compared to the control Alzheimer’s rats ( ES=53; p= 0.007, ES= 92.22; p= 0.001). Also, we showed 4 weeks of special aerobic training increased CBS level in hippocampus of Alzheimer’s rats compared to the healthy cotrol group (ES= 44.07;
p= 0.014).
Conclusion: It seems that the special aerobic training can be used as a useful non-pharmacologically effective therapeutic treatment for Alzheimer's patients through positive regulation of hydrogen sulfide via CBS and SAM enzymes.

Abstract

Abnormal and uncontrolled growth of the cells can lead to cancer. In advanced countries, cancer is the second leading cause of death, and in our country, it is the third cause of death (after cardiovascular diseases and driving accidents). According to a report published by the Institute of Health and Evaluation (2015), for evaluating 32 cancers in 195 countries between 2005 and 2015, the prevalence of cancer has increased by 33% (1).
Radiotherapy is one of the most common cancer treatments that can be used alone or in combination with other therapies such as surgery, chemotherapy or hormone therapy. Approximately 52% of patients with cancer have to be treated by Radiotherapy with a 50% contribution to treatment (2). Radiation therapy uses gamma rays or x-rays or accelerated particles to destroy tumor cells (3). In the past, radiotherapy was done in a two-dimensional fashion, using rectangular fields based on conventional imaging that has now been replaced with 3D conformal radiotherapy. In Three‐dimensional treatment, based on CT or other imaging methods, the treatment volumes such as: GTV (Gross Tumor Volume), target with microscopic spread of tumor that is CTV (Clinical Tumor Volume), ITV (Internal Target Volume), PTV (Planning Target Volume) and also related organs at risk are defined with high accuracy for treatment planning (4). In recent years, with the advancement of computer sciences in treatment planning systems, as well as accelerator equipment for delivering the dose to the patient, treatment can be applied as Intensity Modulated Radiation Therapy (IMRT). In IMRT, each radiation field consists of a beamlets and produces different intensities. This treatment is especially useful for curved areas and when the organs at risk are in the vicinity of the tumor. IMRT can be delivered using linear accelerators with static or Dynamic MLCs, Intensity Modulated Arc Therapy (IMAT), Volumetric Arc Modulated Therapy (VMAT) or tomotherapy (5).
In determination of treatment volumes, the selection of appropriate margin is very important, because small margin may cause loos of the tumor and great margin can damage healthy tissues. The use of IGRT (Image Guide Radiotherapy) reduces these errors and increases the accuracy of treatment. Todays, in developed countries, SRT (Stereotactic Radiation Therapy) is used to destroy the non-surgical tumors, such as some of the brain tumors. In SRS, the prescribed dose is delivered to the tumor up to five sessions. In this method using imobilization devices is important, which usually involve the use of the relevant frames (6). In this regard, Cyber knife is actually a stereotactic system in which the x-ray source is mounted on a robot and can rotates in different angles. This treatment is based on three-dimentional imaging, so the tumor can be identified precisely with the guide of imaging. Cybernayev can be used to treat small tumors with high precision (7).
In addition to treatment with X-rays, ions such as proton can be used to kill cancer cells. One of the important features of treatment with proton is the deliver of the absorbed dose of the particles into the tissue. The absorbed dose curve of this beam in the tissue has a peak at a specified depth, depends on the energy used, called the Bragg peak which can give the highest dose of radiation in the tumor site (8).
There have been many advances in radiotherapy in Iran in recent years, but there is still lack of some advanced treatment equipment. On the other hand, with the regard of the significant cancer rate in the country, it is necessary to have proper information about the incidence of cancer at first. It should be noted that the use of registration systems based on just laboratory information (pathology) leads to a low number of cancer statistic, which this way is recorded in Iran. However, the cancer registry system in developed countries is based on clinical information and mortality in addition to collecting laboratory information.
Another factor causing errors in the cancer record statistics is the population coverage of cancer registries; for example, population coverage in the United States is 99%, Australia and New Zealand is 86%, and the European ::union:: is 57%, while coverage in South and central America is only 21% and in the African and Asian countries is 11 % and 8 %., respectively (1). Therefore, at first, it seems necessary to register the cancer properly in our country and then, based on the needs assessment for the different regions, establish and equipe radiotherapy centers.

Abstract

Background and Aim: Radiation therapy is the destruction of cancer cells that in all patients with breast cancer reduces tumor recurrence, relieves pain in local tumors and metastases. There are different treatment methods around the world such as electron, photon alone or a combination of both types of fields.
Materials and Methods: In this study, photon therapy (PT) and mixed photon-electron therapy (MPET) were used to treat malignancies of the supraclavicular lymph nodes. 30 patients with right-sided breast cancer with local lymph node metastasis were recruited. The ISOgray software was utilized to collect data about treatment planning methods with PT and MPET.
Findings: The maximum and mean delivered doses of radiation to the supraclavicular region were 52.08±1.64, 42.59±0.51 Gy and 54.24±1.64, 43.67±0.43 Gy in the PT and MPET methods, respectively. The mean irradiated volumes of supraclavicular fossa that received 90% of the radiation dose were 59.74±1.94% and 70.26±0.94% in the PT and MPET methods, respectively (p=0.004). The maximum doses delivered to the spine were 14.66±1.9 Gy and 10.22±0.92 Gy and the thyroid were 42.62±3.1 Gy and 37.67±5.02 Gy in the PT and MPET methods, respectively.
Conclusion: The maximum doses delivered to the spine and thyroid significantly diminished by the novel method. Additionally, supraclavicular region received higher maximum and mean doses in the new treatment modality compared to the conventional methods. The new method improved dose coverage for the tumor.

Abstract

Background and Aim: Fennel with phytoestrogenic, antioxidant and anti-diabetic properties is used in traditional Iranian medicine to treat Polycystic Ovarian Syndrome (PCOS). Chronic inflammation is a new proposed mechanism in PCOS pathogenesis that suggests the role of immune system in this disease. Due to the main role of Treg cells in maintaining immune homeostasis, their impaired function, along with increased production of inflammatory compounds such as nitric oxide, can lead to persistent inflammation.
Methods & Materials: In this experimental study, 18 BALB/c mice were randomly divided into three groups including control group, untreated PCOS-induced group, and fennel-treated group. The control group received 0.1 ml sesame oil for 21 days. PCOS was induced by subcutaneous injection of Dehydroepiandrosterone (60 mg/kg/day) plus 0.1 mL sesame oil for 21 days. Mice in third group were treated by intraperitoneal injection of 500 mg/kg/day fennel extract after 21 days. The regulatory of T cells (Tregs) were analyzed by flow cytometry method and nitric oxide level determination was performed by Griess Reaction method. Ferric Reducing/Antioxidant Power (FRAP) assay was used to measure the total antioxidant power.
Ethical Considerations: This study was approved by the Ethical Committee of Arak University of Medical Sciences (Code: IR.ARAKMU.REC.1397.312).
Results: The estrous cycle was normalized after treatment with fennel. Fennel significantly reduced the number of ovarian cysts. Nitric oxide concentration decreased significantly while FRAP concentrations did not significantly alter. The number of Treg cells was also increased significantly.
Conclusion: Due to the role of immune system in the incidence of inflammation, concurrent modulation of both immune and endocrine systems may open new horizons in the design of new therapeutic interventions for PCOS.

Abstract

Background and Aim: Breast cancer is among the most prevalent cancers in women with extensive associated biopsychological problems. The present study aimed to determine the effects of holographic reprocessing therapy on cognitive flexibility and posttraumatic growth in women with breast cancer.
Methods & Materials: This was a quasi-experimental study with pretest-posttest and a control group design. The statistical population included all patients with breast cancer who underwent mastectomy and who were being treated in the chemotherapy ward of Imam Khomeini Hospital in Ardabil City, Iran, in 2020.
By convenience sampling method, 30 women with breast cancer were selected and randomly divided into the experimental and control groups (n=15/group). The experimental group, while receiving pharmacotherapy, collectively underwent holographic reprocessing therapy in 9 one-hour sessions (two sessions per week) for 5 weeks; however, the control group received only pharmacotherapy. Both research groups were assessed in pretest and posttest phases by the Cognitive Flexibility Inventory (Dennis & Vander Wal, 2010) and the Posttraumatic Growth Inventory (Tedeschi & Calhoun, 1996). Finally, the obtained data were analyzed using Multivariate Analysis of Covariance (MANCOVA) in SPSS v. 23.
Ethical Considerations: All study subjects provided a signed written informed consent form. The Research Ethics Committee of Ardabil University of Medical Sciences approved this study (Code: IR.ARUMS.REC.1399.393).
Results: The current study results indicated that holographic reprocessing therapy was effective on cognitive flexibility (F= 42.319, P<0.01) and posttraumatic growth (F= 18.300, P<0.01) in women with breast cancer; this method also increased their cognitive flexibility and posttraumatic growth.
Conclusion: Considering psychological factors and information processing methods in patients with cancer is highly important. Thus, counselors and psychotherapists are advised to use holographic reprocessing therapy as a complementary treatment without adverse effects along with medical treatments to reduce the psychosocial problems caused by the disease.
