Abstract

Introduction: Although more  than  two  decades  is  passed  from  the  discovery  of  H.pylori  and  its  role  in  pathogenesis  of  upper  GI  disease, a  uniform  protocol  for  H.pylori  treatment  is  not  administrated  yet.  This  research  is  conducted  to  evaluate  knowledge,  attitude  and  practice  of  physicians  about  H.pylori  disease  and  treatment  and  its  changes  after  reading  a  review  article.
Materials  and  Methods: This  is  an  interventional  study. First we  prepared  a  compact, review  article  for  the  purpose  of  teaching  H.pylori  pathophysiology, diagnosis  and  treatment.  Then  thirty  questions  was  brought  out  to  evaluate  knowledge, attitude  and  practice  of  the  physicians  about  H.pilory   diagnosis  and  treatment. After  the  first  evaluations  we  gave  our  article  to  the  physicians  to  study.  One  month  later  we  repeated  the  evaluation  with  the  same  questionnaire.  Data  was  analyzed  by  SPSS  software.
Results:  304  physicians  participated  in  this  study.  The  mean  evaluated  knowledge  was  49.64-58.75  with  95%  confidence  interval;  which  after  education  increased  to 57.94-74.14 (p<0.05). Mean  level  of  attitude  and  practice  was  53.01-71.67  with  95%  confidence  interval. After  studying  it  increased  to  61.54-80.83 (p<0.05).
Conclusion: The  first  step  of  the  study  showed  that  test  and  treat  is  the  protocol  of  choice  for  many  physicians.  After primary  evaluation  and  introducing  the  compact  article  there  was  a  noticeable  change.

Abstract

Introduction: Sweet,s syndrome (SS) is characterized by painful plaque on extremities, face and neck fever and peripheral leukocytosis. In this study patients with Sweet,s syndrome are described in a 10 years period.
Cases: This is a case series study in which 9 patients admitted to dermatology unit of Bu-Ali hospital were studied between 1992 to 2002. Basic characteristics of skin lesions in each patient were as following: red, painful and raised lesions on hand and lower leg of the first patient large, flaccid bullae containing hemorrhagic fluid on both lower legs and upper portion of one thigh and scar over both ankles in the second patient papule and nodule on left hand with pain, itching and hemorrhagic secretion in the third patient bullae over internal surface of thighs and trunk and hyperpigmented, diffused macula over abdomen in the forth patient painful papule and nodule on hands and right lower leg in the fifth patient painful plaque and nodule in the sixth patient erythematous and painful papule on both lower leg and abdomen in the seventh patient erythematose papule and nodule with burning sensation over abdomen and right hand in the eight patient and painful bullae on neck and right lower leg in the ninth patient.
Conclusion: Some special findings were found in these patients. For example, hemorrhagic and flaccid bullae, epidermal changes such as regional hyper keratosis and acanthosis and scar formation due to the illness which were not mentioned in previous reports of the disease.

Abstract

Background: Remaining of crude oil in storage tanks usually lead to accumulation of oily sludge at the bottom of the tank which should be treated and disposed of in a suitable manner. In this research the feasibility of chemical oxidation with H2O2 and Fenton&#39;s reagent for removal of total petroleum hydrocarbons (TPH) from composted storage tank sludges was investigated.

Materials and Methods: The sludge was mixed with immature compost at various sludge to compost ratios including 1:2, 1:4, 1:6, 1:8 and 1:10 and composted for 82 days. Then, hydrogen peroxide and Fenton’s reagent were added to the composted mixture in six concentrations including 2%, 5%, 10%, 15%, 20% and 30% w w-1 for a period of 24 and 48 hr.

Results: TPH removal in composting reactors with the ratios of 1:2, 1:4, 1:6, 1:8 and 1:10 were 66.59, 73.19, 74.81, 80.20 and 79.91 percent, respectively. The mean TPH removal of 1:8 composted mixtures with 2%, 5%, 10%, 15%, 20% and 30% of oxidant concentrations were 1, 5.09, 19.37, 28.16, 34.37 and 38.05 percent, respectively. The highest removal efficiency was achieved in stepwise addition to the sludge. As well, increasing oxidation time from 24 to 48 hr had a little effect on TPH removal increase and the removal efficiencies of H2O2 and Fenton were nearly similar.

Conclusion: Chemical oxidation with hydrogen peroxide and Fenton as a post-treatment step is an acceptable process in TPH removal from bottom sludge of storage tanks.

Abstract

Abstract

Background: Osteoporosis is describesd as a disorder and skeletal disease that decrease bone strength and increases the risk of a bone fractures. Genetic factors have effect role in the progression of the osteoporosis. The aim of this study was to investigate the association between LRP4 gene polymorphism with osteoporosis in a population of postmenopausal women from north of Iran.

Materials and Methods: In this case-control study, 80 female patients with osteoporosis and 80 healthy females without osteoporosis with average age of 45-60 has been investigated. After DNA extraction from genome samples, polymorphism of LRP4 (rs4752947) gene have been investigated by PCR-RFLP method. Data were analyzed by SPSS software.

Results: Our results showed no significant relationship between polymorphism of LRP4(rs4752947) gene and the risk of osteoporosis disease in two patients and control groups. Also, AT genotype and TT genotype compared with AA genotype increased the chance of disease by 1379 and 3.5, respectively. In addition, TT alleles compared with AA alleles, increased the chance of osteoporosis up to 1.605 times.

Conclusion: Of course, more complementary studies considering other LRP4 gene subtypes with more individuals for better findings are needed.

Abstract

Abstract

Background: The problem of learning disabilities is the reason of academic backwardness of students and dyslexia is considered the most common of these disorders.Therefore, the present study aimed to investigate the comparison of the effectiveness of mental rotation and phonological awareness training on reading performance of  students with dyslexia.

Materials and Methods: The design of the study was quasi-experimental in pre-test and post- test with control group. Statistical population composed of all dyslexic students in the city of Tabriz in 2015-2016. The sample of present research consisted of 45 students with dyslexia who were selected via available sampling and then were assigned randomly to experimental) phonological awareness and mental rotation training) and control groups(n=15 in each). To collect data, revised Wechsler intelligence scale for children and reading improvement and dyslexia test were used. Multivariate Covariance (MANCOVA) was used to analyze the data.

Results: Findings indicated that scores of mental rotation and phonological awareness training have a significant effect on reading performance of dyslexic students compared with control group (p<0.001). Furthermore, there is no difference between mental rotation and phonological awareness training effectiveness on reading performance of dyslexic students (p>0.05).

Conclusion: It can be concluded that mental rotation and phonological awareness training are effective on accuracy, speed and comprehension of reading in students with dyslexia.

Abstract

Abstract
Background: Antibiotic resistance rates in E. coli are rapidly rising, especially with regard to fluoroquinolones. One of the mechanisms that lead to antibiotic resistance is efflux pumps. The aim of this study was phonotypic and genotypic analysis of efflux pump role in fluoroquinolones resistance of E. coli strains isolated from hospitalized patients in Kermanshah 2013.
Materials and Methods: In this cross-sectional study, 100 isolates of E. coli were collected from hospitalized patients from Kermanshah. All isolates were identified by standard biochemical tests. The antimicrobial susceptibility patterns were determined by disk diffusion method according to CLSI guidelines. The presence of Efflux pump genes was determined by a PCR method.
Results: The rates of resistance to Ceftazidime, Nalidixic Acid, Ciprofloxacin, Norfloxacin, Ofloxacin, Gentamicin, and Tetracycline were 73%, 67%, 55%, 54%, 45%, 38%, and 24%, respectively. According to the results of PCR test, of 100 E. coli isolates, 99% of isolates were positive for acrA, 98% for acrB, 95% for acrE, 98% for acrF, 94% for mdfA, 96% for norE, and 96% for tolC.
Conclusion: In Strains with positive gene acrA, acrB, acrA, acrB, tolC, mdfA, norE, the presence of efflux pump inhibitor reduced the amount of resistance to antibiotics. So, efflux pumps are important in antibiotic resistance.

Abstract

Abstract
Background: High morbidity and limited therapies of hepatic fibro genesis are important factor for better understanding the molecular mechanisms of the disease. Advances in the understanding of the molecular behavior of hepatic stellate cells (HSC) allow the progress of a field dedicated to anti-fibrotic therapy. Melanoma differentiation associated gene-7 (IL-24/mda-7) as a gene induced during terminal differentiation in human melanoma cells, but the inflammatory response of cells to IL-24/mda-7 is not entirely cleared.
Materias and Methods: LX-2 cells (a human hepatic stellate cell) were treated by leptin (positive control), media (control negative), or were transfected by empty plasmid and pcDNA3.1/mda-7. The inflammatory state was evaluated through measuring the mRNA expression level of inflammatory molecule, IL-1β. The role of IL-24/mda-7 modulation on inflammatory response was assayed using SOCS1 and SOCS3 gene expressions.
Results: The expression levels of IL-1β, SOCS1 and SOCS3 were compared in LX-2 cell line groups. The expression of the IL-1β in the transfected cells was higher than the control cell, but it was not significant. The results indicated that the expressions of SOCS1 and SOCS3 were up-regulated following pcDNA 3.1/mda-7 transfection into LX-2 cells compared to control plasmids (p=0.0179, p=0.0428).
Conclusion: The endogenous IL-24/mda-7 exhibited a significant modulatory effect on stellate cells. Therefore, IL-24/mda-7 and relevant signaling pathways could be employed as a target for fibrosis treatment.

Abstract

Background and Aim: The purpose of this research is to provide nanofibers from cellulose with Wheat bran as an agricultural residue and its antimicrobial activity by dipping ciprofloxacin hydrochloride on Staphylococcus aureus.
Materials and Methods: In this experimental study, cellulose disks were prepared from nano-impregnated fiber. Subsequently, these disks were placed on Staphylococcus aureus with standard paper disks and the results were measured as a non-growth zone after 24 hours. The amount of ciprofloxacin hydrochloride adsorbed in cellulose by comparing the effect of cellulosic discs containing different concentrations of antibiotics and standard ciprofloxacin disks was determined. Subsequently, the evaluation of the time of recovery of the ulcer in the skin of the rats was carried out with the drug-mediated formulation and without drug.
Findings: The disk (nano alpha-cellulosic) dipped by ciprofloxacin hydrochloride created a non-growth zone in Staphylococcus aureus. Measuring the size of the wound was done by digital imaging and the ImageJ software. The results of the recovery process were analyzed by ANOVA and pathological tests in five days.
Conclusion: The results showed that the nano-fibers disk could be useful in controlling bacteria in the culture medium, and the area of the wounds in rats (rats) dressed with nano-fibers impregnated with the drug was significantly less than the control group (p <0.05).

Abstract

Background and Aim: Low Back Pain is one of the most common complains of Human societies. LDH (Lumber Disk Herniation) is one of the relatively common causes of Low Back Pain. This study aimed to assay serum zinc cofactor in patients compared to healthy group.
Materials and Methods: Sixty patients which were diagnosed as a "known case of LDH" by the specialist and based on tests were chosen by simple selection method (30 men and 30 women). Also, 57 people were chosen as a control group (29 men and 28 women). measurement was done based on single reagent direct calorimetric method using the kit made by "Ziestchem diagnostics company, Iran".
Findings: Total, 117 objects were observed in case and control groups. Serum zinc level was measured in all of them. Obtained results were analyzed by Student t-test, and ꭕ2, by using SPSS software with the confidence interval of 95 percent. Results showed that 21.7% of patients with LDH were having high physical activity and 36.7% having low physical activity.
Conclusion: There was a significant difference between the mean serum zinc levels in patients with LDH compared to control group. Decrement in serum zinc may be considered as an distruption factor in the activity of enzymes involved in proteins biosynthesis.

Abstract

Background and Aim The flavin reductase DszD enzyme is a key enzyme for providing required reduction potential in the bacterial desulfurization process. Considering the low speed of desulfurization process because of low catalytic power of this enzyme, it is necessary to increase the catalytic power of flavin reductase for industrial use of this enzyme as biocatalyst.
Methods & Materials The three-dimensional structure of the flavin reductase DszD enzyme was predicted by a CPHmodel server and its amino acid sequence was searched in the protein data bank to identify the homologue molecules. Based on the alignment of the amino acid sequence and the model molecules, the key residues at the flavin mononucleotide substrate were identified. The key residue of asparagine at position 77 was replaced with phenylalanine using the site-directed mutagenesis method. 
Ethical Considerations This study with research ethics code IR.NIGEB.EC.1398.6.24 A has been approved by research ethics committee at National Institute of Genetic Engineering and Biotechnology, Tehran, Iran.
Results The cloning and expression of each of the wild-type and mutant genes were performed separately. The catalytic power of the produced wild-type and mutant enzymes were compared. The catalytic activity measurements showed that the mutant enzyme had a 2.5 fold increase in catalytic power.
Conclusion Replacing phenylalanine with asparagine at position 77 of flavin reductase DszD enzyme leads to an increase in enzyme catalytic power to increase the speed of bacterial desulfurization process.

Abstract

Background and Aim: High occupational stress reduces the workers’ concentrations and decision-making power. In industries with high risks in term of exposure to hazards, there may be negative consequence. This study aimed to investigate relationship between Occupational Stress (OS) and the job-related risks in the steel industry. 
Methods & Materials: This is a descriptive-analytical study with cross-sectional design carried out in a steel industry located in Lorestan province of Iran in 2017. For risk and OS assessments, the Melbourne University’s 3D risk assessment model and the United Kingdom's Health and Safety Executive (HSE) questionnaire were used. Statistical analysis was performed by using Pearson correlation and Kruskal-Wallis tests in SPSS V. 22.
Ethical Considerations: This study was approved by Ethics Committee of Arak University of Medical Sciences with the Code: IR.NREC.007.1394.05.
Results: A total of 182 job-related risks were identified for 14 job titles. 19.3% of subjects had high OS level. The risk rank had a statistically significant correlation with overall OS score (P=0.03), and OS dimensions of control (P=0.02) and demand  (P=0.02). There was no significant difference between the mean OS score and age, work experience, educational level.
Conclusion: Due to the relationship of risk amount and OS among workers in the steel industry, Planning to control stressors in the workplace and managing occupational risks by reducing the likelihood and consequence of job-related risks seems essential.

Abstract

Background and Aim: Staphylococcus aureus is common pathogens of nosocomial infections. Nasal swabs in hospital staff is main sources of hospital infections are considered. The aim of this study was to determine the frequency of nasopharyngeal carriers of methicillin-resistant Staphylococcus aureus and microbial contamination of health care workers' cell phones in Emergency department of Vali-e-Asr Hospital in Arak City.
Methods & Materials: In this descriptive study, nose swabs and cell phone levels were taken from 70 health care workers in the emergency ward of Vali-e-Asr Hospital. The Staphylococcus aureus clinical isolates were identified using standard microbiological methods (catalase, coagulase, mannitol fermentation and DNase). The susceptibility to oxacillin and cefoxitin was detected by the disk diffusion and the mecA genes in this clinically isolated strain of staphylococci was investigated by Polymerase Chain Reaction (PCR).
Ethical Considerations: This study was approved by the Research Ethics committee of Arak University of Medical Sciences. (Code : IR.ARAKMU.REC.1396.282).
Results: According the results, Staphylococcus aureus was isolated in 16 cases, which 5 cases were methicillin-sensitive S. aureus (MSSA), and 11 cases were Methicillin resistant S. aureus (MRSA). Also, 3 cell phones were infected with Staphylococcus aureus, which 1 case was MRSA and 2 cases were MSSA.
Conclusion: The results of this study showed that frequency of MSSA strains is significant in emergency personnel of the hospital. Thus, regarding to the risk of epidemics due to nosocomial infections, periodic testing for the identification and treatment of carriers among employees for controlling and preventing hospital infections seems necessary.

Abstract

Background and Aim: The research results show tobacco use increases cardiovascular and lung disorders or the probability of periodontitis in humans, while no definitive relations were yet confirmed. This study evaluates the relationship between periodontitis and tobacco use by panoramic radiography in Bandar Abbas City.
Methods & Materials: A total of 198 panoramic radiographs were classified into periapical healthy (n=99) and patient (n=99) groups. Radiologists assessed radiographs for periodontitis via Total Teeth Numbers (TTN), the quality and length of Root Filling Treatment (RFT), the number (NoRFT), and inadequate root filling treatment (RFTin), age, gender, the cigarette and hookah use, and dental debris. Data were analyzed by Chi-Square, t-test, and logistic regression.
Ethical Considerations: Due to the lack of therapeutic intervention and the preservation of patients’ characteristics, ethical considerations are not included in this study (Code: IR.HUMS.REC.1399.384)
Results: Tobacco, hookah use, TTN, and RFTin were different (P<0.01) in groups, but cigarette smoking, debris, RFT, NoRFT, and age were not. Women in the periodontitis group showed high (P<0.01) in tobacco, hookah use, and RFTin, while no gender differences were observed in other parameters. The logistic regression results showed that based on the Odd Ratio NoRFT (39.7), RFTin (36.8), `hookah (8.1), tobacco (6.57), debris (3.62), smoking (3.59), RFT (1.4) and TTN (0.89) were the highest (P<0.01). At the same time, age and gender did not affect periapical tissues. 
Conclusion: Hokka, tobacco, and cigarette use negatively affected the periapical tissues, respectively. The decrease in TTN and increase in debris and RFT were probably the effects of tobacco smoking on the periapical tissues. Age and gender were not related to the periapical lesions.
