Abstract

Introduction: Saffron (crocus sativus L.), an important spice rich in carotenoids, is used in folk medicine to treat numerous diseases. This study examines the effects of saffron consumption on innate and humoral immunity of healthy male students.
Materials and Methods: 41 healthy volunteered university students in two control and case groups were included in this study and consumed respectively milk and milk containing saffron for 6 weeks. Blood samples were collected from the subjects at weeks 0, 3 and 6. Innate immunity was evaluated by determining the percentage and counts of monocytes, neutrophils, eosinophils, basophils, platelets and the total white cells, CRP concentration and C3 and C4 concentrations and humoral immunity was assessed by deterning the percentage of lymphocytes and the concentrations of IgG, IgM and IgA.
Results: Three weeks after saffron consumption, monocytes percentage and IgG concentration increased significantly (P<0.05, P=0.01). Total white blood cells and platelets count also reduced significantly (P<0.05). After 6 weeks, monocytes percentage and IgG concentration reduced significantly (P<0.05, P< 0.001) and platelets count increased significantly (P<0.05) as compared with week 3.
Conclusion: This investigation demonstrated that saffron consumption enhances innate and humoral immunity systems and has anti-inflammatory effects. These effects depending on the duration of saffron consumption.

Abstract

Background: Hydatid cyst, a parasite disease, can involve liver more than other organs. The most serious complication of the cyst is rupture which may spread out to biliary duct, hollow organ, or abdominal cavity directly. Elective treatment is emergent surgery. Case: A 28 rural woman referring with abdominal pain, nausea and vomiting due to car accident. Abnormal findings consisted of: abdominal distention, reduction of bowel sounds, tenderness and generalized guarding and hive lesions in the anterior chest wall, upper abdomen, and both forearms. In sonography, a lot of free liquid in abdominal and pelvic cavity, and three hetroechoic masses in the right lobe of the liver were reported. Four days after abdominal surgery the patient was discharged with Albendazole order. Conclusion: Intra-abdominal rupture of hydatid cyst is the most serious that due to trauma complication and elective treatment is emergency surgery.

Abstract

Background: The existence of estrogen and progesterone receptors, p53, human epidermal receptor-2(HER-2) and cathepsin-D are among the prognostic markers for breast cancer. In this study, we investigated the relationship between these factors and lymph-node involvement. Materials and Methods: In this case-control analytic study, 105 patients with breast cancer were investigated. After detecting breast mass, surgical biopsy was done and the status of the estrogen and progesterone receptors, p53, HER-2, and cathepsin-D were studied. Collected data were registered in a checklist and were subjected to analysis. Results: There was no relationship between lymph-node involvement and estrogen and progesterone receptors, p53, cathepsin-D and HER-2. Conclusion: In order to get more precise results about hormonal receptors, p53, HER-2 and cathespin-D, a similar research with a larger sample size over a longer period of time is needed.

Abstract

Background: Colorectal cancer is one of the most important and most common fatal types of cancer in the world. Identifying new agents which individually or in combination with other agents induce apoptosis in tumor cells is surely of great significance in treatment of colon cancer. The aim of the present study was to assess the effectiveness of applying lovastatin and α-tocopherol individually or in combination with each other in the induction of apoptosis in human colorectal cancer cells. Materials and Methods: In this trial, HT29 cells were exposed to various concenterations of lovastatin (5, 10, and 20 μmol) and/or alpha tocopherol (10, 20, and 25, and 30 μmol). After cell count, these cells were examined through trypan blue method and DNA fragmentation technique. Results: The findings of DNA fragmentation technique showed that each of the two drugs could induce apoptosis at all of the given concentrations. In the combination of 10 μmol concentration of lovastatin and 5 and 10 μmol concentrations of α-tocopherol, induction of apoptosis was not observed. Conclusion: Based on the extensive effects of statins, the concentration of lovastatin is seen as determining in its apoptosis function, and its combination with tocopheroles in high concentrations, by inducing apoptosis, can provide novel effective strategies for prevention of human colorectal cancer.

Abstract

Background: Motivation is one of the most important tools to push faculty members to perform effectively and efficiently in the workplace is. This study is based on Herzberg's theory of motivating factors to consider requiring faculty at the University of Medical Sciences deals.

Materials and Methods: In this cross-sectional study, a total of 168 faculty members of Tehran University of Medical Sciences, 102 persons conducting a completely free and informed, a 40-question questionnaire based on Herzberg's two-factor theory, design, a questionnaire. Data using t-test and Mann-Whitney U and indicators of central tendency and dispersion were analyzed by SPSS 16 software.

Results: Response rate to the questionnaires, 7.60, respectively. 5.24% of the faculty in the basic sciences and 5.74% at the clinical level of activity. Of "communication" and "progress and development", the motivation faculty have the greatest impact. Factors "physical condition" and "salary" have been less effective. Between the ages of faculty motivating factors, "nature", "responsibility and career" and "supervision and monitoring", there was a significant correlation. The relationship between work experience and of "salary" and "job security" was also significant.

Conclusion: Communication is one of the most important factors in increasing the motivation of faculty members in the physical conditions of work and wages are minimal impact in this area. With an aging faculty, salary, supervision and monitoring, and job security are more important motivation. In other words, with increasing age and work experience, academic expectations of the management of change and the proper evaluation of the activities of the officials do not have the decision-making, and because the quality of activities, good participate Ndah encouraged do not motivate members to create good will.

Abstract

Background: Post ischemic myocardial reperfusion can impair CABG outcomes and cause increased mortality, hospital stay and costs. Then, increased oxygen free radicals lead to lipid peroxidation and sulfhydryl group oxidation. The aim of this study is to evaluate whether N-acetylcysteine and Vitamin C as antioxidants could effect postoperative outcomes of on-pump CABG.

Materials and Methods: In this clinical trial, patients who had undergone CABG were studied in 4 groups receiving Vitamin C, N-acetylcysteine, Vitamin C & N-acetylcysteine and traditional drugs. The study population consisted of 50 patients in each group. Data were recorded in a checklist and analyzed by using SPSS 20.

Results: Mean pump time, ICU stay, hospital stay, the distribution of troponin positive serum, arrhythmia and need for reoperation were significantly different between groups. Gender distribution, mean cross-clamp time, serum creatinine level before and after surgery, the need for inotropic drugs and required dose level of it, showed no significant differences between groups.

Conclusion: It seems that the combination of N-acetylcysteine and  Vitamin C had a stronger effect on the defference between the mean of pump time, hospital length stay and  length of ICU stay versus the administration of N-acetylcysteine or Vitamin C alone.

Abstract

Background: Determining the level of manual ability of children with cerebral palsy has significant role in scheduling care and providing supportive services by organizations such as the a social welfare office. Manual Ability Classification System (MACS) is responsible for this critical matter. In Mazandaran Province, the prediction of manual ability is intuitive and is done without tools. This study aimed to investigate the consistency between operational therapists prediction of future manual function of children with cerebral palsy and MACS scale.

Materials and Methods: This study was designed as a cross sectional trial. The study population consisted of 12 occupational therapists working in Mazandaran’s rehabilitation center under the social welfare office. Firstly, occupational therapists chose 100 children with cerebral palsy and classified their future manual ability into five levels according to their prediction. Then the researcher classified the children based on MACS scale. Finally, the amount of prediction consistency between therapists and researcher was statisticaly analyzed.

Results: The weighted kappa coefficients of MACS scale were 0.671 in first level and 0.747 in fifth level that show good agreement in these two levels. This coefficient was 0.417 in third level and 0.444 in forth level that shows fair agreement. The weighted kappa coefficient was 0.358 in second level that indicates slight agreement.

Conclusion: With prediction consistency between operational therapists and researcher, MACS is used as a suitable means for classifying the level of manual ability in children with cerebral palsy and predicting their needs to adaptive and auxiliary devices by occupational therapists in rehabilitation centers and social welfare offices.

Abstract

Abstract

Background: Evaluation of performance is one of the hardest aspects of management responsibilities. Today, performance evaluation process is a characteristic of work world that can be useful in making workforce more effective. This study aimed to assess the job performance and its determinants in healthcare workers.

Materials and Methods: This cross sectional and descriptive –analytical study conducted in capitation way on all workers in healthcare centers and health houses of Islamabad-e Gharb city in 2016. Data collection tools were questionnaires of demographics and job performance of Hersey and Goldsmith. Descriptive statistics, one way ANOVA and Independent sample T-test used in order to data analysis.

Results: The mean and standard deviation of job performance score was 52.5± 9. There was a significant relationship between job performance with sex and education level (p<0.05). Also, there were significant relationships between feedback and environment with sex, and support aspect with type of employment (p<0.05).

Conclusion: Success and progress of each organization is based on high levels of job performance in its employees. Solutions such increasing salary and providing good physical environment, can be useful to improve job performance of employees.

Abstract

Background and Aim: Sicne in many dosimetry calculations, the water and soft tissue phantoms are used, this study aimed to investigate the difference of these two phantoms with a phantom consisted of realistic liver materials in proton therapy for liver cancer.
Methods & Materials: Three phantoms with different materials of water, soft tissue and realistic liver materials were used for the study. A spherical tumor with 2 cm radius was considered in the liver. The Spread-out Bragg Peaks (SOBPs) were measured to cover the complete tumor for the three phantoms. Dose distribution and deposited dose ratio in tumor and surrounding organs were calculated using Monte Carlo N-Particle Extended (MCNPX) code. 
Ethical Considerations: The best proton energy interval to complete the coverage of tumor in the liver for phantoms with realistic and soft tissue materials was 90-120 MeV and for water phantom, it was 88-116 MeV. The shift of the Bragg peaks depth per energy in the water phantom mm relative to two other phantoms was about 4.5. The dose parameters were evaluated according to the International Commission on Radiation Units and Measurements (ICRU), and the results showed no any significant difference between them. The dose distribution in the tumor and surrounding organs showed that for all three phantoms, the dose distribution around the tumor was negligible.
Results: The use of soft tissue phantom has more acceptable results than water phantom in simulating treatment and can be replaced with realistic liver tissue. More realistic phantoms should be used in treatment plan.

Abstract

Introduction: Increasing skeletal muscle contraction ability via increasing its neural stimulations, is one of the methods can be effective in sport performance improvement. The aim of the present study was to evaluate the effect of supramaximal isometric conditioning contractions on muscle neural excitation and performance indices during bench press exercise.
Methods: Current research is a semi experimental study performed on 8 athlete men. Age range of them was 19 – 23. Doing resistance trainings was a part of their exercise program at least 2 months before study onset. Bench press with Barbell was used in the protocol of these study. Electromyography was used in order to assay the neural excitation of main muscles which are activated during bench press. In addition, 1-RM test was done so as to assess the performance of these muscles. The protocol of this study was approved by the ethics committee of Arak University of Medical Sciences (Code: IR.ARAKMU.REC.1400.357). All subjects participated in this study voluntarily and they had no illness or injury at the start of the research.
Results: Neural excitation and performance of the muscles increased significantly during strength bench press, following conditioning contractions. This research was reviewed in Islamic Azad University - Arak Unit and approved with the ethics code IR.IAU.ARAK.REC.1401.096. Informed consent was obtained from the participants and they were assured that their information would be confidential
Conclusions: Doing supramaximal isometric conditioning contractions prior to doing strength bench press with barbell, increased neural excitation and performance indices of main muscles activated in this exercise.
