Abstract
Nowadays the securing of children health and then the maintenance of community health is training programs about health, but before every health programs. Knowing the believes and tendency of people is necessary. The knowledge and attitude of mother regarding to supplementary food before and after the training were measured based on KAP study .( knowledge, Attitude , Practice ). The result of this study is as follows:
1-3.58% of studied mothers had good knowledge, 41.2% moderate knowledge, 55.38% weak knowledge before training and raised to 80.51% good knowledge, 19.48% moderate knowledge following training.
2- 95.56% of studied mothers had positive attitude and 4.46% negative attitude before training that raised to 100% positive attitude to supplementary food following training.
3- Statistically, there is no meaningful relationship between the knowledge of mothers regarding to supplementary food and number of child, job, but there is a relationship with the level of education.(0.025)

Abstract
As might be expected iron deficiency leads to anemia, and it is found around the world. Due to its need during the pregnancy, WHO paid attention to "childbearing health" in 1998. Absorbance of iron depends on gastric secretion and HCL for efficient absorption. In present research three groups of rats, which contain 15ones, with the same diet, sex, age and condition were selected. Diet of groups A and B contained 5g/kg and 10g/kg sodium bicarbonate respectively and the diet of group C as a control group was free of sodium bicarbonate. After 3 months, we used ether as the anesthesia and removed blood directly from the heart, serum were separated and Iron, TIBC measured. Also PH of gastric secretions was estimated, the mean of PH in group C(3), group a (3.8) and group B(3.8) was observed. Finally we found out that the levels of Iron and TIBC in group C were 72.5mg/dl and 35mg/dl, in group A 69.1mg/dl and 480.2mg/dl and in group B 52.8 and 309.9mg/dl respectively. On the basis of our results, we suggest that the using of sodium bicarbonate along diet effects on the absorption of Iron- which is due to the changes of gastric acidity comparing to control group.

Abstract
Introduction: Glycosylated hemoglobin gives an average plasma glucose level over the past two to three months period. Diabetes is one of the most important causes of mortality and morbidity in the old ages, that usually due to its slow progress, patients do not pay attention to the disease. Early diagnosis can decrease many complications. This investigation is carried out with the aim of comparison results of FBs, GTT and HbA1c especially in persons with impaired dfasting glucose.
Materials and Methods: Our study is a cross sectional analytical study. 81 persons, 40 to 65 years old, without any history of diabetes, with FBs 103 to 140 mg/dl were selected according to convenience sampling method. FBS, GTT and HbA1c tests were done for all samples. Sensitivity, specially, positive predict value and negative predict value of tests were determined.
Results: In this study comparison of HbA1c with FBS showed 100% sensitivity and 12.5% specificity with positive predict value of 82% and negative predict value of 62.7%. In comparison of HbA1c with GTT, we observed 100% sensitivity and 63% specificity with positive oredict value of 6.3% and negative predict value of 100%. In other part of this investigation in comparison of FBS with GTT, 9.7% sensitivity and 100% specificity and positive predict value of 100% and negative predict value of 18.8% was reported.
Conclusion: According to results of the present study, due to very high sensitivity and significant positive predict value of HbA1c as compared to FBS, HbA1c measurement seems to be a good test in screening and diagnosis of diabetes.
Therefore, in impaired fasting glucose, we recommend measurement of both HbA1c and FBS. Also HbA1c can be used as a good replacement test for GTT in person with impaired fasting glucose.

Abstract
Introduction: Recent studies have demonstrated that hypothermia after hemorrhagic shock (HS) can decrease need of fluid therapy and increase arterial pressure that results in increase the survival rate. But it has not been determined how hypothermia causes these beneficial effects. This study reviews hypothermia effects in this area.
Materials and Methods: This is an experiential study in which 24 rats were divided into four groups. Rats were anesthetized by pentobarbital. In groups 1and3, bilateral renal sympathectomy was done and in the groups 2 and 4 renal nerves remained intact and only lateral incisions were done to preserve similar conditions. Then rats underwent HS phase (phase I) for 90 minutes, by withdrawing 4-6 ml of blood during 10 mines. After shock induction, only in groups 1 and 2, body temperature was lowered to 32oC. At the end of phase I, rats underwent resuscitation phase (phase II) by reinfusion of blood & additional Ringer Lactate to retain normal arterial pressure. Stage II lasted 60 minutes.
Results: In hypothermic groups (1 and 2) in comparison to normothermic groups (3 and 4) there was less blood or fluids needed for resuscitation (p<0.05). Arterial pressure before shock was the same in all groups but at the end of phaseII was higher in hypothermic ones (p<0.05).
Conclusion: It seems that mild hypothermia has beneficial effects on survival of animals with HS. Although it is obvious that intact kidneys have vital role in maintaining body fluid balance during HS, but it seems hypothermic rats requires lesser fluid therapy even without nerve of kidney. More studies could be probably required to confirm this.

Abstract
Introduction: Diabetes is the most important metabolic disease in human. The prevalence of both types of diabetes is rapidly increasing ocross the world. Diabetes causes many complications including End Stage Renal Disease (ESRD). Diabetes is responsible for 30% of ESRD. The prevalence of diabetic nephropathy in Iran is also high. Many of these patients are becoming dialysis dependent. Many studies have shown the changes of trace metals’ levels in diabetic patients including Copper, Zinc, Manganese and Chromium. This study evaluates the correlation between urinary Copper and diabetic nephropathy Materials and Methods: This is a case-control study. Samples were selected among type 2 diabetic patients attending to diabetes clinic in Vali-e-Asr hospital in Arak. Diabetic patients were divided in two groups based on microalbuminuria, 42 patients in case and 40 patients in control group. Then the patients were classified based on duration of diabetes into 4 groups and based on the HbA1c into two groups. Then urinary Copper was determined with atomic absorption spectophotometry and compared. Independent t-test was used to analyze data. Results: Patients were 28.1% male and 69.9% female in case group and 37.5% male and 62.5% female in control group. The mean Copper level was 36.14µg /L (14.54-57.74) in case group and 14.77% µg /L (10.17-19.37) in control group. There was a statistically significant difference between the two groups (p=0.003). Conclusion: The results show a positive relation between urinary Copper and diabetic nephropathy and confirmed the results of other studies that reported the elevation of Copper in microalbuminuria. This study also showed that age, gender, duration of diabetes and HbA1c level have no effect on urinary Copper.

Abstract
Abstract Background: Different studies have shown the inability of traditional lecture method to active teaching method in concept transmitting of higher learning domain. The aim of this study was comparing the effects of feedback lecture method and conventional lecture method on learning and quality of teaching. Materials and Methods: In quasi-experimental study, 27 Arak medical science midwifery students who were in fourth semester, randomly, were divided to two groups of “teaching by feedback lecture” and “teaching by conventional lecture”. Desired topic were presented by one teacher as a 90- minute lecture in control group and as two 35-minute lectures with 10-minute discussion after each part in case group. Informational form, and post test and student' opinion form were used respectively to identify the demographic characteristics, short tem and long term learning and quality of teaching form. Results: Demographic characteristics, quality of teaching form student’s opinion, total scores of pre and post test were similar in two groups. Results of teaching score in post test in knowledge and perception level were similar. But there was a significant difference between application and analysis level in two groups(p=0.011). Also, total and domain scores of post test after a month were not different in both groups. Conclusion: Results indicated that learning by feedback lecture can be more effective than conventional lecture method in application and analysis domain.

Abstract
Abstract Background: What makes a physician to a proficient one is accompanying science and moral together, especially observing religious features of the patient. Therefore, present study was conducted to evaluate the knowledge of Arak University of Medical Sciences students in medical religious in pre and post training, 2005-06. Materials and Methods: In a descriptive cross-sectional study, questionnaire was prepared including 50 questions on medical religious commandments. After filling questionnaires by 227 students, were confidentially collected in a box. In next stage, the book on medical commandments according to four religious authorities was freely presented to the participants to be read within two weeks then 50 questions about medical commandments were prepared again. Results: In the present study, 81.9% were female and 18.1% were male. The basic sciences students were lower acquaintance before reading the book and midwifery students had better acquaintance (43%). The changing percentage of knowledge in all groups had significant difference with medical commandments in second period as compared to the first period, (p‹0.001). Conclusion: The knowledge of students in different courses in medical religious commandments was not significant. But, after training, there have been considerable changes in their medical commandments knowledge. Therefore, it is recommended to train medical commandments in parallel with training scientific and practical courses.

Abstract
Background: Brucellosis is one of the zoonotic diseases that has always been considered a health problem in Iran. Since this disease is endemic in Arak and Markazi Province, this study was done to investigate its epidemiology during 2001-2010.
Materials and Methods: In this epidemiological study, data on patients with brucellosis were obtained from the medical records in Arak Health Center during 2001-2010. The collected data were analyzed by SPSS software version 16.
Results: During this period, 3880 new cases were reported. The mean incidence rate of brucellosis was 60 per 100000 people during 2001-2010. The greatest incidence was in 2004 (111.5 per 100000 people), whereas the lowest incidence was in 2006 (40.5 per 100000 people). Overall, 72% of the patients lived in villages, and 60% of them were male. The majority of the patients were in the second decade of their lives (10-19 years old). The majority of the patients had Wright test titer=1:320 and 2ME titer=1:80 in serology titration.
Conclusion: Arak is one of the regions with high incidence of brucellosis. In this region, the patients are mostly male, live in villages, and age 10-19.

Abstract
Background: The aim of this study is to assess antioxidative and pro-oxidative efficacy of ascorbate on serum albumin during iron-induced oxidative stress. Materials and Methods: In this experimental study, albumin was placed in the oxidative system containing iron ions and different concentrations of ascorbate. To monitor albumin degradation, sodium dodecyl sulfate polyacrylamide gel electrophoresis was performed according to Laemmli procedure. Oxidative modification of albumin was demonstrated using a method for determination of carbonyl groups by 2,4-dinitrophenylhydrazine. Results: By applying the carbonyl assay, ascorbate showed a dual effect: initial pro-oxidative effect on albumin changed to an antioxidant one in a dose-dependent manner. Our findings showed prooxidant effects for ascorbate in low concentrations (0-100 µM) and antioxidant effects in higher concentrations (100-300 µM). Also, electrophoretic pattern of plasma proteins showed significant protein aggregations in the range of 35 to 45 kDa of MW and protein degradations in the range of 115 to 180 kDa. Conclusion: Ascorbate can produce reactive oxygen species and can also inhibit the production of these oxidants in the presence of iron ions as well. These findings may be directly applicable to oxidative states during the administration of ascorbate and may be important in preventing oxidative modifications of proteins in blood circulation and other biological fluids.

Abstract
Background: Dysmenorrhea is a common problem in women of reproductive age and despite the current treatments, it still remains a common problem in women’s health. This study was done to determine the effect of aromatherapy on pain severity in primary dysmenorrhea. Materials and Methods: This randomized clinical trial was done on 108 students with primary dysmenorrhea with pain score of 2 and 3 based on the multidimensional spoken criteria. After random allocation, the participants received massage with almond oil, massage alone, and massage with a mixture of lavender and peppermint essential oil a week before menstrual period until the presence of pain in two continuous cycles. During cycles zero, one, and two, the participants recorded the highest level of pain during the first, second, and third days according to the visual analogue scale. Data were analyzed using SPSS software. Results: There was a significant difference in the total of the most pain in days 1, 2, and 3 among the groups(p=0.038). A reduction was observed in massage and aromatherapy groups(p=0.014). There were significant differences in within group comparisons (p=0.002) for zero and first cycles(p=0.016) and first and second cycles(p=0.002). Conclusion: Aromatherapy reduces menstrual pain and considering the prevalence of dysmenorrheal, the low price, and harmless nature of this essence, its application in relieving menstrual cramping and pain is recommended.

Abstract
Background: Measurement of mean arterial blood pressure is better predictive factor for pregnancy-induced hypertension than measurement of the systolic and diastolic blood pressure or systolic blood pressure. According to the theory of “protective effect of fish oil supplementation on pregnancy-induced hypertension “and the importance of blood pressure control in pregnancy, the present study was conducted to investigate the effect of fish oil supplementation in pregnancy on mean arterial blood pressure. Materials and Methods: In this clinical trial study, 120 healthy pregnant women were randomly divided into two groups of 60 persons. Same prenatal care was performed for both groups and the intervention group was received Salmon fish oil capsule 1000 mg/day, from 16th week to the end of the pregnancy. Systolic and diastolic blood pressure were measured in both groups once every four weeks up to 28th week, every 2 weeks from 28th till 36th week and once every week up to the end of pregnancy. Mean arterial blood pressure was calculated and all gathering data was analyzed with T test and Mann-Whitney test. Results: Mean arterial blood pressure in the intervention and control group was 80.76±5.50 and 78.46±4.50 mmHg which was not significantly different (p>0.05). Conclusion: The results showed that adding fish oil supplements to the diet of pregnant mothers had no significant effect on mean arterial blood pressure.

Abstract
Background: Funnel plot is a graphical tool for investigating publication bias in meta-analysis studies. The aim of this study is to introduce another application of funnel plot that is monitoring one of the Iranian family performance indices and determining the position of each health center of Markazi province in terms of patient referral rates by general practitioners to specialists and to compare it with bar and caterpillar plots.
Materials and Methods: In order to draw the funnel plot, the average monthly percent of referrals for each district of Markazi province in 2011 was computed. In this study, Iranian Ministry of Health and Medical Education standard figure of 10% was considered as the limit of referral to the second level of Iranian health services. The 99% confidence interval of the control limit for each district was computed according to 3 standard deviations of the percent of referrals relative to the total patient visits.
Results: Based on funnel plot results, five out of ten districts were in the expected range of referral (Ashtian, Saveh, Tafresh, Khomein, and Komijan). However, in other five districts, the referral rate was greater than the expected rate (Arak, Shazand, Zarandiyeh, Delijan, and Mahallat). Mahallat district had the highest rate of referral.
Conclusion: The funnel plot was more informative than the other two plots Thus, it can be viewed as a useful tool in monitoring health programs throughout the country.

Abstract
Background: Depression is one of the most common problems in postmenopausal women. Diosgenin present in fenugreek seeds present study the effects of menopause on depression in menopausal women took the city in 1390.
Materials and Methods: In a randomized controlled trial enrolled 60 women with complaints of depression caused by menopause. Beck Depression Inventory on a sample of postmenopausal women and the inclusion criteria were enrolled. The samples were divided randomly into two groups, Diosgenin (6 g Fenugreek seed daily containing 25 mg Diosgenin) and sorbitol (25 mg daily) for 8 weeks were placed before and after 8 weeks of intervention questionnaire, Beck completed SPSS software was used to test the statistical Paired test scores before and after treatment were compared.
Results: Mean depression score decreased after 8 weeks took Fenugreek containing Diosgenin and sorbitol as compared with before of intervention of this study and it was significant.The comparison did not indicate a significant difference between the two groups.
Conclusion: According to the finding of this study, administration of 6 g Fenugreek seed containing Diosgenin for 8 weeks decreased menopausal depression . The need for further studies with longer duration and higher doses before any recommendations can be felt.

Abstract
Background: There is abundant evidence indicating that inflammatory mechanisms within the central nervous system contribute to cognitive impairment via cytokine-mediated interactions between neurons and glial cells. BAT1, a member of the DEAD-box family of RNA helicases, appears to regulate the production of inflammatory cytokines associated with AD pathology. In the current study BAT1 -22 promoter polymorphism was analyzed in AD and control subjects.
Materials and Methods: In this case-control study, genomic DNA from peripheral blood samples of 153 Alzheimer’s patients and 153 healthy controls was extracted using salting-out method. DNA analysis was performed by PCR-RFLP method and p<0.05 was considered statistically significant.
Results: After genotyping and statistical analysis the results failed to show any association between BAT1 -22 promoter polymorphism and sporadic Alzheimer’s disease.
Conclusion: BAT1 -22 is not associated with Alzheimer’s disease in Iranian population and so has no effect on predisposition to sporadic Alzheimer’s disease.

Abstract
Background: Hematopoietic stem cell transplants are routinely used to treat patients with cancers and other disorders of blood and immune systems. Osteoblasts constitute part of the stromal cell support system in marrow for hematopoiesis by participating in the formation of the HSC niche. It is believed that interaction between hematopoietic cells and bone forming osteoblasts regulate each other’s function. It is established that acute blood loss in animal models activates bone formation and niche development because of EPO stimulation. In this experimental study we have examined the co-culture of HSCs derived from cord blood which treated with EPO in vitro, on osteoblastic differentiation of mesenchymal stem cells.
Materials and Methods: In this experimental study MSCs isolated from bone marrow and co-cultured with CD 34+ CD38- HSCs isolated from cord blood. These co-cultured cells were treated with different doses of erythropoietin for 14 days, after that RNA were extracted from MSCs and analysed with RT-PCR to evaluate the expression of osteopontin and osteocalcin. Alizarin red and alkaline phosphatase staining were done for osteoblastic differentiation.
Results: Osteopontin and osteocalcin were expressed in MSCs. Cellular staining were positive for osteoblastic differentiation. Differentiated cells expressed osteoblastic markers.
Conclusion: These data suggest that EPO regulates the osteoblastic differentiation from bone marrow MSCs in vitro.

Abstract
Background: Quality of life in Tracheostomy patients who are discharged from hospital and take care of your home will be affected by the current situation. Therefore, proper training can improve their quality of life also will increase the satisfaction of the training provided. Based on this, the researcher was to investigate the effects of video tape on quality of life and satisfaction in patient with tracheostomy.
Materials and Methods: This research is a Quasi-experimental study in Imam Khomeini and Amir alam Hospital of Tehran University of Medical Sciences in1392. In this study 80 patient were randomly assigned to intervention and control groups. In the discharge questionnaire on demographic characteristics, quality of life (SF-36) and satisfaction (VAS) were completed. The intervention group received education film for home use addition to the routine teaching of hospital. Two months later the patient's quality of life and patient satisfaction were assessed again with the above instruments. The data were analyzed with using statistical SPSS (V.20).
Results: Statistical analysis showed the difference was significant in both groups after the intervention (p=0.003). The quality of life in the intervention group at two months after the discharge in role limitation due to emotional problems, emotional status, social functioning, energy, public health, and the pain had increased. Average satisfaction with the training of the two groups was statistically significant difference (p<0.001).
Conclusion: The results showed that in addition to routine education to provide educational CDs for home use at discharge can improve the quality of life as well as increased satisfaction with training provided to patient’s tracheostomy.

Abstract
Background: Head circumference (and its related morphologic factors such as head length and width) is one of the morphologic indices that people with autism spectrum disorders (ASD) have different developmental patterns in comparison to their peers. So the aim of this study is to assess head circumference indices in relation to cognitive factors in ASD children and adolescents.
Materials and Methods: In this cross-sectional study, the case group was consisted of 129 students with ASD aged 7-18 years old. The control group was consisted of 103 students with intellectual disability (ID) aged 7-18 years old. Head circumference, head length and width was measured by meter and caliper respectively. In addition, socio- cognitive factors questionnaire was responded by parents.
Results: Results showed that the head circumference, length and width of ASD group was significantly greater than ID group (p<0.001). Moreover, ASD group achieved lower score on SSIS questionnaire than ID group (p<0.001). Furthermore, results showed that in ASD group with increasing the size of head circumference the score on socio-cognitive abilites and skills decreased (p<0.05).
Conclusion: Generally, this study indicated that the developmental trajectory as well as the relation of anthropmetric indices of head circumference and socio-cognitive factors in children and adolescents with ASDs is different from ID population.

Abstract
Background: Diabetes is one of the most common disease resulted from metabolism disorders. Thus, Studying medication adherence in patients to explain educational interventions to improve the health of these patients is necessary. Therefore, the present study aimed to assess the factors affecting medication adherence in diabetics patients based on health belief model in Arak 2014.
Materials and Methods: In this analytical study, 366 diabetic patients in Arak in 2014 were selected and data were collected through a researcher made questionnaire about patients behavior in the field of medication adherence and its influencing factors based on the health belief model. Linear regression model was used to determine the predictive power of structures.
Results: The mean knowledge score and the medication adherence was 31.34 and 84.34, respectively. In this study, perceived susceptibility, perceived barriers and internal action guide were as the strongest predictors of medication adherence behavior in patients and predicted their behavior variance 0.36. Between function and susceptibility and severity and perceived benefits and self- efficacy of manual internal and external action guides in the field of medication adherence with diabetes, there was a direct correlation that the highest correlation was related to the perceived susceptibility structure (r=0.42 , p<0.001).
Conclusion: The results showed that the educational interventions with an emphasis on building the perceived sensitivity of patients with diabetes for medication adherence and emphasis on internal incentives as motivators and incentives for patients to achieve better results in the field of medication adherence should be at the heart of educational interventions.

Abstract
Abstract
Background: Physical exercise is an important and effective part of comprehensive care of seniors, which declines aging progression. Because of the importance of physical activity in cardiovascular diseases prevention this study intends to investigate the comparision of structural and functional characterictics of the heart between middle- aged montaineer men and non-athlete peers.
Materials and Methods: In this cross-sectional and descriptive–analytical study, 13 middle- aged montaineer (age: 54.5±2.0 y, body mass index: 25.59±2.4 kg/m2) who have continues mountain activities during previous 24 months for at least 2 sessions per week, each session lasted 120 minute, and 14 sedentary, healthy peers (age: 54.1±2.2 y, body mass index: 26.8±2.3 kg/m2) who were not currently experiencing any regular physical activity (at least 6 months), were selected. All subjects underwent standard two-dimensional and Doppler echocardiography at rest. Cardio respiratory fitness was assessed using Bruce test. T test was used to compare groups with α=0.05.
Results: The results showed that mountain activities significantly increased left ventricular mass (p=0.03) and left-ventricular-end-diastolic-diameter (p=0.04). We also observed that systolic blood pressure (p=0.04), ejection fraction (p=0.05), stroke volume (p=0.03) and cardio respiratory fitness (p=0.03) were significantly improved by mountain climbing. In some of parameters such as shortening fraction, interventicular septum and left ventricular posterior wall there were no significant differences between groups (p>0.05).
Conclusion: These results suggest that regular mountain sports activities can have beneficial effects on structural and functional characterictics of the heart in middle-aged men.

Abstract
Background and Aim: Low Back Pain is one of the most common complains of Human societies. LDH (Lumber Disk Herniation) is one of the relatively common causes of Low Back Pain. This study aimed to assay serum zinc cofactor in patients compared to healthy group.
Materials and Methods: Sixty patients which were diagnosed as a "known case of LDH" by the specialist and based on tests were chosen by simple selection method (30 men and 30 women). Also, 57 people were chosen as a control group (29 men and 28 women). measurement was done based on single reagent direct calorimetric method using the kit made by "Ziestchem diagnostics company, Iran".
Findings: Total, 117 objects were observed in case and control groups. Serum zinc level was measured in all of them. Obtained results were analyzed by Student t-test, and ꭕ2, by using SPSS software with the confidence interval of 95 percent. Results showed that 21.7% of patients with LDH were having high physical activity and 36.7% having low physical activity.
Conclusion: There was a significant difference between the mean serum zinc levels in patients with LDH compared to control group. Decrement in serum zinc may be considered as an distruption factor in the activity of enzymes involved in proteins biosynthesis.
