Abstract
Serum concentration of lipoprotein(a) is a genetically determined. Independent risk factor for coronary artery disease. Different studies suggest a possible role for a antioxidant and free radicals in the rate of lipoprotein (a) synthesis. Patients suffering from chronic renal failure (CRF) are known to be less guarded against oxidative products in comparison with normal population. In order to investigate the pattern of lipoprotein (a) serum concentration in CRF group, 87 number of patients undergoing supportive hemodialysis was selected and their Lp(a) serum concentration were compared with a control group (n=100). The result shows a significant elevation of LP(a) serum concentration in the patients in comparison with the control group (p,0.05).

Abstract
Introduction: Sex ratio is referred to male, female ration which is usually 1.06. This ratio is decreased during the last few years. One of its reasons seems to be poisons and environmental pollutions such as cigarette smoke. In this investigation we studied the relationship between fathers cigarette smoking and sex ratio in children.
Materials and Methods: This is a historical cohort study. Study population were neonates born in Arak hospitals during summer and autumn 1383. Sample size was determined 710 neonates, based on α=5%, β=10%. Sampling method was census in which all newborns with inclusion criteria were selected and questionnaires were filled. Samples were divided into three groups. 1-newborns with nonsmoker fathers 2- newborns with smoker fathers (less than 20 cigarette a day) 3- newborns with smoker fathers (20 or more cigarettes a day) (fathers cigarette smoking at least 3 months before conception in second and third group). Newborns sex ratio was determined in each group and caparisoned. Data analysis was done by frequency, ratio, mean and standard deviation indexes and k-s, leven, t, mann-whitney, chi square tests and relative risk, using SPSS software. At all stages study Helsinki declaration was regarded.
Results: Overall sex ratio was 1.04. This ratio was 1.272 for neonates with nonsmoker fathers and 0.77 for neonates with smoker fathers. Also sex ratio in newborns with smoker fathers using less than 20 cigarettes a day was 0.846 and for those with fathers using 20 or more was 0.60. Based on chi square there was a meaningful relationship between the three groups (p<0.00001). Relative risk in low cigarette group was 1.23 and in high cigarette group 1.42.
Conclusion: Based on findings it seems that fathers cigarette smoking near conception results in decreasing sex ratio and the increased use of cigarettes the decreased born of male neonates.

Abstract
Introduction: Studies have shown that there is a relationship between metabolic syndrome and coronary heart disease. Hypertension is a common risk factor of both cardiovascular diseases and metabolic syndrome. The aim of this study is to determine the prevalence of metabolic syndrome phenotypes in patients with hypertension in Isfahan and Markazi provinces in center areas of Iran. Materials and Methods: This cross-sectional study was implemented in Isfahan and Markazi provinces in year 2001. Sampling method was random-clustering. 12514 people from both sexes who were above 19 years old were considered which based on ATP III, 1936 person of them had metabolic syndrome. Based on the definition, these people were those who at least had three of the following factors abnormally: HTN, FBS, TG, HDL and WC. Also hypertension was considered for all groups. Data was analyzed using Chi square test and Odds ratio. Results: The prevalence of metabolic syndrome was 0.6% to 7.5% in central areas of Iran. The most prevalent of metabolic syndrome was in hypertensive women with high TG and WC and the least prevalent was in hypertensive men with these two phenotypes: 1-high FBS and WC, and 2-high FBS and low HDL (p<0.05). Overall prevalence of metabolic syndrome in hypertensive people in the most of urban population was more than rural population. Conclusion: Results showed that hypertension is associated with different metabolic syndromes, especially in women with central obesity and dyslipidemia which necessitates more attention in preventing and treatment of the disease in this high risk group.

Abstract
Introduction: Clubfoot is one of the most common foot deformities in children. The initial treatment of clubfoot is nonsurgical. The succes rate of this treatment is 5-80%. If clubfoot does not respond to this treatment, surgery is indicated. Surgical treatment of clubfoot has several complications. The preferred method of surgical release in textbooks is a modified Mc Kay procedure with Cincinnati incision but it may have various complications such necrosis, excessive scar, infection, dehiscence and recurrence. The purpose of this article is to investigate surgical treatment of clubfoot with two incision procedure instead of Cincinnati incision. Materials and Methods: In this descriptive study 50 children (81 feet) with clubfoot disease who did not respond to nonsurgical treatment, since 1999 until 2002, in Al-Zahra hospital were selected and underwent soft tissue release procedure by posteromedial and lateral incisions. After operation they were followed for at least two years. Every six months patients were assessed for early postoperative complications, clinical and radiologic recurrence. Data was presented using descriptive statistics. Results: 74% of patients were male. Their mean age was 3.40.69. There was positive family history in 14% of patients. Clinical recurrence was seen in 6 patients, (9 feet) (5 male, one female) that all had bilateral involvement. The most common type of recurrence was metatarsus adductus. There was no skin necrosis or deep wound infection after surgery but 2 cases of superficial infection was seen which were treated by drugs. Conclusion: Regarding the excellent results of this procedure and obtained correction and the least skin complications in comparison to textbooks we suggest the two incision procedure instead of Cincinnati incision in treatment of most club foots

Abstract
Background: Angiogenesis is a complex process that occurs in many physiologic and pathologic conditions such as invasion and metastasis of tumors. Therefore, it is the target of many clinical treatments. Rapamycin is one of the immune system inhibitor drugs that recently has been used for controlling different types of cancer. In this study, the effect of Rapamycin on angiogenesis in chicks' chorioalantoic membrane was investigated. Methods and Materials: In this experimental study, we used 42 Ross fertilized eggs that were divided into 3 random groups: control, sham-exposed (treated by Dimethyle sulfoxide-DMSO- ) and treated with Rapamycin. In 2th day, a window was opened on eggs in the sterile condition. Later, in 8th day, a gelatin sponge appeared on chorioalantoic membrane and was soaked with 5 μl Rapamycin in treatment group and 5 μl DMSO in the sham-exposed group. In 12th day, CAMs were examined and photographed by Research Photostereomicroscope in all cases. The numbers and lengths of vessels around the sponges were measured and compared with each other by T-Test (p<0.05). Results: The mean of number (42 ±7.26 ) and length (57.25±5.05 cm ) for vessels in the control group and mean of number (42.93±8.37 ) and length (55.66±10.44 cm) in sham-exposed group was'nt any significant differences. There was a significant decrease in mean number (29.36±5.28) and length (44.55±10.22) of vessels in Rapamycin with control group. Conclusion: It seems Rapamycin has an inhibitory effect on angiogenesis in chicks' chorioalantoic membrane. It decreases the number and length of vessels around treated area

Abstract
As the world is moving at the fastest possible speed toward industrialization, great technical developments, and becoming a place devoid of difficulties and troubles, the issue of illnesses and mental problems still persists as an unpleasant matter which is quite noticeable. This study was done through reviewing the available Quranic books and resources. In such matters, Quran and Hadith are rich in both content and foundation and play a vital role in decreasing and alleviating mental pressures and enhancing and establishing mental health in the society. In this regard, Islam (Quran and Hadith) enjoys and offers recommendations and solutions for fortifying the individual’s mental and spiritual basis in the society in a way that it eliminates the peoples’ economic and mental pressures and strengthens or establishes their economic mental hygiene. In this regard, the solutions and recommendations of Quran and Hadith can be applied to the treatment of the majority of economic mental diseases and their consequences.

Abstract
Background: Retinoic acid (RA) is a vitamin A derivative and one of the most important inducing signals in vertebrates that is involved in differentiation, morphogenesis, apoptosis, and reproduction. This study was done to evaluate the role of RA in in vitro neural patterning of mouse embryonic stem cells (mESCs).
Materials and Methods: In this experimental study, upon formation, embryoid bodies (EBs) from mESCs, Royan B1, were induced by 1 µM RA for four days and then plated for eight days. Untreated EBs were considered as the control group. Finally, in both groups, neural induction and patterning of EB-derived neural cells were evaluated by using immunostaining, flowcytometry, and RT-PCR methods.
Results: RA induced neurogenesis in ES cells, from which 35% showed to express MAP2. RT-PCR analysis also indicated that RA-treated neural cells derived from ES cells could at the same time express Mash1, Pax6/7, and Dbx1/2 as dorso-ventral (DV) pattering markers and Hoxb4, Hoxc5, and Hoxc8 as the rostro-caudal (RC) axis markers.
Conclusion: RA induces in vitro neural induction along with neural patterning of ES-derived neural cells in DV and RC axes. Keywords: Mouse embryonic stem cells, neural patterning, retinoic acid

Abstract
Background: Comprehending of suicidal ideation and related factors has valuable clinical outcomes in domains of prevention and crisis intervention. The aim of the present study was to explore the predictive role of psychological, demographical and socio-economic factors in suicidal ideation.
Material and Methods: In a cross-sectional descriptive study, 671 persons among general population of Semnan City were selected by voluntary sampling. Data were collected with NEO-Five Factor Personality Inventory, Symptom Check-List Revised, The Defense Style Questionnaire, The Beck Scale for suicide Ideation, The Beck Hopelessness Scale, The Social Support Questionnaire and demographical questionnaire, then analyzed by correlation and Logistic Regression with SPSS.
Results: Literacy level (OR=1.237 P<0.014), Family relationships (OR=1.873 P<0.010), Outcome (OR=4.211 P<0.003), Job status (OR=3.149 P<0.008), History of psychiatric disorders (OR=5.754 P<0.003), Hospitalization in psychiatric ward (OR=2.003 P<0.011), Neuroticism (OR=6.136 P<0.001), Extraversion (OR=3.612 P<0.005), Neurotic defense style (OR=2.468 P<0.005), Mature Defense Style (OR=4.107 P<0.002), Immature Defense Style (OR=3.059 P<0.003), Social Support (OR=7.609 P<0.001), and Hopelessness (OR=9.834 P<0.001) significantly differentiated persons with suicidal ideation from persons without suicidal ideation (P<0.001). These variables totally could correctly predict 97.4% of suicidal ideation.
Conclusion: In this model, risky psychological, demographical and socio-economic factors increase the odds ratio of suicidal ideation. These findings have importance for designing of preventive interventions and interventional programs for self-harm behaviors.

Abstract
Background: Wharton's Jelly Mesenchymal Stem Cells (WJMSc) are potential renewable source of cells in replacement therapies of many diseases. Different biomaterials have been used as a scaffold to mimic the stem cell niche, which is important for promoting cellular interactions, cell proliferation and differentiation. Encapsulation involves entrapment of living cells within the semi-permeable membrane for the exchange of nutrients, oxygen and stimuli, whereas antibodies and host immune cells are kept out. In this study, a new approach for culturing and differentiating Wharton's Jelly Mesenchymal Stem Cells to definitive endoderm in a three dimensional environment using alginate capsules is presented.
Materials and Methods: In this experimental study, Wharton's Jelly Mesenchymal Stem Cells were capsulated and Trypan blue exclusion method was applied to determine cells viability. Then, encapsulated cells have been cultured in medium contain differentiating factors and to investigate the expression of definitive endoderm related genes, Real- time PCR was performed.
Results: The encapsulation procedure did not alter the morphology and viability of the encapsulated cells. Post-differentiation analysis confirmed the expression of FOXa2 and SOX17 as definitive endoderm specific markers.
Conclusion: Alginate has potential to be used as a three dimensional scaffold for culturing and differentiation of WJMSCs to definitive endoderm.

Abstract
Background and Aim: Globozoospermia is a severe sperm morphological abnormality in men that characterized by round-headed spermatozoa with low or absence acrosome structure in their sperm samples. In these men, high level of DNA damage and abnormal chromatin packaging also were reported. These deficiencies can consider as the main etiologies of infertility in these infertile men. The aim of this article is to study the sperm chromatin structure in infertile men with globozoospermia.
Materials and Methods: In this systematic review article, 77 articles related to protamine deficiency, DNA damage, aneuploidy in globozoospermic men were collected via data bases such as PubMed, Google Scholar, Scopus since 1971-2017.
Ethical Considerations: This study with research ethics code IR.ACECR.ROYAN.REC.1396.204 have been approved at research ethics committee of Royan Institute.
Findings: Mean percentage of sperm DNA fragmentation and protamine deficiency were significantly higher in infertile men with globozoospermia compared to fertile men. While, the results of chromosome aneuploidy were controversial in infertile men with globozoospermia within studies.
Conclusion: In addition to abnormal acrosome formation, as main etiology of failed fertilization, in infertile men with globozoospermia, high level of sperm abnormal chromatin packaging and DNA damage can be also involved in this phenomenon. Therefore, antioxidant therapy before intra-cytoplasmic sperm injection technique were suggested for these individuals to minimize sperm chromatin damage.

Abstract
Background and Aim The flavin reductase DszD enzyme is a key enzyme for providing required reduction potential in the bacterial desulfurization process. Considering the low speed of desulfurization process because of low catalytic power of this enzyme, it is necessary to increase the catalytic power of flavin reductase for industrial use of this enzyme as biocatalyst.
Methods & Materials The three-dimensional structure of the flavin reductase DszD enzyme was predicted by a CPHmodel server and its amino acid sequence was searched in the protein data bank to identify the homologue molecules. Based on the alignment of the amino acid sequence and the model molecules, the key residues at the flavin mononucleotide substrate were identified. The key residue of asparagine at position 77 was replaced with phenylalanine using the site-directed mutagenesis method.
Ethical Considerations This study with research ethics code IR.NIGEB.EC.1398.6.24 A has been approved by research ethics committee at National Institute of Genetic Engineering and Biotechnology, Tehran, Iran.
Results The cloning and expression of each of the wild-type and mutant genes were performed separately. The catalytic power of the produced wild-type and mutant enzymes were compared. The catalytic activity measurements showed that the mutant enzyme had a 2.5 fold increase in catalytic power.
Conclusion Replacing phenylalanine with asparagine at position 77 of flavin reductase DszD enzyme leads to an increase in enzyme catalytic power to increase the speed of bacterial desulfurization process.

Abstract
Background and Aim: Breast cancer is the most common cancer among women which leaves a profound impact on their psychosocial health and pain experience. The aim of this study was to determine the effect of group psychotherapy based on Acceptance and Commitment Therapy (ACT) on pain-related anxiety and cognitive emotion regulation of women with breast cancer.
Methods & Materials: In this quasi-experimental study, subjects were 68 women with breast cancer refereed to Shahid Ghazi Hospitalin Tabriz, Iran who were selected using a convenience sampling method. Then, they were randomly assigned into two groups of intervention (n = 34) and control (n = 34). The intervention group underwent eight 90-min sessions of ACT-based group therapy, while the control group received no any intervention. They completed Pain Anxiety Symptom Scale Short Form and Cognitive Emotion Regulation Questionnaire before and one week after treatment. Collected ata were analyzed by using t test, chi-square test, ANCOA and MANCOVA.
Ethical Considerations This study has obtained its ethical approval from the Research Ethics Committee of Tabriz University of Medical Sciences (code: IR.TBZMED.REC.1397.287) and has been registered by Iranian Registry of Clinical Trials (code: IRCT2017100615590N5).
Results: The ACT-based intervention improved the dimensions of pain-related anxiety (8.44±1.5) and cognitive regulation of negative (8.40±3.5) and positive (8.39±7.5) affects (P<0.001).
Conclusion: ACT-based intervention can help breast cancer patients to accept their negative thoughts and current conditions.
