Abstract

Introduction: Acute  renal  failure (ARF)  is  one  of  the  relatively  incident  complications  in  surgical  emergency  wards  and  is  defined  as  increasing  serum urea  and  creatinine   and  decreasing  GRF. In  this  study  we  investigated  acute  renal  failure  incidence  in  patients  admitted  to  emergency  surgical  ward.
Materials  and  Methods: This  is  a  cross  sectional  analytical  study  in  which  all patients  with  primary  diagnosis  of  emergency  surgery  (traumatic  or  non traumatic) admitted  in  emergency  surgery  ward  were  selected. After  history  taking  and  physical  assessment  patients  having  history  of  kidney  disease  were  excluded. Urinalysis, BUN  and  creatinine  tests  were  performed  for  all  samples. Data  was  analyzed  by  SPSS  software.
Results: In  this  study  2100  patients  (1280  male)  were  assessed  during  a  10  month  period. 5.66%  of  samples  had  some  degree  of  ARF. 1136  patients  undergone surgery  due  to  accidents  and  trauma  (72%  multiple  trauma  and  28%  single  trauma) and  964  patients  due  to  nontraumatic  emergencies.  ARF  was  seen  in 6.84%  of  the  first  and  4.25%  of  the  second  group. ARF incidence  in  male  and  female  was  5.6%  and  5.7%  respectively.
 Conclusion: The overall  incidence  of  ARF  in  this  study  was  not  so  different  from  developed  countries.   ARF  was  significantly  more  incident  in  traumatic (especially  multiple  trauma)  and  old  aged  patients.  This  factors  must  be  considered  as  ARF  risk  factors  in  emergency  wards.

Abstract

Introduction: Benign Prostatic Hyperplasia (BPH) is the most common benign tumor in men. Due to the side effects of chemical drugs, phytotherapy has become a treatment method in BPH since 1990s. Prostatan is a plant extract widely available in Iran. In this study, we evaluated the therapeutic effects of Prostatan drop in combination with Prazosin in alleviating BPH symptoms. Materials and Methods: In a single blind randomized clinical trial , 66 men over 50 years with BPH symptoms who were visited in Vali-e-Asr hospital of Arak were randomly divided in two groups and treated with either Prostatan (40 drop,TDS) plus Prazosin (1mg, BD)or Prazosin (1mg, BD)alone . AUA symptom score, PSA (prostate-specific antigen) levels, Prostate volume, post voidal residue (PVR) by sonograms and uroflowmetry results were determined before and 12 weeks after treatment. Data was analyzed by T and Man- Whithey u tests. Results: Before treatment there were no significant differences in age, PSA, PVR, AUA symptom score, Prostate volume and uroflowmetry between the two groups. After treatment the mean of PSA levels, Prostate volume and PVR did not differ between the two groups. In the Prostatan plus Prazosin group, the mean of AUA symptom score was 3.3 less (p<0.0001) and the mean of maximal urine flow rate was 2.7 ml/s more than the Prazosin group (p<0.0001). Conclusion: Regarding results, it seems that adding Prostatan to Prazosin against Prazosin alone enhances its effects on controlling BPH symptoms and increases urine flow rates.

Abstract

Abstract Background: Prostate cancer is the most common cancer in American's men and is the most common malignancy after skin in men. The purpose of this research was comparison of demographic characteristic of patients with advanced prostate cancer treated with surgical and medical orchiectomy. Materials and Methods: This retrospective cross- sectional analytic study carried out on 53 patients with locally advanced and metastatic (stages III- IV) prostate cancer referring to oncology wards in Buali hospital in Hamadanand and Vali-e-Asr hospital in Arak for adjuvant therapy of chemotherapy or radiotherapy during 2003-7. Demographic characteristics such as age, disease stage, cultural status, habitancy, socioeconomic status and patients accepted treatment of advanced prostate cancer treated with surgical and medical orchiectomy (hormone therapy), were determined and compared. Results: There was a significant difference between age mean of patients treating by orchiectomy and hormone (p=0.007). Also socioeconomic status distribution in two groups was different (p= 0.025). Distribution of disease stage, cultural level, habitancy in two groups were similar (respectively p=0.207, p=0.617, p=0.272). Conclusion: People less tend to orchiectomy to hormone therapy. Also, in high socioeconomic status and in elder, tendency to surgical orchiectomy is more.

Abstract

Abstract Background: Approximately 15-20% of pregnancies terminate to abortion. More than 50% of spontaneous abortions in 8-15 weeks pregnant mothers are related to genetically abnormalities. So, approximately 95% of them are related to numerical and 5% structural chromosome aberrations. Until now, neither of known human chromosomal abnormalities are treatable, and only way against these diseases limit to prenatal diagnosis and abortion of affected fetuses. The purpose of this study was determined the chromosome aberrations with cytogenetically methods. Materials and Methods: In this cross-sectional analytic study, 56 aborted fetuses were studied to detection of abnormalities frequency. Amnion-chorine culture, placenta, fetal tissue and aborted products from metaphase cells for gathering metaphase cells were used. Results: After cell culture, chromosome typing and GTG-banding Technique, trisomy 21 with highest frequency (12.5%) and isochromosomy 21, X and monosomy X with lowest frequency (1.8% for each them) was resulted. Conclusion: Not only chromosomal aberrations have important role in recurrent miscarriages, but also frequency rates of chromosomal aberrations in our country are similarity with other countries. Also cytogenetically diagnostic methods such as GTG-banding are a powerful and reliable technique for investigation of parents with recurrent abortion.

Abstract

Background: APACHE IV scoring system is one of the most useful scoring systems for evaluating the quality of health care services in ICU. The aim of this study was to evaluate the quality of health care services in ICU wards of Valiasr and Amiralmomenin Hospitals of Arak. Materials and Methods: This cross-sectional study was conducted on 100 patients admitted to ICU wards in Valiasr and Amiralmomenin Hospitals of Arak. First, the predicted mortality and length of stay (LOS) in ICU were calculated by APACHE IV scoring system. Then the real mortality and the real length of stay in ICU were recorded. Data were analyzed by SPSS software version 11.5 and t-test. Results: Real mortality in men and women were 57.2% and 61.9%, respectively. The real length of stay in men and women were 11.58 and 11.62 days, respectively. Predicted mortality calculated by APACHE IV in men was 23.72% while it was 24.1% in women. Predicted LOS by APACHE IV in men was 15.02 days while it was 16.64 days in women. There was a significant difference between real mortality and predicted mortality (P = 0.001). Conclusion: The findings of this study indicate that the quality of health care services in ICU wards of Valiasr and Amiralmomenin Hospitals of Arak are far from international standards.

Abstract

Background: Morphine is one of the derivations of opium alkaloids. Contradictory reports exist on hyperglycemic and hypoglycemic effects of morphine. The aim of this study was to evaluate the role of opioid receptors involved in blood glucose changes in morphine-treated Balb/c mice. Materials and Methods: This experimental study was carried out on 8 groups of male Balb/c mice (n=6), including group1(morphine), group 2 (naloxone (morphine antagonist) + morphine), group 3 (naltrindole ( receptor antagonist) + morphine), group 4 (norbinaltorphimine ( receptor antagonist) + morphine), group 5 (CTOP ( receptor antagonist) + morphine), group 6 (saline), group 7 (saline + saline), and group 8 (saline + morphine). Blood samples were obtained from retro-orbital sinus at 0, 1, 2, and 3 hours after injection. Blood glucose level was measured by enzymatic technique. Data were analyzed by SPSS software. Results: The application of morphine resulted in significant hypoglycemia in comparison with the control group which was significantly compensated by naloxone compared to the morphine group. The application of naltrindole could significantly inhibit hypoglycemia induced by morphine compared to the control group, whereas norbinaltorphimine and CTOP failed to do so. Conclusion: Since naltrindole could compensate for hypoglycemia due to morphine, hypoglycemia caused by morphine is likely to be mediated by opioid receptors

Abstract

Background: Transitional cell carcinoma is the most common bladder tumor that can be treated by surgery, BCG instillation, chemotherapy, and radiotherapy. This study deals with urethral squamous cell carcinoma (SCC) in a patient with history of transurethral resection (TUR-T) of bladder transitional cell carcinoma treated by BCG instillation. Case: A 68-year-old man was admitted with gross hematuria, lateral prostatic lobe kissing, and a pedunculated 2×3mm mass in bladder in cystoscopy. He had undergone TUR-T and BCG instillation and had been followed regularly every 3 months with cystoscopy. In the follow up, 9 months later, several pedunculated tumors in penile urethra that were of SCC type were indicated in pathological analysis. Conclusion:: Since membranous urethra is the most common site and TCC is the most common type of urethral cancers in this site, SCC in penile urethra was unexpected. This could be because of chronic inflammation due to treatment and cystoscopic follow ups.

Abstract

Background: Prostate cancer is one of the most prevalent type of cancer and the second cause of death among American men. Its prevalence has a direct relationship with increasing age. The present study was carried out to determine the level of prostate specific antigen (PSA) and calcium level in blood in men with prostate cancer and BPH in Arak.

Materials and Methods: In this prospective study, out of 400 Arak males, were selected from different inner-city areas through cluster sampling during a period of two years (august 2011-august 2012). After the population was approved to have the necessary criteria to be included in the study, their PSA and calcium level was measured and its mean in over 40 years. In order to determine mean difference among the seven groups T-test analysis and to determine "PSA", PSA Assay and Tandem-R kits were utilized and for measurement of serum calcium, pars azmoon kit was used.

Results: In this study, there was a markedly relationship between age and serum calcium and serum PSA in both groups.in this way that there was a negative relationship between age and serum calcium and a positive relationship between age and serum PSA.

Conclusion: With respect to the findings of the study, it could be confirmed that was a markedly relationship between serum PSA and serum calcium only in prostatic cancer patients,according to this study in future maybe we used serum calcium as a tumor-marker for screening for prostate cancer.

Abstract

Introduction: Paints are composed of extensive variety of hazardous substances, such as organic solvents and heavy metals. Occupational exposure to paint increases oxidative stress, hematological disorders, and DNA injury. This study was performed to explore the beneficial effects of Selvit on the paint workers by measuring total antioxidant capacity, lipid peroxidation, and blood parameters as main oxidative stress biomarkers.

Materials and Methods: This clinical trial study was conducted on 25 male workers who worked in the paint factory. Serum markers such as total antioxidant capacity (TAC), lipid peroxidation (LPO) and also hematological parameters were measured before and after using Selvit (14 days). SPSS software was used for data analyses and paired t-test and Pearson correlation analysis were applied.

Results: Results showed that after using the Selvit, an increase in TAC and a decrease in LPO were observed but they were not significant. Also, aspartate transaninase significantly decreased after using Selvit and high density lipoprotein significantly increased.

Conclusion: Selvit have beneficial effect on hematological parameters due to its antioxidant capacity.

Abstract

Background: Human &beta-defensin 126 (12kDa) is a small cationic glycoprotein that is highly rich of cysteine. DEFB126 gene is located on the subtelomeric end of 20p1.3 in human. High expression of this protein is reported in epididymis. This polypeptide coats the plasma membrane of sperm during epididy‌mal transit. It is likely that &beta -defensin 126 might have role in unexplained male infertility since it involves in sperm maturation and capacitation. The current research designed to investigate if there is relation between &beta-defensin 126 gene mutation and unexplained male infertility.

Materials and Methods: In this case-control study we followed a two cytosine nucleotides deletion of &beta-defensin 126 gene in 35 Iranian men with unexplained infertility and 40 fertile men with normal spermogeram as control group. Standard PCR, SSCP(Single strand conformational polymorphism), and sequencing were used to detect genetic alteration of &beta-defensin 126. ELISA was performed for the assessment of the protein expression on sperm cells.

Results: Analysis of genetic data revealed 28.6% homozygote deletion in unexplained infertile men while this deletion was detected in 7.5% of controls. The deletion frequency was statistically higher in infertile patients than normal control group (p<0.05). The protein expression was less in men with del/del genotype compare to the other genotypes (p<0.005).

Conclusion: Our study shows that this common sequence variation of &beta-defensin 126 takes part in impairment of male reproductive function. Consequently, men with the del/del genotype are significantly less fertile than men who carry the wild type allele.

Abstract

Background: Nickel and chromium are heavy metals whieh after arriving in biological systems can produce highly reactive free radicals in the body and interact with macromolecules cause damage to them. The purpose of this study, the effect of bilberry and cinnamon extract on blood biochemical factors in workers exposed to nickel and chromium.

Materials and Methods: In this clinical trial, 99 workers were divided into 3 groups: group 1 (control), group 2 workers, welders exposed to nickel and chromium without extract (G) and group 3 workers, welders exposed to nickel and chromium with extracts (B). First biochemical parameters were measured in blood serum. Then the group 3, 2 times a day for four weeks, billberry juice and tea - Cinnamon received. Finally, Biochemical parameters in blood were measured and compared. SPSS software and descriptive statistics (mean±SD), T- independent and one-way ANOVA test were used to analyze the data.

Results: Between group Variance in some biochemical variables between the three groups, control, G and B showed statistically significant differences (p<0.05). Within-group variance was significant in some variables.

Conclusion: After consumption of billberry juice and tea-cinnamon positive changes in biochemical parameters in workers who are exposed to the toxic effects of nickel and chromium was obtained.

Abstract

Background: Heart failure with normal ejection fraction (HFNEF) is commonly seen in adults. It is possible that old patients with HFNEF have severe clinical status. The aim of current study is to evaluate clinical and echocardiographic findings in old patients compared to young patients with HFNEF.

Materials and Methods: In this cross-sectional analytical study, 126 patients with HFNEF were evaluated in two groups of patients &ge60 years old (n=52) and <60 years old (n=74) for demographic, clinical and echocardiographic findings.

Results: Patients &ge60 years old had significantly more hyperlipidemia, less hypertension and more FC II. Patients &ge60 years old also had significantly larger septal wall thickness, lower end diastolic and systolic volume, end systolic diameter, E/A ratio and septal E’, lower diastolic dysfunction grade, higher left ventricle ejection fraction, left atrial volume index, E/E’ ratio and deceleration time compared to patients <60 years old.

Conclusion: Old patients with HFNEF have different clinical and echocardiographic findings compared to younger patients which are indicative of the disease severity in some cases. So, exact evaluation of the patients could be helpful in early diagnosis of these patients and providing an adequate treatment.

Abstract

Abstract
Background: Oxidative Stress and mitochondrial dysfunction leading to apoptotic death of neurons play key role in the pathogenesis of the Parkinson's disease. On the other hand، misregulation of microRNAs can cause several neurodegenerative diseases. Sirt1 and Bcl2 as two key genes, regulate pathogenic processes in neurodegenerative diseases such as mitochondrial dyfunction, oxidative stress and apoptosis.
 Materials and Methods: To investigate the role of microRNAs in model of Parkinson's disease, miRWalk 2.0 and TargetScan (v7) databases were served to predict microRNA-target interactions.
Results: Possible targeting effects of different microRNAs on Bcl2 and Sirt1 genes in Rat organism were analyzed. Merging data from databases has shown that rno-miR-449a, rno-miR-182, rno-miR-211, rno-miR-34b, rno-miR-34c, rno-miR-448, rno-miR-466b and rno-miR-96 with strong possibility can inhibit expression of Bcl2 gene. Also, rno-miR-181, rno-miR-211, rno-miR-27a, rno-miR-449a, rno-miR-34c, rno-miR-30, rno-miR-200a and rno-miR-448can inhibit Sirt1 gene with high possibility.
Conclusion: According to the findings, it can be predicted that regarding to high interaction scores of rno-miR-211, rno-miR-34c and rno-miR-448 and 449awith Bcl2 and Sirt1 genes in above-mentioned databases, these microRNAs probably can have critical role in disease process. Thus, these microRNAs can be introduced as appropriate candidates for investigations in in vitro model of Parkinson's disease.

Abstract

Background and Aim We need the next-generation of whole-inactivated influenza vaccines to create stronger cross-protection against different influenza subtypes. This research aimed to apply the inactivation process of the influenza virus through gamma radiation as a candidate for the development of whole-inactivated vaccines.
Methods and Materials The influenza virus strain A/PR/8/34 (A/Puerto Rico/8/34 [H1N1]) was propagated in Madin-Darby Canine Kidney (MDCK) epithelial cells. After ultrafiltration, the virus infectivity titer was calculated by 50% Tissue Culture Infectious Dose (TCID 50%) method based on the Karber formula. Alternatively, the gamma cell-220 was applied for virus inactivation via gamma rays. The D10 value factor and optimum dose of virus inactivation were calculated based on the dose/survival curve and the initial viral titer. In addition, antigenic properties of irradiated viruses compared to un-irradiated viruses and complete inactivation of the irradiated samples with optimum dose were also evaluated by hemagglutination assay and safety test, respectively.
Ethical Considerations The Research Ethics Committee of Islamic Azad University, Tehran Medical Branch, Iran approved this study (Code: IR.IAU.TMU.REC.1397.309).
Results According to the concentrated virus titer (TCID50: 105.75/ml) and dose/survival curve, the D10 value factor and optimum dose of virus inactivation were calculated at 4.878 and 28.048 kGy, respectively. On the other hand, owing to the results obtained from the safety test and hemagglutination assay, the optimum dose of virus inactivation was determined to be 28 kGy by maintaining the antigenic properties.
Conclusion Gamma radiation appears to be a good candidate for vaccine development through maintaining the antigenic structures.

Abstract

Background and Aim: Diabetes mellitus is one of the leading causes of death, and its prevalence is increasing annually because of the change in lifestyle. Increased blood glucose level and dyslipidemia are the major symptoms of this metabolic disease. Currently, the main and effective treatment for diabetes is the use of medication such as insulin. Its control by using herbal products has received a lot of attention in the world. The aim of this study is to evaluate the effects of bioflora (a probiotic supplement) and aqueous extract of cinnamon in improvement of blood glucose level, dyslipidemia, and reduction of cardiovascular diseases in diabetic rats. 
Methods & Materials: Thirty-five male Wistar rats were prepared and randomly divided into five groups of negative control, positive control, probiotic (treated with bioflora 3.2×108 CFUs/day for 30 days), cinnamon (treated with 200 mg/kg of cinnamon aqueous extract for 30 days), and probiotic + cinnamon. Diabetes was induced by intra-peritoneally injection of streptozotocin. The rats’ weight, blood glucose level, lipid profile, high sensitivity C-Reactive Protein (hs-CRP) and Atherogenic Index (AI) were measured at the end of experiment to predict the risk of cardiovascular diseases.  
Ethical Considerations: This study ethically approved in Research Centre of Islamic Azad University of Arak (Code: IR.IAU.ARAKREC1397.005). All interventions performed in accordance with the Guide for Care and Use of Laboratory Animals prepared by the Institute of Laboratory Animal Resources.
Results: Induction of diabetes caused severe weight lost in rats, but the weight loss was lower in groups treated with probiotic. The blood glucose level in probiotic + cinnamon group was significantly reduced. Bioflora reduced triglyceride, cholesterol, and Low-Density Lipoprotein Cholesterol (LDL-C) levels, while cinnamon extract significantly reduced triglyceride and LDL-C levels compared to the positive control group. AI and hs-CRP values were reduced in the probiotic group compared to control groups. There was no synergistic effect by combined use of bioflora and cinnamon extract. 
Conclusion: Bioflora effectively prevented dyslipidemia by improving intestinal microbiota, lowering blood glucose level, and improving lipid profile and, therefore, reduced the risk of cardiovascular diseases.
