Abstract

Background: Hospital readmission rate as a global health problem has increased over the last few decades. Congestive heart failure (CHF) is one of the most common causes of hospital readmission. Little is known about barriers of readmission rate control. The aim of this study was used for data exploring the barriers of readmission rate control. Methods and Materials: A qualitative study was designed with grounded theory approach. Data were gathered with interview. This study was started with proposly sampling and continues with theoretical sampling during 6 months. A convenience sample of 42 people was recruited from two public educational hospitals in Tehran during 6 month. The data were analyzed using constant comparative analysis. Results: Data analysis demonstrated lake of sensitivity on barriers of readmission incorrect patient’s health believes and expectations insufficient patient education, drug and diet adherence, and incorrect life style lack of active medical system , distrust on physicians and lack of communication psychological issues and patient - family challenges, were as the barriers of readmission control. Conclusion: This research had shown that the barriers of readmission control recognized by applying Qualitative research method with source triangulation better than the biomedical approach with single source. On the other hand, lack of sensitivity of partcipants on barriers of readmission control is a new finding. So, for controlling the barriers of readmission despite of exploring the barriers, making sensitive of involved people to readmission, is also proposed

Abstract

Background: Although miR-124 molecule has been known as an inducer of neurogenesis, few researches have been done on the targets of this molecule and its functional mechanisms in differentiation toward neurons and maintaining neuronal state. The microarray technique has been established as the reference method for studying the genes under the control of miRNAs. However, the high cost of this method has hampered its use in most research centers. On the other hand, the improvement of bioinformatical algorithms and computer modeling systems has led to the development of the bioinformatical softwares that can predict mRNA targets for miRNAs. Therefore, the aim of this theoretical study was to bioinformatically evaluate the effect of miR-124 on transcription factors that can be involved in neurogenesis and neuronal cell amplification, by using various specific softwares. Materials and Methods: Using different algorithms in TargetScan, DIANA and miRWalk databases, the potential transcription factors targets of miR-124 were identified. Then, a score table was prepared from the candidate genes, based on the affinity of the seed region of miR-124 and the number of targets in the 3`-UTR region of transcription factors. Finally, transcription factors with higher scores were chosen as candidates for practical analysis.

Results: The results of bioinformaical analysis showed that the LAMC1, ITGB1, PTBP1, SOX9, SP1, and EFNB1 molecules are the most potential factors that might be affected by miR-124 during neurogenesis.

Conclusion: It seems that transcription factor SP1 is under the control of the miR-124 and plays a crucial role in neurogenesis process. Therefore, this protein can be considered as a suitable new candidate for experimental evaluation.

Abstract

Background and Aim: The main objective of the current study was to compare the effectiveness of schema therapy and dialectical behavior therapy (DBT) on health-promoting lifestyle (HPL) and quality of life (QoL) of HIV/AIDS patients. 
Methods & Materials: The current study was an experimental study based on a pre-test-post-test design with a control group, and based on the objectives, it can be considered an applied study. The statistical population of the study included all HIV patients monitored by behavioral disorders clinics of the Healthcare Organization Centers in Hamedan County in Iran in 2019-2020. Using simple random sampling method, 45 patients were randomly selected from Shohada Clinic in Hamedan as the sample of the study, followed by randomly assigning them to three 15-member groups, i.e., two intervention groups and a control group. In order to measure the research variables, the Health-Promoting Lifestyle Profile (HPLP) (1987) and the Short Form (36) Health Survey (SF-36, 2001) were utilized. The obtained data were analyzed using ANCOVA in SPSS v. 12.
Ethical Considerations: The Ethics Committee of Hamedan University of Medical Sciences approved the study (Code: IR.UMSHA.REC.1398.1063). This study has been registered in the Iranian Registry of Clinical Trials (Code: IRCT20120215009014N348).
Results: The results of ANCOVA indicate that schema therapy and DBT affect HPL and quality of life, and there was a significant difference between the two groups (P‌<‌0.001). 
Conclusion: The results show that while both therapy methods affect the selected variables, schema therapy is more effective than DBT on HPL and Quality of Life (QoL).
