Abstract
Introduction: While elderly people enter the hospital in biological crisis, family caregivers enter the hospital in psychological crisis. But professional caregivers haven’t adequate knowledge of family caregivers' experiences. Therefore they forget that family caregivers are in suffering equally. Since suffering is a significant concept in nursing science and knowing and comprehending sources of suffering in family caregivers by professional caregivers is very important, this study is done to describe the nature and structure of sources of suffering in family caregivers of hospitalized elderly patients. Materials and Methods: This is a qualitative study with phenomenological method. Population of the research is all elderly patients' families that were hospitalized in one of the hospitals of Isfahan University of medical sciences. Sampling method was purposive and continued until data saturation. The numbers of participants are 12. Data were gathered through in-dept interview and data analysis done by “Parse” method that was not used in Iran. Results: The finding of this research is summarized in these core concepts: Patients and therapeutic interventions as source of suffering, hospital as source of suffering, disturbance in family and social life as source of suffering, self neglect as source of suffering. These concepts will be discussed in the full article. Conclusion: Participants' experiences reflect the need for planning interventions such as developing supportive groups, providing facility and instruments and continuous support for family caregivers during hospital stay. Also results show that educational programs are necessary for effective adaptation in family caregivers.

Abstract
Introduction: Prolonged labor is an important problem during labor which can have undesirable effects on both maternal and fetal health. Shortening the duration of labor has always been one of the aims of obstetricians. Hence, the purpose of this research was to investigate the effect of massage on the duration of labor in women attending to Bentolhoda Maternity Unit in the city of Bojnord, Northern Khorasan, Iran in 2005. Materials and Methods: In this clinical tial study, 62 primiparous women with normal pregnancy were participated. The indications of normal pregnancy included a gestational age of 37-42 weeks, single tone, and vertex presentation. Sampling was done by purposive method. Women were randomly allocated in two groups. The experimental group (n=32) received a 30-minute directional, reasonably firm and rhythmic effleurage massage in the back in each of the three phases of labor. The control group (n=30), did not receive any form of massage and received a routine 30-minutes nursing care in each of the three phases. Measurements were then made for strength, interval and frequency of contractions and the duration of each phase. Data were analyzed using student t, Chi-square and Mann-Whitney tests. Results: Findings showed a significant decrease in the duration of labor after massage sessions. The mean duration of labor for the massage group was 5.75 hourse and for the routine nursing care group 9.15 houres (p=0.001). Conclusion: According to the above results, there was a significant decrease in duration of the first phase of labor in pregnant women receiving massage sessions. Therefore, this method can be used to decrease the period of hospitalization.

Abstract
Background: Hemorrhoidectomy is the basis of treatment for grades three and four hemorrhoid. One of the major post hemorrhoidectomy problems is pain that is usually due to the spasm of the internal sphincter. There are different methods for the management of postoperative hemorrhoidectomy pain. The aim of this study was to investigate the effect of sphincterotomy on post hemorrhoidectomy pain.
Materials and Methods: This interventional clinical trial was conducted on 60 patients with 3rd and 4th grade hemorrhoid who had randomly been divided into hemorrhoidectomy and hemorrhoidectomy combined with sphincterotomy groups. The degree of pain was measured on visual analogue scale (VAS) and analyzed through Mann-Whitney, Friedman, and Chi square tests via SPSS version 14.
Results: In terms of reduction in pain with sphincterotomy, there was a significant relationship between the two groups on the first, second, and fifth postoperative days (P=0.001). On the third and sixth postoperative days, a significant relationship was observed between the two groups (P=0.001). Days four and seven also indicated a significant relationship between the two groups (P=0.002). However, there were not any significant relationships between the two groups in terms of urinary and fecal incontinence and gas retention (P>0.05). A significant relationship existed between the two groups in terms of reduction in the need for sedation (P=0.006).
Conclusion: Lateral internal sphincterotomy combined with hemorrhoidectomy can reduce the patients’ post hemorrhoidectomy pain and can reduce the use of analgesics, but it does not have any significant influences on the incidence of urinary retention and bleeding after the first defecation and does not increase the risk of fecal and gas incontinence in comparison with the hemorrhoidectomy group.

Abstract
Background: The effects of acute and chronic exercises on insulin resistance index may be related to one or more factors, including morphological changes and hormonal modifications. The purpose of this study was to investigate the effects of a single session of resistance training on adiponectin levels and insulin resistance until 24 hours post-exercise.
Materials and Methods: This experimental study, which followed a pretest-posttest design, was conducted on a group of 10 healthy male volunteers (23±1.4 years) with no history of participation in any regular exercise programs, weight changes more than 2 kg, special diseases, and smoking over the past 6 months that had normal body mass index (BMI) (23.7±1.6 kg.m-2). The subjects performed a single session resistance training (3 sets of 10 repetitions at 75% of 1 repetition maximum). Adiponectin, glucose, insulin, and insulin resistance index levels were measured before and 24 hours after the exercise.
Results: There were no significant differences for adiponectin (μ g.ml-1) pre- (6.98±1.9) and post-exercise (8.07±1.4) and glucose (mg.dl-1) pre- (81.3±7.6) and post-exercise (80.7±6.4). However, insulin resistance index pre- (1.34±0.27) and post-exercise (1.06±0.11) and insulin concentration (UΙμ.ml-1) pre-(6.64±0.88) and post-exercise (5.37±0.43) decreased significantly 24 hours post-exercise (P<0.05).
Conclusion: Based on the findings of this study, it can be suggested that a single session of resistance training with no significant changes in adiponectin level can have positive effects on glycemic indices in sedentary men.

Abstract
Background: Cutaneous leishmaniasis is a health problem in many areas of Iran. Cutaneous leishmaniasisis reported mostly in the central county of Qom province, including Ghanavat and Qomrood villages. This study was done to identify parasite species in human and rodents in order to illustrate the epidemiologic picture of the disease and provide an appropriate control program in 2010 Materials and Methods: This cross-sectional study was done on microscopic smears of 45 human samples and rodents samples hunted around Ghanavat and Qomrood villages in the central county of Qom province in 2010.In total,15 human samples and one hunted rodent sample were positive. In this study,the DNA of the parasites were extracted from the slides and analyzed by Leishmania specific premiers using ITS1 PCR-amplification (Internal Transcribed Spacer1). PCR (PolymeraseChain Reaction) products were digested by Haelll enzyme. Results: Overall, 15 human samples and one rodent sample from Merioneslibycus species were evaluated by PCR-RFLP (Restriction Fragment Length Polymorphism). After electrophoresis, it was demonstrated that the parasite was Leishmaniamajor in both human and rodent species. Conclusion: PCR-RFLP technique is an effective method to determine Leishmania parasite species in Geimsa stained slides from human and rodent reservoirs. One of the advantages of this technique is that it is possible to recognize the pathogen species of Leishmania parasite without gene sequencing. Besides, PCR-RFLP technique is a method of quite high sensitivity and specificity which can identify parasite species in addition to the diagnosis of leishmaniasis within 24 hours.

Abstract
Background: Protein-energy malnutrition is regarded as one of the public health problems in developing countries as a result of poor feeding due to poverty. This study was conducted to compare protein quality of two samples of commercial weaning food, Cerelac (based on dry milk, wheat and banana containing probiotic Bifidobactriumlactis) and Ghoncheh (based on dry milk, wheat, and honey), in rats. Materials and Methods: This experimental study was conducted on 64 male rats aged 23 days in 8 groups under 8 diets, including 2 test diets (Cerelaccontaining probiotic Bifidobactriumlactis and Ghoncheh), 1 standard diet (casein), 1 basal diet (protein free) for true protein digestibility and apparent digestibility study, 2 test diets, 1 standard diet, and 1 basal diet for net protein ratio, protein efficiency ratio, and food efficiency ratio study. Results: The contents of true protein digestibility for casein, Cerelac, and Ghoncheh were 93.77, 84.23 and 89.82, respectively and the results were significant in all of the groups (p<0.001). The content of net protein ratio for casein, Cerelac, and Ghoncheh was 4.38, 4.1 and 3.17, respectively and the results were significantin all of the groups (p=0.009). The contents of protein efficiency ratio for casein, Cerelac, and Ghoncheh were 3.05, 2.59, and 2.01, respectively and the results were significant in all of the groups (p<0.001). Conclusion: The findings of this study indicated that the protein value of Cerelaccontaining Bifidobactriumlactis was higher than Ghoncheh.

Abstract
Background: Psoriasis is a common chronic skin disease which usually appears as white and silvery spots on the skin. The purpose of this study is to investigate the effect of berberine on patients with psoriasis. Materials and Methods: This double-blind clinical trial was conducted on 60 psoriasis patients who referred to Imam Khomeini Hospital, Ahvaz, between July and December 2011. The patients were randomly divided into two equal groups that applied berberine and placebo to the treatment of their lesions twice a day. Lesions severity rate in the patients was assessed 7 times using a Psoriasis Area Severity Index (PASI). Data were analyzed by ANOVA with repeated measurements and Chi-square using SPSS software. Results: After 6 months, severity of lesions showed a significant decrease in the group receiving the berberine cream compared with the placebo group so that the mean PASI score of the group receiving berberine decreased from 3.99 to 2.11 while in the placebo group this decreased from 3.98 to 3.71. Mean difference in PASI score was not significant before receiving the treatment till the third month in berberine and placebo groups, but in the fourth to sixth months it was statistically significant (p=0.013). Conclusion: Berberine cream was more effective than the placebo in reduction of skin lesions of psoriasis patients. Therefore, it is recommended that plant-based drugs, like berberine, be used more besides the main medications of patients.

Abstract
Background: The average age of menarche varies from country to country due to regional variation and different factors, including genetic and environmental factors that affect the onset of menarch. This study performed to survey relationship between nutrition and physical activity with menarche age of girls in Ahvaz city.
Materials and Methods: This survey is a cross-sectional study that was conducted on 1249 female students 8-15 years in 2012- 2013 year. Participants were selected by random cluster sampling from 4areas and 16 elementry and middle and high schoolsin Ahvaz city. The data were collected based on interviews and questionnaire (food frequency and physical activity). After collecting the data, then they were encoded and analyzed by SPSS16 software and descriptive and analytic statistics.
Results: The mean of age at menarche in girls in Ahvaz was 11.86±1.07 and age at menarche was not associated with nutrition and physical activity.
Conclusion: Based on these findings, significant association between nutrition and physical activity and age at menarche (p<0.001)was not observed.

Abstract
Background: Hypertrophic cardiomyopathy (HCM) is a various collection of heart diseases with autosomal dominant inheritance affecting 0.2% of the global population. HCM is also the most common cause of sudden cardiac death in individuals younger than 35 years old. Approximately, 40% of affected cases are associated with MYBPC3 gene. The aim of this study is to investigate the possible presence of mutation in 15 and 18 exons of MYBPC3 gene in patients with HCM in Chaharmahal Va Bakhtiyari province.
Materials and Methods: In this experimental study, 30 HCM patients were selected. DNA was extracted using standard phenol-chloroform method. Certain exons were amplified by PCR method. And then, SSCP and HA methods were run.
Results: Significant differences were observed between the positive control samples and other samples. However, there were no difference in studied exons or shift in the bands.
Conclusion: Mutations in the exons of MYBPC3 gene may cause the HCM disease, and change in other exons may be the causative agent in this geographical region and change in this studied exons may not have contributed to the HCM disease. However, it is necessary to study more patients for getting a better conclusion.

Abstract
Background: ALS is a progressive neuro-muscular disease, which is characterized by motor neuron loss in the Central Nervous System (CNS) and Peripheral Nervous System (PNS). Up to now, no accurate clinical method for diagnosis of the disease have been provided. In most cases, ALS patients are unable to walk normally due to abnormalities in the nervous system. For this reason, one of the most appropriate methods in the diagnosis of ALS from other neurological diseases or from healthy volunteers is the gait motor signal analysis.
Materials and Methods: In this study, gait signals available in Physionet database have been used. The database consists of 13 patients with ALS (ALS1, ALS2, …, ALS13) and 16 normal subjects (CO1, CO2, …, CO16). The patients participating in this study had no history of any psychiatric disorders and did not use any assistive device for walking, like wheelchair. The power spectrum of stride, swing, and stance of normal subjects and patients was computed for both left and right legs. To provide appropriate inputs for the classifier, the frequency band of the power spectrum of all signals was divided into eight equal parts. The area of all regions was computed. Three frequency band of the lower range of power spectra selected as inputs of the classifier.
Results: In this study, power spectra, as frequency attributes, were used to explore probable differences of time series in both patients and healthy subjects.
Conclusion: Artificial Neural Network was used to classify normal and ALS groups with the accuracy of 83% for the test data set. It seems that the present algorithm can be used in discriminating patients from normal subjects in the early stages of the disease.

Abstract
Background: Family is the most important component of community and family health leads to spiritual excellence and intellectual growth. One of the debates in current society is to enhance the bases of marital life. More than half of the sexual problems which cause family life and marital relationships abolishment are due to insufficient knowledge or erroneous beliefs about sexual relationships. The aim of this study is to investigate the effect of acceptance commitment therapy (ACT)-based premarital counseling on the couples’ sexual function.
Materials and Methods: The study population of this clinical trial consisted of all couples referring to premarital counseling centers of Isfahan in 2015. Sixty couples were selected and then randomly divided into two groups, an experimental group(30 couples) and a control group(30 couples). Then they completed a questionnaire of sexual function. The data were analyzed by SPSS 21 using the Mann-Whitney test.
Conclusion: The results of this study with the aforementioned condition showed the lack of significant difference in the couple’s sexual function between premarital counseling and the approach based on acceptance and commitment. So, it seems that performing a research with more samples and a target group having 1-5 marital experience would be necessary.
Results: The results showed no significant difference between the effect of premarital counseling acceptance and commitment therapy – based approach on the couples’ sexual fanction in comparison with a premarital conventional approach(p>0.02).

Abstract
Abstract
Background: The aim of this study was to investigate the acute effect of supplementation Jadvar on aspartate aminotransferase (AST) and alanine aminotransferase (ALT) enzymes after exhaustive incremental exercise in men's handball.
Materials and Methods: In this semi-experimental double blinded study 12 handball players with at least 2 years record in league (with average age=21.42, height=186cm, weight=83.25, and body mass index=24.09kg/m2) divided into 2 groups (n=6) accidentally. Both groups performed maximum Bruce protocol until exhausting level. Before (fasting) and after performing protocol ,blood samples were taken from middle forearm vein and transferred to lab then supplementation group consumed three 500 mg jadvar capsules and control group consumed three 500 mg maltodextrin for 7 days daily. 24 hours after eating last capsule ,they performed maximum Bruce protocol up to exhausting level and the samples were collected like primary protocol blood samples before (fasting)and after performing protocol and were transferred to the laboratory.
Results: The results of dependent and independent t-test showed that consuming jadvar supplements for 7days did not have significant effect on aspartate aminotransferase(AST) and alanine aminotransferase(ALT)(p≥0.05).But alanine aminotransferase in supplementation group (16.83 mg.dl-1) decreased in comparison to placebo group (20.5 mg.dl-1). Also, aspartate aminotransferase was increased from 28 mg.dl-1 to 35.17 mg.dl-1 , but neither decrease nor increase was not meaningful (p≥0.05).
Conclusion: It seems that acute consuming of jadvar supplement and one session incremental exercise does not have meaningful effect on AST and ALT in man handball players.

Abstract
Background and Aim: Extensive prevalence of myocardial infraction, and an increase of 36% in total deaths due to it by 2020, and attention to the causes and methods of alternative therapy is very important. There are different ways in treating these patients that endurance training is one of them. The aim of this study was to evaluate the effect of 8 weeks of incremental endurance training on serum levels of TSP-1 and MMP-1 in male Wistar rats with myocardial infarction.
Materials and Methods: In this controled experimental study with control group, 20 rats weighing 230 ±30 g (8-week-old) were randomly divided into incremental endurance training and control, after induction of infarction with Isoproteronol (150 mg/kg). The training group performed an 8-week training session (3 sessions a week, for 20 to 50 minutes at 12 to 18 meters per minute).Then, 24 hours later, the venous blood sample was collected to evaluate serum concentrations of TSP-1 and MMP-1 and transferred to the laboratory. Independent t-test was used to analyze the data at a significant level of p <0.05 to Graphed Pad software.
Ethical Considerations: This study with research ethics code IR.IAU.ARAK.REC.1397.007 was approved in Research Ethics Committee of Islamic Azad University, Arak branch, Iran.
Findings: The results showed that endurance training significantly increased serum MMP1 levels (p = 0.048) in rats with myocardial infraction, while there was no significant effect (p = 0.092) on the exercise program in serum TSP-1 levels.
Conclusion: the result of this study suggests that increasing endurance training may increase MMP-1 in rats with myocardial infraction, which can play an important role in angiogenesis and replacement of capillaries.

Abstract
Background and Aim: The adherence of diabetic patients to the recommended treatment regimens is among the major challenges in diabetes control. Dietary adherence is low in pregnant women with gestational diabetes. Non-compliance can reduce the beneficial effects of treatment, symptoms, and signs, increase complications, or even cause death. Therefore, further research on the determinants of follow-up behaviors is essential.
Methods & Materials: This was a descriptive-analytical study. In total, 260 women with gestational diabetes treated with diet referring to healthcare centers, and Imam Reza (AS), Ghaem, and Umm Al-Banin Hospitals in Mashhad City, Iran participated in this research. The study subjects were selected using convenience, multistage, class, and cluster sampling methods. The required data were collected using a demographic and pregnancy profile questionnaire and a researcher-made questionnaire of adherence to the treatment regimen. The obtained data were analyzed by inferential statists and regression tests in SPSS v. 16.
Ethical Considerations: This study was registered with ethics code of IR.MUMS.NURSE.REC.1397.091 in the ethics committee of Mashhad University of Medical Sciences.
Results: The regression analysis results indicated that dietary adherence, blood glucose monitoring, and exercise adherence scores presented a significant and positive effect on the total treatment adherence score (P<0.001). Furthermore, education (P=0.044), unwanted (P=0.006), or unplanned pregnancy (P=0.21) provided a positive and significant effect on the overall score of treatment adherence. Women with a history of diabetes in first-degree relatives (P=0.010) and those with a history of miscarriage (P=0.021) obtained higher scores of total treatment adherence.
Conclusion: According to the present study findings, some demographic and pregnancy characteristics can affect the rate of adherence to the treatment regimen in women with gestational diabetes. Healthcare providers need to pay attention to the impact of these factors in improving adherence to patients’ treatment regimens.
